According to the Centers for Disease Control (CDC), asthma is a leading chronic illness in children ages 5 to 17, a leading cause of school absenteeism, and the third highest cause of hospitalizations in children younger than 15 (2011). The costs associated with treating this chronic illness are high.
A number of randomized control trials have examined the effect of instituting a home management program for treating asthma in combination with pharmaceuticals. These studies found that, by implementing a structured home management program, the morbidity, severity, and frequency of asthmatic episodes were reduced (Agrawl, Singh, Mathew, & Malhi, 2005). Recent research is also exploring how and when to cut back on levels of medication as asthma episodes become controlled, further reducing the cost of this chronic illness; however, controversy remains over the long-term effects of reducing levels of medication due to the disparateness of asthma (Rogers & Reiberman, 2012).

Reducing the cases of readmissions among patients.
One of the issues that are affecting the healthcare sector is readmission. When a patient is readmitted, it means that there is a high possibility that the healthcare practitioners failed to intervene effectively. A person might be readmitted because he or she did not fully recover from the previous illness. Also, a person may be readmitted because healthcare practitioners might have missed the cause of the illness. Therefore, it means that they might have administered care that is not in line with the issue of the patient.
Purpose
The purpose of the research topic is to ensure that the problem at hand is addressed. The title of the research is trying to look for ways in which the number of readmission cases can be significantly reduced. In this case, it becomes evident that the topic of research or the research is looking for an intervention to a specific issue. The issue at hand is associated with readmissions, and the intervention is to reduce the number of readmission cases. The research also plays an essential role in informing people or the audience about the issue of readmission (Zuckerman, Sheingold, Orav, Ruhter, & Epstein, 2016). The healthcare sector has been suffering because of the increasing numbers of readmissions. When the numbers of readmissions continue to increase, it means that patients start to lose faith and trust in the healthcare sector fraternity. When research focuses on an issue, it does so because it wants people to know more about the issue and to identify the best ways to deal with the issue.

The practice problem I chose is Central Line-Associated Blood Stream Infections (CLABSIs) in Pediatric Patients in the Home Care Setting. In my department, we have a small pediatric population. The majority of pediatric patients have central lines for parenteral nutrition therapy. These patients represent a small portion of our patients with Central Venous Access Devices (CVADs); however, they represent over 90% of the CLABSIs in our department.
ย ย ย ย ย ย I performed a literature search with the Chamberlain Librarian's assistance, who helped me focus my search for peer-reviewed articles representing CLABSIs in the home's pediatric population.ย This literature review revealed several articles related to the topic. I chose the articles to review for research location, known authors on the subject matter, the journal in which it was published, the subject matter itself, and did it relate to the desired topic.ย I also looked for the level of research.ย I reviewed the abstracts and looked for a methods section. I compared the methods and again looked for validity toward the topic.ย
ย ย ย ย ย ย I found two compelling articles written relatively close together addressing what I was looking for. One article was very clearly a Quality Improvement project done that was identified by a large children's hospital that was seeing an increase of Community-Acquired CLABSI's (CA-CLABSI's) once the patient was discharged to a home health agency. The other article I believe to be a retrospective quantitative research design based on six years of data collection from chart audits. I am choosing to focus on the second article. The article was written by four physicians from the University Children's Hospital in Zurich, Switzerland, between 2009 and 2015. The four physician's contributed equally to the study and are infectious diseases and epidemiology specialists.ย
ย ย ย ย ย ย The intended audience of the article includes medical providers who care for acutely and chronically ill children, who prescribe treatments and order the intended central lines for these treatments, such as IV antibiotics and total parenteral nutrition. In addition, the nurses who care for the central lines on a routine ongoing basis. The unintended focus specifically called out Pediatric Gastroenterologists because their research found that children with gastroenterology type diagnoses were at a significantly increased rate of CA-CLABSI's. The seven-year study was published in January 2020.

