MID SEMESTER ACADEMIC PROGRESS REPORT

For each class you are enrolled in this semester, ask your instructor to complete a Mid Semester
Progress Report. Follow the steps below for completing the reports:
1. Write a course name and number of units in the first two columns. You will need one
report for each class you are taking. 

2. Give one form to each instructor. You are encouraged to use the instructor’s current
office hours to complete this report. We do not recommend approaching the instructor
before or after class if a discussion of academic progress is desired. 

3. If you are enrolled in an online class or self-paced class, you can send an email version
of the mid semester report or ask to speak to the supervisor of your class. 

4. Once your instructor has completed the form, bring it to a counselor to review & to
discuss a plan of action.
5. You must complete a mid semester academic progress report for each class you are
