This document is a release form allowing Priestley College to use photographs or recordings of an individual. It gives the college permission to use the materials for marketing, publicity or other electronic/printed publications. The individual understands the college will own copyright of the materials and can use them as they wish or in combination with other content. While the individual can request to no longer have their image used in the future, they acknowledge materials already published cannot be recalled.

I hereby give Priestley College the right to use (delete as appropriate)

I understand the material(s) will be the copyright of Priestley College and may be used as is or in
conjunction with other material(s), as part of a composite production.
I understand that I have the right to ask the College to stop using my image or photograph(s) in any
future publications. However, I acknowledge that the College cannot recall publications in which my
image has already appeared.
