This document is a membership application for the Lonestar Western Re-enactment Group. It requests basic contact information as well as information on the section and activities the applicant wishes to participate in. Applicants must include photos and agree to abide by the group's rules and assume liability. Membership is renewed annually and requires upholding authentic practices in dress, behavior and equipment during living history displays.

Fall 2014 Soccer Program Registration Formcarol_st_pats
Events will be held weekly Saturday mornings at Cactus Park (7202 E Cactus Rd.) from 9am-10:30am September 20th through December 13th (with the exception of November 29th). Entry fee will include all instruction and a club t-shirt. You will need to make sure your child has shin guards, shoes(soccer cleats preferred) and a soccer ball. Kids 5-8 should have a size 3 ball. Kids 9-11 should have a size 4 ball.

1 .. I acknow ledge that the pyrotechnic effects and weaponry used in the course of the Group’s re-enactments are noisy and capable of inflicting serious
injury. As a condition of joining the Lonestar Western Re-enactment Group, I agree to be bound by its rules, and the bye-law sand codes of conduct and
policies of the Group. I understand and accept that neither the organisers, nor anyone w hose name appears in connection w ith any event organised by
or attended by members of the Group nor the Group or its Officials shallbe liable for any injury, loss or damage w hich I may sufferto my person and/or
my possessions, howeverthey may be caused, and w hether the same may occur before, during or after an event or re-enactment.
