Sulzbacher Clinic Unveils New Name
JACKSONVILLE, FL (May 30, 2014) - On Thursday, June 5, 2014
the Sulzbacher Center, Northeast Florida's largest provider of
comprehensive services to homeless men, women and children,
will announce that its satellite clinic, Beaches Community Healthcare, will
begin operating under a new name. The clinic will now be known
as Sulzbacher Beaches Clinic. At the same time, the organization will
unveiled a new logo and redesigned medical services Web
page, http://sulzbachercenter.org/about-sulzbacher-center-medical. The
new logo will be unveiled at an "Open House" at the Sulzbacher Beaches
Clinic on Thursday, June 5, 2014 between 5:00 and 7:00 P.M.
President and CEO, Cindy Funkhouser, said "We are very excited to unveil
our new name and logo to the public. We believe the name and logo will
become synonymous with the organization's dedication to offering holistic,
patient care for under-insured, uninsured and homeless residents of the
Beaches community and our overall affiliation with Sulzbacher Center.
Since its inception in 2009, Sulzbacher Beaches Clinic has provided more
than 60,000 primary care visits, more than 10,000 dental services and
more than 6,000 behavioral health services. We hope our new name and
logo will help us increase awareness for the Clinic and serve more people
in the Beaches community."
Financial hardship is no longer a reason to be without quality health care.
Any adult residing in the service area and in need of medical care, will find
a Medical Home at the Sulzbacher Beaches Clinic. Sulzbacher Beaches is
a federally qualified health center for the Beaches community. The Clinic's
medical professionals are available five days a week to assist with medical,
dental and behavioral health needs.
Sulzbacher Beaches Clinic offers a wide array of services including:
• Primary health care with comprehensive medical assessment and
treatment for routine and chronic health conditions.
• Prescription Assistance Program where staff helps patients who
meet PAP guidelines access medications for a reduced price or
free of charge.

2. • Comprehensive dental care including fillings, periodontal surgery,
bridges*, crowns*, dentures* and oral surgery (*Note: Patients are
responsible for payment of all laboratory fees incurred during their
treatment)
• Behavioral Health Care and individualized treatment plans,
medication management, counseling and recovery support.
