Introduction
• Chronic wounds are common in primary care settings, require a
high level of resources, and can have a profound impact on a
patient’s quality of life.
• The complexity of the wound care ecosystem may give significant
challenges due to (i) increasing costs of wound care; (ii) an aging
population; (iii) increasing prevalence of comorbidities (eg,
obesity); and (iv) bacterial resistance to antimicrobials.
• Treating wounds in the community requires a new holistic
approach.
• The aims is to promote best practice in wound management and to
advocate for the rights of people with wounds, to receive the best
possible care in a timely manner that meets the needs of the
individual and the health service.

Wound Care In Primary Health Care
• In Malaysia, wound care/dressing is one of services that has been
provided in primary health clinic – focusing more on wound
cleansing and dressing.
• The number of patients came for wound dressing had increased
overtime due to increased number of road-traffic accidents,
increased prevalence of diabetes complications and other chronic
diseases such as stroke and cancer.
• Majority of the patients were referred from the hospital to continue
daily normal saline dressing causing over-crowding of the patients at
the dressing room.
• The wound care service is not optimize enough due to lack of
manpower, inter-personal skills, limitations of infrastructure and
advance dressing material.

• The development of new concepts and insights into
wound assessment and healing process and the wide
variety availability of effective wound dressing
materials triggered the interest of primary health care
providers to this new methods of wound care.
• Modern wound dressing become more popular as this
method had proven shorten the time of wound healing,
more cost-effective, reduce the dressing load and
improve the quality of life of the patients and their
caregivers.

The Implementation Of The Best Practice In Wound
Care At Primary Health
However, modern wound care cannot be implemented wisely
due to many limitations - shortage of manpower, lacking of
knowledge and skills on modern wound care dressing among
health care providers, limited financial support, inadequate
infrastructures and the availability of modern wound dressing
products.
To optimize the quality of wound care services, few important
and drastic interventions need to be planned and implemented
with the full support from the top management in the Ministry
of Health, at state and districts level.

• Holistic patient assessment is essential in order to
identify the causative or contributing factors that
could potentially delay or prevent wound healing.4
• Wound assessment is about assessing the wound
bed, planning appropriate interventions and
reviewing assessments.

WOUND CARE TEAM AT PRIMARY HEALTH CLINIC
• Each member of the Wound Care Team shall be well-trained healthcare professionals in
wound care either domestically or overseas.
• Provide treatment and consultation regarding optimal wound care management to
patients whether new cases or chronic (follow-up) cases.
• Provide advice and guidance on appropriate wound care equipment and methods to other
health personnel.
• Provides health education related to wound care to other health care personnel, patients
or the public.
• Monitor and evaluate the performance and efficacy of wound care in health clinics while
providing reports and retention on wound care management.
• Head of the Wound Care Team is responsible as intermediate officer between the District
Health Department and the Primary Health Clinic.

3. ESTABLISH WOUND CARE CLINIC
• Integrated clinic is manage by trained Family Medicine Specialist
(FMS) together with Medical Officer (MO) & paramedical staff.
• All referred wound cases must be reviewed by MO.
• Holistic Wound assessment will be performed & treatment plan
will be provided.
• The treatment plan will be reviewed according to the progress of
the wound healing process.
• Follow up treatment will be reviewed by FMS/MO - every 1 to 2
weeks or as needed.
• Patients need to be registered & given follow-up appointment.
