This document discusses universal health insurance (UHI) in Ireland and outlines some key issues. It distinguishes between universality, meaning health benefits for everyone funded through compulsory contributions, and insurance, which involves personal contracts, choice of insurers, and separate organizations for payors and providers. Two main problems are identified: fees limiting primary care access and two-tier hospital access. Implementing UHI will involve merging public funding streams with private insurance money flows and clarifying issues around payments, benefits, and the roles of providers, insurers, and the public.
This document discusses Norway's implementation of the Maritime Labour Convention (MLC) in 3 key areas:
1. Norway will adopt the MLC's definition of "shipowner" as the International Safety Management (ISM) Company to ensure one party is responsible for seafarers' working conditions.
2. Norway will implement the joint IMO/ILO medical guidelines but recognize medical certificates nationally at first due to challenges with mutual recognition.
3. Norway delegates authority to recognized organizations to inspect ships and issue labor certificates, and provides guidance while requiring shipowners to develop compliance plans.

This document summarizes a study that analyzed the phytochemical composition of ethanolic leaf extracts from four plants: Acalypha wilkesiana, Leucaena leucocephala, Peperomia pellucida, and Sena alata. The results found that the extracts contained several important phytochemicals including tannins, flavonoids, saponins, alkaloids, and cardiac glycosides. These phytochemicals have pharmacological properties that could be useful for treating bacterial infections, inflammation, and heart conditions in both fish and humans. The study demonstrates that these four plants contain medicinally active compounds and could potentially be developed for use in chemotherapeutics.

Connected Health & Me - Prof George Crooks - Nov 24th 2014ipposi
The document discusses connected health and telehealth initiatives in Scotland. It provides context on Scotland's population of 5.3 million people and devolved health system with £12 billion in annual NHS funding. The vision by 2020 is for integrated health and social care focused on prevention, self-management, and home-based or community care when possible. Key drivers for change include an aging population, chronic conditions, workforce shortages, and financial unsustainability. The Scottish Centre for Telehealth and Telecare coordinates national telehealth and telecare programs to help manage demand on the NHS and deliver clinical services more efficiently using technologies like telehealth.
The Affordable Care Act is making health care more secure and giving consumers more control by:
1) Outlawing discriminatory practices like denying coverage due to preexisting conditions.
2) Providing protections and benefits that are already in effect like allowing young adults to stay on their parents' plans.
3) Creating a competitive health marketplace in 2014 that will guarantee affordable coverage for all.

A presentation I gave at the EGM of Ireland's National Association of General Practitioners. Shows progress in some areas of health; payments to GPs since 2002; and argues that general practice should embrace measures which show its value and contribution to healthcare.
The document discusses health spending trends and sustainability. It finds that while spending is growing, it is not unsustainable and growth is not out of control. Age-related cost increases can be managed through anticipation. However, the application of resources may not be efficient. Reforms aimed at improving efficiency, such as reducing staffing and supply costs, could achieve annual savings of up to 5% of GDP.
Ceohealth matters paper 4 sept12 forum jb oocOliver O'Connor
This document discusses how the introduction of a Money Follows the Patient (MFtP) payment system in Ireland could be influenced by European law and the Single Market. It notes that MFtP is envisioned as a step towards establishing universal health insurance in Ireland. The document outlines some key questions around how MFtP might work in practice, such as whether private providers could bid for publicly funded services, and how MFtP might interact with a future system of competing health insurers commissioning care. It also briefly discusses the potential relevance of the Fair Deal nursing home scheme, which already uses a form of money following the patient.
The document summarizes the key economic and political factors surrounding Ireland's "Celtic Tiger" boom and subsequent banking crisis from the late 1980s to 2010. It discusses how Ireland experienced rapid economic growth until the 2008 global financial crisis, which then led to a massive banking and property crash in Ireland. Loose fiscal policy, easy access to credit, and overreliance on construction contributed to an unsustainable boom. The crash forced Ireland to accept an EU-IMF bailout and has left lasting economic and political impacts.

