After the information revolution produced a global village, the economic revolution with its global flows of goods and services, finance and people, produces globalization of life. While research on social determinants of health means mental health must be understood globally, we talk about mental health in local and static terms, confining mental health to the margins. Health problems are seen as entities rather processes, solutions aimed at individuals instead of communities, and we overvalue biological explanations and undervalue family, social and cultural contexts for mental and relational problems. As the WHO/World Bank conference declared, “it’s time to move mental health from the margins to the mainstream of the global development agenda.”
This symposium by the Global Mental Health (GMH) Caucus addresses key developments in the GMH movement, integrating social determinants of health, primary care, school mental health, bio-ethics and cultural psychiatry, and emerging themes in medicine and the humanities on migrants, refugees and borders. Presentations by senior physicians address: (1) global initiatives in primary mental health to deal with health disparities, complexity and co-morbidity; (2) ethical dimensions of cultural sensitivity, ethical sustainability and culture-fair guidelines; (3) a pilot study for school mental health in several school settings in Kenya; and (4) needs of migrants and refugees in the light of changing definitions of borders and belonging. Two international fellows and residents provide the perspectives of psychiatrists-in-training on GMH.
Transforming Our Realities
The keynote addresses focused on highlighting both the challenges and successes of Indigenous peoples in Canada. While poverty continues to be a challenge, there have also been many important achievements and successes that have made a real difference in communities. These include outstanding Indigenous graduates in many professions and disciplines, as well as over 40,000 Indigenous-run businesses across various sectors. These successes demonstrate the talent and intelligence of Indigenous peoples and their important contributions to Canada when given opportunities. Moving forward, it is important to continue recognizing both challenges and successes in order to build upon areas of strength.

are staggering. In the last few years, the world has become more aware of this enormous burden and the potential
for mental health gains. We can make a difference using existing knowledge ready to be applied.
We need to enhance our investment in mental health substantially and we need to do it now.
Proposal for Integrated Treatment Behchoko

This report explores the significance of border towns and spaces for strengthening responses to young people on the move. In particular it explores the linkages of young people to local service centres with the aim of further developing service, protection, and support strategies for migrant children in border areas across the region. The report is based on a small-scale fieldwork study in the border towns of Chipata and Katete in Zambia conducted in July 2023. Border towns and spaces provide a rich source of information about issues related to the informal or irregular movement of young people across borders, including smuggling and trafficking. They can help build a picture of the nature and scope of the type of movement young migrants undertake and also the forms of protection available to them. Border towns and spaces also provide a lens through which we can better understand the vulnerabilities of young people on the move and, critically, the strategies they use to navigate challenges and access support.
The findings in this report highlight some of the key factors shaping the experiences and vulnerabilities of young people on the move – particularly their proximity to border spaces and how this affects the risks that they face. The report describes strategies that young people on the move employ to remain below the radar of visibility to state and non-state actors due to fear of arrest, detention, and deportation while also trying to keep themselves safe and access support in border towns. These strategies of (in)visibility provide a way to protect themselves yet at the same time also heighten some of the risks young people face as their vulnerabilities are not always recognised by those who could offer support.
In this report we show that the realities and challenges of life and migration in this region and in Zambia need to be better understood for support to be strengthened and tuned to meet the specific needs of young people on the move. This includes understanding the role of state and non-state stakeholders, the impact of laws and policies and, critically, the experiences of the young people themselves. We provide recommendations for immediate action, recommendations for programming to support young people on the move in the two towns that would reduce risk for young people in this area, and recommendations for longer term policy advocacy.

This is the result of a government project aiming to energize the shanty town and create a warmer welcome to those from displacement camps post-earthquake. Port-au-Prince is the setting of this two-year project funded by Grand Challenges Canada.
- 2. Dr. Martineau Guerrier, the co-Principal Investigator from the project partner site: the Commission Nationale de Lutte contre la Drogue, shares his perspective on the significance and potential impact of this project for the Haitian population with one of many media outlets covering program activities throughout the week.
- 3. Gaining wide exposure through a press conference with newspapers, television and radio stations at the opening day and throughout the week to showcase the learning sessions on cognitive behavioural therapy (CBT) and thought-provoking discussions about mental health in Haiti.
- 4. Before the information session begins, a group of individuals from the Vodou sector perform a special ceremony filled with drumming, song and dance to initiate the start of program activities. As part of the ceremony, spirits were welcomed in all corners of the room.
- 5. Community members from the Catholic sector in a moment of prayer and well wishes before the learning session on cognitive behavioural therapy begins.
- 6. Dr. Christine Courbasson, psychologist and our CBT expert, provides one of many learning sessions over the course of a week to build a foundation for discussion and feedback. Each session focuses on the overall purpose of the therapy, some basics on how it can be delivered and a cognitive distortions exercise to apply learning.
- 7. Vising the Lakou of Ati Max Beauvior, the Chief of Haitian Vodou; a family compound intimately tied to Vodou practice and described as a place where spirits reside.
- 8. A round table discussion with Dr. Courbasson and the Ati Max Beauvoir’s community of academics, ethnographers, Huongans (Vodou Priests) and Mambos (Vodou priestesses) and others on the goals of the project and background information on CBT.
