3. Guidelines Simplified
• The association recently clarified the Medicare
guidelines for billing services provided by a qualified
non-physician practitioner (NPP) as “incident-to” a
physician in the same office.
In order to bill the encounter as “incident-to” and receive
correct payment for it through Medicare, the NPP must
meet certain strict criteria. The NPP must be licensed in
the state and be an employee of the physician or
physician group practice. The services the NPP provides
must be an integral part of and “incident-to” the
physician’s care of the patient. Lastly, the NPP must be
under the direct supervision of the physician when
providing these services.