Objective. To compare the effectiveness of conventional (CF),
laser (LF), and Z-plasty (ZF) frenotomies for the treatment
of ankyloglossia in the pediatric population.
Data Sources. A comprehensive search of PUBMED,
EMBASE, and COCHRANE databases was performed.
Review Methods. Relevant articles were independently assessed
by 2 reviewers according to the Preferred Reporting Items for
Systematic Reviews and Meta-Analysis (PRISMA) guidelines.
Results. Thirty-five articles assessing CF (27 articles), LF (4
articles), ZF (3 articles), and/or rhomboid plasty frenotomy
(1 article) were included. A high level of outcome heterogeneity
prevented pooling of data. All 7 randomized controlled
trials (RCTs) were of low quality. Both CF (5 articles with
589 patients) and LF (2 articles with 78 patients) were independently
shown to reduce maternal nipple pain on a visual
analog or numeric rating scale. There were reports of
improvement with breastfeeding outcomes as assessed on
validated assessment tools for 88% (7/8) of CF articles (588
patients) and 2 LF articles (78 patients). ZF improved
breastfeeding outcomes on subjective maternal reports (1
article with 18 infants) only. One RCT with a high risk of
bias concluded greater speech articulation improvements
with ZF compared to CF. Only minor adverse events were
reported for all frenotomy techniques.
Conclusions. Current literature does not demonstrate a clear
advantage for one frenotomy technique when managing children
with ankyloglossia. Recommendations for future research
are provided to overcome the methodological shortcomings in
the literature. We conclude that all frenotomy techniques are
safe and effective for treating symptomatic ankyloglossia.

A clinical pediatric nurse has noticed a rise in childhood cancer diagnoses among the Hispanic population served by the local clinic. The nurse is concerned about this increase in cancer incidence in the patient population and turns to the literature to explore current research on this topic. Reducing the cases of readmissions among patients.
One of the issues that are affecting the healthcare sector is readmission. When a patient is readmitted, it means that there is a high possibility that the healthcare practitioners failed to intervene effectively. A person might be readmitted because he or she did not fully recover from the previous illness. Also, a person may be readmitted because healthcare practitioners might have missed the cause of the illness. Therefore, it means that they might have administered care that is not in line with the issue of the patient.
Purpose
The purpose of the research topic is to ensure that the problem at hand is addressed. The title of the research is trying to look for ways in which the number of readmission cases can be significantly reduced. In this case, it becomes evident that the topic of research or the research is looking for an intervention to a specific issue. The issue at hand is associated with readmissions, and the intervention is to reduce the number of readmission cases. The research also plays an essential role in informing people or the audience about the issue of readmission (Zuckerman, Sheingold, Orav, Ruhter, & Epstein, 2016). The healthcare sector has been suffering because of the increasing numbers of readmissions. When the numbers of readmissions continue to increase, it means that patients start to lose faith and trust in the healthcare sector fraternity. When research focuses on an issue, it does so because it wants people to know more about the issue and to identify the best ways to deal with the issue.

