This document summarizes the investigation into the death of musician Simon P. Ano. Ano had been left quadriplegic after an accident but had recently found a way to create music again. He was found dead with blue fingernails and lips after his Baclofen pump, which treated his spasticity, had been beeping. The preliminary findings revealed that Ano had been depressed and that his wife and a male associate were romantically involved. The investigation uncovered that the associate, who also suffered from bipolar disorder, may have been jealous of Ano's musical success and reprogrammed his pump to deliver a fatal dose of medication.
