No person may work in any unguarded area or place where the possibility exists that they may fall 6 feet
(1.8 meters) or more unless they are protected from injury by the use of fall protection equipment that is
connected to an approved anchor point

ENSURE 100% FALL PROTECTION IS ESTABLISHED - The lanyard or reel will be attached to a suitable anchorage at
all times while the person is in an exposed position. At times, this requirement may necessitate the use of 2 lanyards so
thatonecanbeleftattachedwhilethesecondisbeingmovedtothenextanchorage,otherwiseknownas100%tieoff

ENSURETHERE ARESUITABLE ANDPROPERLYPOSITIONEDANCHORAGEPOINTS-Anchor points serve as a
securepointofattachmentforlifelines,lanyards,ordecelerationdevices.
• An anchorage will be capable of supporting a minimum dead weight of 5,000 pounds (2268 kilograms) for each
personattachedtoit
• The anchorage for a personal fall-restraint system will support at least 3,000 pounds or be designed, installed and
usedunderthesupervisionofaqualifiedpersonandhaveasafetyfactorofatleasttwo
• Anchorages will be approved by a Competent Person to meet the strength requirements, or a recognized typical
structuralmember

INSPECT PERSONAL FALL PROTECTION EQUIPMENT– All equipment used for work at height
must be within a regime of inspection / maintenance as required under the manufacturer’s guidance
andlocallegislation(whereappropriate),andrecordsmaintainedfortheservicelifeoftheequipment.
Pre use and periodic inspections of fall protection equipment will be performed by the user and a
CompetentPersoninaccordancethemanufacturerecommendationsandinstructions.
• Pre-UseInspection
• Equipmentusersshallensurepre-useinspectioneverytimebeforeuse
• Equipment found to have missed a required, periodic inspection required to tagged “Danger -
Don’tUseIt–UnsafeTag”orsimilartaggingtoensuretheeffectivecommunication.
• Iftheequipmenthasadefect,itistaggedoutofserviceandwithdrawnfromuseimmediately
• PeriodicInspection
• Visualinspectionsshallbeperformedinaccordancewithmanufacturer’srequirements-atleast
once each quarter. Inspections are made by a Competent Person who examines for general
conditionanddamagebycuts,abrasions,andburns
• The inspection will be documented in accordance with manufacturer's
requirements/inspectionsonthemanufacturer-suppliedtag,alongwithappropriatecolorcode

LADDERS INSPECTED, PROPERLY POSITIONED, AND SECURED –
Whenworkingfromladders,fallprotectionwillbeusedwheneverworkingat
heightsof6feet(1.8meters)orabove,asmeasuredfromtheladderbaseto
the employee’s feet, unless it can be demonstrated that that it is infeasible
or it creates a greater hazard to install and use conventional fall protection
equipment
Laddersmustbe:
• Suitableandsufficientstrengthfortheirintendedpurpose
• Erectedastoavoiddisplacement
• Where used as a means of access between places of work, sufficiently
securedtopreventslippingand/orfalling.
• Erectedonsurfaces whichare stableandfirm,ofsufficientstrength,and
ofsuitablecompositionsafelytosupporttheladderanditsload
• Erectedonamaximumangleof75degreestothehorizontal(1:4)

WHEN USING LADDERS, ALWAYS MAINTAIN 3 POINTS OF CONTACT - When ascending, working from, or
descending aladder,personnelshallfacetheladderandhavebothhandsfreeforclimbing.Whenworkingfromaladder,
personnel shall maintain 3-point contact at all times and shall not lean past the side rails to reach the work (Belt buckle
shallremainbetweenladdersiderails).Theladdershallbemoved(unoccupied) toavoidoverreaching

ESTABLISH EXCLUSIVE ZONES TO KEEP PEOPE SAFE
FROM DROPPED OBJECTS - When work is being performed
above ground, where there is the potential for suspended loads,
tools, equipment, waste or other items to fall and create a risk to
workers or members of the public, an exclusive zone or drop zone
hastobeestablished.
Controlmeasuresforexclusivezones:
• Closure of pathways and pedestrian detour to keep
pedestriansoutofexclusivezone
• Use of traffic management and traffic control devices to
preventorcontrolvehicleentryintotheexclusivezone
• Use of physical barriers to clearly define and restrict entry to
theexclusivezone
• Supervision/Observationanduseofhighvisibilitywarningtape
toprevententryintotheexclusivezone

USE SAFETY NETS TO CATCH FALLING OBJECTS OR
PERSONNEL - Safety nets can be used for accidental fall of
people or objects from the site. Safety nets when installed,
must be close as possible under the walking/working surface
onwhichemployeesareworking,andextendbeyondtheedge
oftheworksurface
Safetynetcontrolmeasures:
• Install safety nets with enough clearance under them to
prevent contact with the surface or structures below when
subjectedtoanimpactforceequaltothedroptest
• Inspect safety nets at least once a week, and after any
occurrence which could affect the integrity of the system,
forwear,damage,anddeterioration
• Remove all materials, scrap, equipment, and tools that
have fallen into the net as soon as possible, but at least
beforethenextworkshift

USETOE-BOARDSTOREDUCETHERISKOFOBJECTS
BEING KICKED OFF PLATFORMS - Toe-boards shall be
erectedalongtheedgeofallworkingplatformstokeepsmall
orunstablematerialsfromfallingtothelowerworksurface
Toe-boardsmustbe:
• Capableofwithstanding,withoutfailure,aforceofatleast
50 pounds (22.7 kilograms) applied in any downward or
horizontaldirectionatanypointalongthetoeboard
• At least 3 1/2 inches (8.9 centimeters) high from the top
edge of the toe board to the level of the walking/working
surface
• Securely fastened in place at the outermost edge of the
platform and have no more than 1/4 inch (0.6
centimeters) clearance above the walking/working
surface
• Solidorwithopeningsnotover1inch(2.5centimeters)in
thegreatestdimension

USEOFTOOLLANYARDSTOPREVENTDROPPEDTOOLS -Thetoollanyard’spurposeistopreventtoolsfrom
falling and minimizing the risk to workers below when working at height by securing the tools to fixed points with a
carabiner.
Whenusingtoollanyards:
• Ensurethelanyardisdesignedtowithstandthearrestofthefallofthetoolbeingused
• Ensure personnel do not attach tool lanyards to their bodies or clothing when working near rotating or moving
equipment
• Ensureworkersareadequatelytrainedonhowtosecuretoolswiththetoollanyardorremovetools whennotinuseto
preventthepossibilityoffallingorrollingoffanedgeandstrikingworkersbelow