The practice problem I chose is Central Line-Associated Blood Stream Infections (CLABSIs) in Pediatric Patients in the Home Care Setting. In my department, we have a small pediatric population. The majority of pediatric patients have central lines for parenteral nutrition therapy. These patients represent a small portion of our patients with Central Venous Access Devices (CVADs); however, they represent over 90% of the CLABSIs in our department.
ย ย ย ย ย ย I performed a literature search with the Chamberlain Librarian's assistance, who helped me focus my search for peer-reviewed articles representing CLABSIs in the home's pediatric population.ย This literature review revealed several articles related to the topic. I chose the articles to review for research location, known authors on the subject matter, the journal in which it was published, the subject matter itself, and did it relate to the desired topic.ย I also looked for the level of research.ย I reviewed the abstracts and looked for a methods section. I compared the methods and again looked for validity toward the topic.ย
ย ย ย ย ย ย I found two compelling articles written relatively close together addressing what I was looking for. One article was very clearly a Quality Improvement project done that was identified by a large children's hospital that was seeing an increase of Community-Acquired CLABSI's (CA-CLABSI's) once the patient was discharged to a home health agency. The other article I believe to be a retrospective quantitative research design based on six years of data collection from chart audits. I am choosing to focus on the second article. The article was written by four physicians from the University Children's Hospital in Zurich, Switzerland, between 2009 and 2015. The four physician's contributed equally to the study and are infectious diseases and epidemiology specialists.ย
ย ย ย ย ย ย The intended audience of the article includes medical providers who care for acutely and chronically ill children, who prescribe treatments and order the intended central lines for these treatments, such as IV antibiotics and total parenteral nutrition. In addition, the nurses who care for the central lines on a routine ongoing basis. The unintended focus specifically called out Pediatric Gastroenterologists because their research found that children with gastroenterology type diagnoses were at a significantly increased rate of CA-CLABSI's. The seven-year study was published in January 2020.

Objective. To compare the effectiveness of conventional (CF),
laser (LF), and Z-plasty (ZF) frenotomies for the treatment
of ankyloglossia in the pediatric population.
Data Sources. A comprehensive search of PUBMED,
EMBASE, and COCHRANE databases was performed.
Review Methods. Relevant articles were independently assessed
by 2 reviewers according to the Preferred Reporting Items for
Systematic Reviews and Meta-Analysis (PRISMA) guidelines.
Results. Thirty-five articles assessing CF (27 articles), LF (4
articles), ZF (3 articles), and/or rhomboid plasty frenotomy
(1 article) were included. A high level of outcome heterogeneity
prevented pooling of data. All 7 randomized controlled
trials (RCTs) were of low quality. Both CF (5 articles with
589 patients) and LF (2 articles with 78 patients) were independently
shown to reduce maternal nipple pain on a visual
analog or numeric rating scale. There were reports of
improvement with breastfeeding outcomes as assessed on
validated assessment tools for 88% (7/8) of CF articles (588
patients) and 2 LF articles (78 patients). ZF improved
breastfeeding outcomes on subjective maternal reports (1
article with 18 infants) only. One RCT with a high risk of
bias concluded greater speech articulation improvements
with ZF compared to CF. Only minor adverse events were
reported for all frenotomy techniques.
Conclusions. Current literature does not demonstrate a clear
advantage for one frenotomy technique when managing children
with ankyloglossia. Recommendations for future research
are provided to overcome the methodological shortcomings in
the literature. We conclude that all frenotomy techniques are
safe and effective for treating symptomatic ankyloglossia.

APUS library to search relevant databases to find at least 4 research articles from peer-
reviewed sources, no older than 7 years, unless it is a landmark study. Two of these articles
must be authored by at least one nurse. Students will then evaluate the evidence found in

practice. The paper will consist of the following sections:โ Introduction to the problem or
issue-The PICOT question being asked, both in a sentence format and broken down into
each elementPopulation: Hemodialysis patients with fistulas, grafts and PICCsORDER NOW
FOR ORIGINAL, PLAGIARISM-FREE PAPERSIntervention: the intervention is the use of the
ethanol lock solution in the PICCs and use of daily 2% chlorhexidine wash. This is the
change proposal that is aimed at reducing the rates of infections associated caused by the
presence of the microbes on the fistula, graft, and PICC access sites.Comparison: the
intervention is compared to the heparin lock solution and lack of use of 2% chlorhexidine
wash.Outcome: Reduction in the rates of the infection in hemodialysis patients in the unit.-A
brief discussion of the databases searched, including key words, limiters used, and total
number of articles retrieved-A brief summary of each of the articles and the Level of
Evidence rating for each, including the evidence rating scale used (i.e. Level I, Johns Hopkins
Nursing Evidence Based Practice Scale)-In-depth discussion of major findings from the
