It outlines the measures introduced by Ministry of Health to implement EHRs across the health sector in New Zealand. Furthermore, the review will provide an international comparison in implementing EHRs with that of New Zealand.

• La pressione arteriosa è la pressione che il sangue esercita
sulle pareti delle arterie del sistema cardiocircolatorio.
Essa dipende sia dalla quantità di sangue pompato dal
cuore,sia dalla resistenza opposta al flusso sanguigno.
• La pressione sale e scende seguendo le contrazioni del
cuore: è massima quando il cuore si contrae (sistole) per
spingere il sangue in circolo (pressione sistolica) ed è
minima quando il cuore si rilascia (diastole) per riempirsi
di sangue (pressione diastolica).
3. VALORIVALORI
• L’unità di misura della pressione arteriosa è il millimetro di
mercurio (mmHg).
• Parliamo di ipertensione (pressione alta) quando i valori
abituali di pressione sono uguali o superano i 90 mmHg di
minima e/o i 140 mmHg di massima.
• Parliamo di ipotensione (pressione bassa) quando i valori
abituali di pressione sono uguali o inferiori i 60 mmHg di
minima e/o i 90 mmHg di massima.
4. PERCHÉ MISURARE LA PRESSIONE?PERCHÉ MISURARE LA PRESSIONE?
• La maggior parte degli ipertesi non avverte particolari
sintomi. Il solo modo per sapere se si è ipertesi consiste
nel sottoporsi periodicamente a dei controlli. I dati
ottenuti in singola misurazione vanno interpretati con
cautela:la pressione arteriosa è infatti soggetta a
oscillazioni. Per questo la diagnosi di ipertensione richiede
numerose misurazioni effettuate nel corso di diverse visite
mediche.
6. REGOLE GENERALI PER UNA CORRETTAREGOLE GENERALI PER UNA CORRETTA
MISURAZIONE DELLA PRESSIONE ARTERIOSAMISURAZIONE DELLA PRESSIONE ARTERIOSA
• La pressione arteriosa viene comunemente misurata al
livello dell’arteria del braccio (arteria omerale).
• Chi si sottopone alla misurazione deve essere sdraiato o
seduto da almeno 5-10 minuti,con il braccio libero da
indumenti,appoggiato su una superficie piana. Se si è
seduti,il braccio deve essere all’altezza del cuore ed il
palmo rivolto verso l’alto.
• La prima volta la pressione va misurata in ambedue le
braccia ed annotata. In caso di differenza significativa tra i
due arti,le successive misurazioni andranno effettuate sul
braccio con pressione più elevata.
7. • La misurazione può essere manuale o automatica.
• Nel primo caso bisogna impiegare un fonendoscopio,cioè
uno strumento che permette di udire i rumori che vengono
generati dal passaggio del sangue nell’arteria del braccio e
che di norma viene utilizzato per ascoltare i battiti del cuore.
• È opportuno ripetere la misurazione,calcolare la media delle
due misurazioni e di essa tenere conto. Prima di eseguire la
successiva misurazione,è necessario sgonfiare
completamente il bracciale ed attendere 2-3 minuti per
lasciare defluire il sangue.
• Il soggetto deve essere rilassato,in posizione comoda,in un
ambiente tranquillo,a temperatura confortevole. È
necessario evitare di misurare la pressione subito dopo
l’ingestione di pasti,o dopo aver fatto uno sforzo fisico o
avere subito una forte emozione. Non bisogna aver fumato
nei 15-30 minuti precedenti,né aver assunto caffè o altre
bevande contenenti caffeina.
8. TECNICA DI ESECUZIONETECNICA DI ESECUZIONE
• Lo strumento da utilizzare si chiama Sfigmomanometro: è
costituito da un bracciale che viene avvolto attorno al
braccio del soggetto e mantenuto all’altezza del cuore.
• Il bracciale deve essere avvolto tra l’ascella e la piega del
gomito;in corrispondenza di quest’ultima va appoggiato il
Fonendoscopio,nel punto in cui appoggiando le dita si sente
pulsare l’arteria del braccio. Contemporaneamente,si palpa
il polso radiale,cioè la pulsazione dell’arteria che passa al
livello del polso,dallo stesso lato in cui si trova il pollice.
9. • A questo punto il bracciale viene gonfiato sino alla scomparsa sia
dei rumori provenienti dal fonendoscopio che del polso radiale:in
questo momento la pressione del bracciale è superiore alla
pressione arteriosa.
• Successivamente si riduce lentamente la pressione del
bracciale,facendo uscire l’aria in esso contenuta,azionando
l’apposita valvola. Quando la pressione sarà uguale a quella
arteriosa,un po’ di sangue riuscirà a passare nell’arteria
producendo un rumore:il primo rumore udito chiaramente
corrisponderà alla pressione sistolica (detta anche massima).
• Riducendo ulteriormente la pressione i rumori diventeranno
inizialmente più intensi,quindi via via più deboli:la completa
scomparsa dei rumori corrisponderà alla pressione diastolica
(detta anche minima).
• La pressione arteriosa,viene osservata sul manometro,che può
essere a colonna di mercurio o aneroide. Quest’ultimo deve
essere tarato almeno una volta all’anno.
10. CONCLUSIONICONCLUSIONI
• Negli ultimi anni si sono resi disponibili degli apparecchi per
l’automisurazione domiciliare della pressione arteriosa che
consentono una rilevazione automatica o semiautomatica e
che si basano prevalentemente su una tecnica oscillometrica
e con i quali i valori pressori vengono visualizzati su appositi
display.
• In alcune persone la rilevazione della pressione arteriosa da
parte del medico può causare un aumento dei valori
pressori dovuto ad una reazione emotiva (effetto da camice
bianco). Per questo la diagnosi di ipertensione richiede
numerose misurazioni effettuate nel corso di diverse visite
mediche; ripetendo più volte la misurazione, l'eventuale
reazione emotiva tende ad attenuarsi o a scomparire.
