This article discusses the role of genomic instability in the pathogenesis of squamous cell carcinoma of the head and neck (HNSCC). Genomic instability results in the natural selection of a genomically heterogeneous tumor mass that threatens the organism. Multiple genetic alterations are required for cells to become cancerous, and genomic instability provides an enabling mechanism for tumors to evolve. Understanding genomic instability may help explain the genetic heterogeneity seen in HNSCC tumors and identify new therapeutic targets.

The American Society of General Surgeons (ASGS) supports the use of transoral fundoplication by trained surgeons as a treatment for chronic gastroesophageal reflux disease (GERD) in patients who do not get satisfactory relief from proton pump inhibitors or who wish to avoid lifelong medication dependence. The ASGS believes transoral fundoplication adheres to the same surgical principles as traditional fundoplication techniques by creating a full thickness wrap of the stomach around the esophagus. The ASGS position is supported by peer-reviewed literature demonstrating transoral fundoplication offers comparable results to open and laparoscopic surgery, with high patient satisfaction and relief of GERD symptoms.

This chapter discusses the 100-year history of surgery for achalasia. Early treatments involved dilation, but the cause was unknown. In the early 1900s, theories proposed causes like esophageal irritability or paralysis. The term "achalasia" was coined in 1913. Heller performed the first cardiomyotomy in 1913, dramatically improving symptoms. Later, fundoplications were added to reduce side effects. Today, treatments include minimally invasive options like pneumatic dilation and peroral endoscopic myotomy (POEM), with excellent outcomes. The evolution of understanding and treatment of this disease over the last century is described.

This document summarizes current approaches for treating gastric cancer. It reviewed literature on staging, endoscopy, classification, laparoscopy, chemotherapy and palliation. The conclusion discusses differences between European and Eastern approaches to lymphadenectomy in gastric surgery. While D1 lymphadenectomy is considered the safest option in Europe, D2 lymphadenectomy is preferred in Japan for greater locoregional control and lower recurrence rates when curative resection is possible.
This document provides a history of the medical and surgical management of acute cholecystitis. Ancient civilizations like the Babylonians and Etruscans depicted the liver and biliary system in clay models, showing early recognition of its importance. However, understanding of the gallbladder's function remained limited for many centuries. It was not until the 1500s BCE that the first gallstones were discovered in Egyptian mummies. Gradually through the works of Hippocrates and Aristotle, jaundice began to be associated with liver disease. However, significant advances in understanding the pathophysiology of biliary tract diseases did not occur until the late 19th century with the development of surgical techniques and technologies like laparoscopy. The history
This study conducted in Mulago Hospital ICU in Uganda aimed to determine if abnormal chest x-rays and Modified Early Warning Scores (MEWS) could predict patient mortality. The prospective observational study analyzed 87 patients admitted to the ICU requiring mechanical ventilation. Results found patients with MEWS ≥5 and abnormal superior mediastinum on chest x-rays were associated with higher mortality. While abnormal chest x-rays may help predict mortality, the relationship was not statistically significant likely due to small sample size. MEWS was a better predictor of mortality and could help prioritize treatment. Larger studies are still needed to further evaluate chest x-rays as a predictive tool in low-resource ICU settings.

This chapter discusses the anatomical and physiological changes that occur in pregnancy to support fetal development. During pregnancy, the cardiovascular, respiratory, renal, and gastrointestinal systems undergo adaptations to increase blood volume, cardiac output, and kidney and liver function which benefit fetal growth. The hormonal changes that facilitate these adaptations, including high levels of progesterone, estrogen, human chorionic gonadotropin, and cortisol, can also cause discomforts for the mother like nausea, breast tenderness, and skin changes.

This document summarizes research demonstrating a 10Gb/s duobinary transponder using an optical dispersion compensator (ODC) receiver to achieve transmission over 9000ps/nm of uncompensated fiber. The transponder uses an integrated ODC at the receiver to compensate for chromatic dispersion and restore the signal quality. Testing showed the ODC improved the required OSNR by 1.3dB compared to no ODC, extending the dispersion tolerance to ±6300ps/nm and achieving error-free transmission over 11,000ps/nm when the ODC was configured for double pass compensation. This approach removes the penalty from intersymbol interference in duobinary coding and provides transmission performance similar to high-quality NRZ
