The intern observed various services including prosthetics, orthotics, pediatrics and mastectomy. Duties included shadowing practitioners, working in the shop, and administrative tasks. Projects included making orthotics, a case study, and obtaining approval for machinery. The intern concluded it was a valuable experience to learn about the field and healthcare system.

Defecation
Normal defecation begins with movement in the left colon, moving stool toward the anus. When stool reaches the rectum, the distention causes relaxation of the internal sphincter and an awareness of the need to defecate. At the time of defecation, the external sphincter relaxes, and abdominal muscles contract, increasing intrarectal pressure and forcing the stool out
The Valsalva maneuver exerts pressure to expel faeces through a voluntary contraction of the abdominal muscles while maintaining forced expiration against a closed airway. Patients with cardiovascular disease, glaucoma, increased intracranial pressure, or a new surgical wound are at greater risk for cardiac dysrhythmias and elevated blood pressure with the Valsalva maneuver and need to avoid straining to pass the stool.
Normal defecation is painless, resulting in passage of soft, formed stool
CONSTIPATION
Constipation is a symptom, not a disease. Improper diet, reduced fluid intake, lack of exercise, and certain medications can cause constipation. For example, patients receiving opiates for pain after surgery often require a stool softener or laxative to prevent constipation. The signs of constipation include infrequent bowel movements (less than every 3 days), difficulty passing stools, excessive straining, inability to defecate at will, and hard feaces
IMPACTION
Fecal impaction results from unrelieved constipation. It is a collection of hardened feces wedged in the rectum that a person cannot expel. In cases of severe impaction the mass extends up into the sigmoid colon.
DIARRHEA
Diarrhea is an increase in the number of stools and the passage of liquid, unformed feces. It is associated with disorders affecting digestion, absorption, and secretion in the GI tract. Intestinal contents pass through the small and large intestine too quickly to allow for the usual absorption of fluid and nutrients. Irritation within the colon results in increased mucus secretion. As a result, feces become watery, and the patient is unable to control the urge to defecate. Normally an anal bag is safe and effective in long-term treatment of patients with fecal incontinence at home, in hospice, or in the hospital. Fecal incontinence is expensive and a potentially dangerous condition in terms of contamination and risk of skin ulceration

Hemorrhoids are dilated, engorged veins in the lining of the rectum. They are either external or internal.
FLATULENCE
As gas accumulates in the lumen of the intestines, the bowel wall stretches and distends (flatulence). It is a common cause of abdominal fullness, pain, and cramping. Normally intestinal gas escapes through the mouth (belching) or the anus (passing of flatus)

“GHS Center for Prosthetics & Orthotics provides comprehensive
prosthetic, custom orthotic and off-the-shelf orthotic services. Our
clinical team provides prosthetic and orthotic intervention for infants,
toddlers, adolescents, and adults. We work in collaboration with your
physician and therapist to provide the most functional and
comfortable solution individualized specifically for you.”

Available for pre-surgical consultation
Available in hospital for post-surgical and pre-prosthetic
management and will provide continued services during
rehabilitation.
Provide orthotic solutions related to injury, illness, disease, and
trauma. Extensive orthotic intervention for pediatric population,
focusing on cranial remolding orthosis, scoliosis management,
and lower extremity bracing.

Services
Upper- and lower-extremity prostheses, myoelectric
prosthetics, and computer controlled leg prosthetics.
Comprehensive follow-up and care for people with
amputations.
Collaborate with vascular surgeons, orthopedists, internists,
family medicine physicians, prosthetists, physical therapists,
occupational therapists.

12. Internship Preceptor
My Internship Preceptor was Todd O’Hare,
PT, CPO.
Todd is the manager of the GHS Center for
Prosthetics and Orthotics. He was providing
direct supervision most of the time during my
internship as I completed projects and worked on
miscellaneous tasks throughout the office.
13. Other Practitioners
There were 4 other CPO’s (Certified
Prosthetists/Orthotists) at the Edgewood Office
where I interned. There was also an amputee
coordinator who is also a PTA, 3 certified orthotic
fitters and 3 office staff who worked with reception
duties, all administrative tasks, and billing. I was
able to observe patient appointments with each of
them and help with administrative tasks with the
front office staff.
14. Roles and Duties
During my internship, I was able to shadow practitioners during
patient visits, work in the shop, and help with some
administrative tasks.
I observed halo adjustments and removals, orthotic and prosthetic
fittings and castings, cranial remolding scans, and diabetic shoe
fittings.
I was able to make heel inserts, my own custom foot orthotic, a
custom ankle-foot orthotic, and learn how to use all of the
machines in the workshop.
I helped make a patient instruction booklet and helped with the
process of getting approved by the American Board for
Certification in Orthotics and Prosthetics.
16. I was able to make a custom Ankle-Foot Orthotic from a cast of
my own leg with the help of technicians at the office. I helped
with the shaping of the cast, pulling the plastic, and grinding
and shaping the AFO.

I was able to go through the full process of fabricating a
custom foot orthotic of my own foot from taking an impression
to modification. I completed all of the steps with assistance and
supervision from certified Orthotic technicians.

18. Internship Projects: Case Study
I was able to complete a case
study of a child who was
treated with a custom cranial
remolding orthosis (helmet).
This case study was approved
by my preceptor.
19. Conclusions
I very much enjoyed my time at the GHS Center for Prosthetics
and Orthotics. It was helpful for me to see how an office works
as part of a larger healthcare system. I was able to get a lot of
unique hands-on experience, for which I am very thankful. The
GHS O&P staff were very welcoming and taught me so many
things that I did previously not know about their field.