The document discusses the challenges facing Ireland's goal of introducing universal health insurance by 2016. It will be an immense undertaking requiring extensive economic, legal and administrative changes across the entire healthcare system. Many crucial details about benefits, costs, and rules still need clarification for patients, providers and insurers. Creating such vast reforms to a functioning healthcare system within just six years will be an ambitious challenge.
Application of EU Single Market rules to providers of healthcare in Ireland anticipating move to Universal Health Insurance - legal and practical arguments
Submission to Dilnot Commission on Social Care UKOliver O'Connor
The document discusses Ireland's reform of financing long-term residential care. It introduced a new scheme in 2009 that shares costs between individuals and the state based on means testing, without requiring individuals to sell their homes to pay for care. The reform aimed to establish a coherent system with protections for users and choice among public and private homes meeting quality standards. Significant political commitment was needed to overcome initial opposition during the multi-year design and implementation process.
Tariff setting in Dutch Healthcare system, Johan van ManenOliver O'Connor
This document discusses health care reform and tariff setting in the Netherlands. It outlines two models used to calculate tariffs. Model I calculates average costs per diagnostic treatment combination (DBC) procedure based on a sample of hospitals, while Model II calculates total costs per DBC for each hospital. The document also describes how the proportion of regulated versus negotiable hospital costs and physician fees has changed over time as the system transitioned from centralized regulation to more market-based negotiations between insurers and providers.
The document discusses plans for implementing a universal health insurance (UHI) system in Ireland. It outlines that UHI will provide equal access to healthcare for all through compulsory health insurance paid through income-based premiums. However, implementing UHI will be an immensely complex process requiring changes to healthcare funding, providers, doctors' contracts, and legal frameworks. The timeline implied for transitioning to UHI by 2016 is optimistic given capacity constraints and lack of progress on necessary precursors like reforming payment systems and hospital governance.
Initial Analysis of Universal Health Insurance 24 March 2011Oliver O'Connor
This document discusses issues surrounding the implementation of universal health insurance (UHI) in Ireland. It notes that UHI would involve many stakeholders, including government officials, healthcare providers, insurers, taxpayers, and the public. It also identifies challenges such as balancing costs and services, determining funding mechanisms, ensuring fair competition between public and private insurers/providers, and providing clarity around benefits and responsibilities. The document emphasizes that successfully implementing UHI will require navigating many complex tradeoffs and choosing a model tailored to Ireland's unique circumstances.
Cost dynamics in Irish Health Care Society of Actuaries presentation Oct 2012Oliver O'Connor
The document discusses rising health care costs as a threat to economies and sustainability. It notes that while aging populations contribute somewhat to increased costs, other factors like new technologies and income growth are larger drivers. International data shows health spending as a percentage of GDP has risen significantly over 40 years. While most countries see continued increases projected, Ireland is estimated to have relatively low excess cost growth. The document advocates for policies like budget caps, more competition, and supply-side reforms to manage costs over the long run.

4. And Insurance implies…
• BeneJits speciJied by rules
• not just HSE/hospitals funded to do their best
• A personal contract‐based entitlement to a service
• not limited by resources of a hospital or insurer
• radical difference in law and budgeting
• A requirement to maintain solvency and reserves
• Usually, payor and provider are different organisations
• Choice: social insurance (like unemployment beneJit)
or
• Run through commercial insurance companies
• With choice of insurers ✔

5. To distinguish…
• We could have Universal Healthcare (NHS, Sweden,
Spain)
• without
• Universal Health Insurance (NL, Germany, France)
• but decision made: it will be insurance
• it will be multi‐insurers, not just one
• Still, helpful to identify which elements of the
Government plan involve ‘universality’ and which
‘insurance’
www.oliveroconnor.co
5
7. Common issues for both
• People: create new public law entitlements and obligations
• You all get the same beneJits
• But some people pay more than others, and more than now
• Money: merge two funding streams
• Public capitation and private fees
• Exchequer funding and private health insurance
• Insurers as intermediaries
• Customer facing: administer all beneJits and claims
• Innovation: scope to design new policies, beneJits
• Money: receive funding and pay providers

