Limitations of documenting workplace incidents
1) Lack of transparency and adequate record-keeping
➢ Reporting based on
emotional, opinionated,
biased or favoritism
statements
➢ Not aware of the place
of the company’s
incidents and health
records
Limitations of documenting workplace incidents
2) Lack of accuracy of the incidents data
➢ Overlook the errors
- Typos and spelling grammar
E.g.: Names of people involved,
contact number and the date and
time of the incident.
➢ Giving vague statements and
providing details that are not
specific
- May create confusion
3) Time consuming
➢ The process of collecting data and
information of the incidents could
take a lot of time as it needs to be
investigated and completed in
order for the documenatation to
be valid.
6 steps that need to be followed
when documenting incidents in the
workplace:-
1) Evaluate and document the scene
-Identify witnesses
-Take photos of the scene or sketch the scene if
camera not available
2) Investigate the safety incident
-Establish the Who,What,When,Where,Why and
How
3) Interview people involved in the
incident
4) Document the details of the incident
-File and retain the file in chronological order
5) Protect privacy
-Ensure the investigations report are not released
to anyone without proper authorization
6) Review the investigations
-Review all incident and near-miss investigations
that have occured

  • 10. Strengths of documenting workplace incidents 1. Raise awareness among employees and employers 2. Enforce the safety culture - about the things that are hazardous to the workers so that the corrective and preventive actions can be taken by the upper management of the company. - OSHA requires a Log of Work-Related Injuries and Illnesses to be maintained by the organization - Incident research focused on recognizing and correcting the root causes increases the morale and attitude of workers towards health and safety. - employees will be cooperative in implementing the new safety prevention steps as they are aware of the incidents that might happen.

Limitations of documenting workplace incidents 2) Lack of accuracy of the incidents data ➢ Overlook the errors - Typos and spelling grammar E.g.: Names of people involved, contact number and the date and time of the incident. ➢ Giving vague statements and providing details that are not specific - May create confusion
  • 14. 3) Time consuming ➢ The process of collecting data and information of the incidents could take a lot of time as it needs to be investigated and completed in order for the documenatation to be valid. 6 steps that need to be followed when documenting incidents in the workplace:- 1) Evaluate and document the scene -Identify witnesses -Take photos of the scene or sketch the scene if camera not available 2) Investigate the safety incident -Establish the Who,What,When,Where,Why and How 3) Interview people involved in the incident 4) Document the details of the incident -File and retain the file in chronological order 5) Protect privacy -Ensure the investigations report are not released to anyone without proper authorization 6) Review the investigations -Review all incident and near-miss investigations that have occured Continued..