9. Primary care ‐ free GP care
www.oliveroconnor.co
9
• Cost of extension of GP care: €40m for under‐5s
• GP fee: €70 per year per child capita8on
• Avg 3 visits per year per child under 5*
• Will GPs agree? Difficult to implement without agreement
• Free GP care for whole popula8on:
• €445m paid now to cover c.43% of popula8on
• Cost for rest depends on GP payment
• Anywhere from €400m up: more usage
• What will 2,800 GPs agree? Fee, capita8on or salary? How much? A
replacement of all private fees?
• Health policy balance: cost as barrier to access vs over‐use of ‘free’
service
• Hiring of new primary care staff for full service: slow
*Source: ESRI, Resource Alloca8on Group Report, 2010, old data back
to 1987
11. Primary care – insurance issues
www.oliveroconnor.co
11
• Will free GP care first be paid by Exchequer?
• Increase taxa8on, increase public spending
• Later insurance policies will incorporate this benefit. Will this
then reduce Exchequer spending?
• One way or another, a compulsory annual payment by some
people to eliminate GP fees at point of use
• More logical to require people to buy GP insurance now as “free
GP care”, saving the Exchequer cost
• What else will be covered by the insurance policy?
• All primary care
• No fees (deduc8bles) at the point of use for any service?
• Insurers not ready for some years to fund primary care services
• Apart from limited cash‐plans => fees, not capita8on, for doctors
12. Hospital UHI – all about Equity
• Whole population can already use public hospital care
• 1970 Act Jirst, then extended to all in 1993/4
• Universality in place but…
• Equity test:
• What about faster access to private care in public hospitals?
• Access to private care in private hospitals?
• Policy op8ons
• Implement single appointments system in public hospitals
• Underpin with legal obliga8on to give certainty of equity
• Allow private hospitals par8cipate in public benefits provision
• On condi8on they offer first‐come, first‐serve appointments
• Actual policy: equity will be achieved the same insurance
contracts for all, using both public and private hospitals

• Perhaps everyone happy that all have equal access and everyone

18. UHI ‐ Hospital Care ‐ Progress?
• Universality/equity: no change yet
• implementation of 2008 consultant contract
• Insurance: VHI authorisation near, risk equalisation needing
revision, market under price/cost pressure
• Governance: creation of 7 hospital groups: good, but not
directly relevant to universality, equity and insurance
• Access: private hospitals still limited to private insurance
• Charging all private patients in public hospitals:
• aimed at increasing income for public hospitals
• payment remains bed night charge, not per procedure
• increases incentive for public hospitals for more private patients
• Awaiting UHI White Paper at end this year
www.oliveroconnor.co
18
21. Clarity for the public
• Free GP care: 3 options
• Pay by Exchequer – raise taxes, increase public spending
• Compulsory private cash‐plan contracts, part subsidised
• Modify: include graduated private fee payments (as per
Resource Allocation Group Report 2010); rest paid by taxes
• UHI in hospitals: make clear it has beneJits and costs
• Who will pay, and how much
• Means‐tested contributions: what are the limits
• What choice will there be of doctor, hospital, beneJits
www.oliveroconnor.co
21
22. What is needed, when
• White Paper with serious policy detail, 2013‐14
• Including an estimate of cost of UHI and how it is borne
• Decisions on free GP care or variations: 2013‐15
• Negotiations with GPs: 2013‐14
• Huge amount of technical work with insurance experts:
2013‐20
• Stabilisation of insurance market and risk equalisation:
2014 ‐>
• New consultant contract: 2018?
• Strategic agreement with public and private hospitals to
allow planning: 2014‐15
• Post‐White Paper implementation plan: 2014‐15
