Predictors of Posterior Reversible
Encephalopathy Syndrome (PRES) in
Women With Pre-eclampsia/Eclampsia :
A Retrospective Analysis
Posterior Reversible Encephalopathy Syndrome (PRES)
● a clinical-radiological entity
● characterized by acute neurological symptoms (e.g., seizures, encephalopathy,
headache, and visual disturbances) with reversible subcortical vasogenic brain edema.
● Studied 15 patients from
1988-1994
● Reversible syndrome of
headache
● Altered mental status
● Seizures
● loss of vision.
● 3 eclampsia
● Concluded: may develop
in patients with renal
insufficiency or
hypertension or
immunosuppressed.
● Neuroimaging-
subcortical edema
without infarct
Pre-eclampsia/ Eclampsia AND PRES:
● Single centre, retrospective
study from 2001-2010
● 23 antepartum eclampsia/ 24
postpartum eclampsia
● 46 of 47 patients had PRES
● Conclusion: PRES is a core
component of pathogenesis
of eclampsia.
● Retrospective study of 46 women with eclampsia/ pre-eclampsia with clinical suspicion of
PRES between August 2007-August 2012.
● 38 patients with pre eclampsia and 8 with eclampsia underwent MRI → 4 (pre-eclampsia)
and 5 (eclampsia) had PRES
● Conclusion: PRES was common in younger female with higher systolic/ diastolic B.P and
was also seen in pre-eclampsia (10.5%). No single or set of findings were reliable predictor
of PRES.
● Single-center, retrospective, cohort study between 2008 and 2013.
● 81 eclamptic women were divided into two groups: 45 (PRES group) and 36 (non-PRES
group)
● PRES group- Most common- headache and visual symptoms(60%) vs Non-PRES
group- only headache (50%)
● PRES group-higher body mass index value (p = 0.005), longer hospitalization time (p =
0.001), and higher level of proteinuria (p = 0.012).
● Non-PRES group had higher Apgar scores (p = 0.002) than those in the PRES group.
● Conclusion:PRES manifests predominantly with headache and visual impairment
together. Adverse neonatal outcomes are also common in these patients.
AIMS:
● To compare the clinical and radiological characteristics of PRES with those without PRES
in patients with pre-eclampsia/eclampsia and attempts to find independent predictors of
PRES.
● Early recognition of pre-eclamptic/eclamptic women at risk of developing PRES.
Methods and materials:
● Single centre observational study
● conducted in unit two of the obstetrics and
gynecology department during 2018-2021.
● Inclusion criteria - Women diagnosed
with pre-eclampsia/eclampsia in the
antepartum or postpartum period.
● Diagnosis of pre-eclampsia-criteria set by
the American College of Obstetricians &
Gynecologists (Table no.1) The occurrence
of manifest seizures confirmed the
diagnosis of eclampsia in these patients.
● Exclusion criteria: History of any
neurologic disorder
Systolic blood pressure ≥140 mmHg or diastolic blood pressure ≥90
mmHg on at least two occasions at least 4 h apart after 20 weeks of
gestation in a previously normotensive patient AND the new onset of
one or more of the following:
Proteinuria ≥0.3 g in a 24-h urine specimen or protein/creatinine ratio ≥0.3
(mg/mg) (30 mg/mmol) in a random urine specimen or dipstick ≥2+ if a
quantitative measurement is unavailable
Platelet count <100,000/microL
Serum creatinine >1.1 mg/dL (97.2 micromol/L) or doubling of the creatinine
concentration in the absence of other renal diseases
Liver transaminases at least twice the upper limit of the normal
concentrations for the local laboratory
Pulmonary edema
New-onset and persistent headache
Visual symptoms (eg, blurred vision, flashing lights or sparks, scotomata)

● Detailed obstetrics history
● Calculate period of gestation from LMP and ultrasonography
● Detailed clinical examination
● B.P monitoring
● Symptoms suggestive of PRES were noted
● Assessment of sensorium by glasgow coma scale
● Brain imaging: to exclude alternate diagnosis and confirm PRES (MRI or MR angiography)
Diagnosis of PRES- presence of vasogenic edema, characterized by T1 iso/hypo and
T2W/FLAIR hyper-intensity and the absence of secondary causes.
● Management:No randomized controlled trials on the interventions for treating PRES. The
mainstay of management is supportive care, hydration, dyselectrolytemia correction along
with control of hypertension. It is important to expedite the delivery of the fetus and
placenta in the pregnant patient.

suggestive of PRES.
Predictors of PRES:
● Multivariate logistic regression analysis
● On regression analysis, eclampsia was found to be the independent predictor of PRES
(OR 20.9; 95% CI 3.0-147.0, p = 0.02). Age (OR 1.3; 95% CI 0.95-1.67, p = 0.12) and
parity (OR 6.8; 95% CI 0.5-91.3, p = 0.15) did not predict PRES.
Discussion:
Conclusion:
● In this study, patients with PRES were younger and significantly more likely to be primiparous
and have eclampsia
● Headache followed by seizures and altered sensorium were the most common clinical
manifestations
● Subcortical white matter hyperintensities involving fronto-parieto-occipital lobes were the most
common radiological finding.
● Eclampsia emerged as an independent risk factor for PRES.

  • 2. Posterior Reversible Encephalopathy Syndrome (PRES) ● a clinical-radiological entity ● characterized by acute neurological symptoms (e.g., seizures, encephalopathy, headache, and visual disturbances) with reversible subcortical vasogenic brain edema.

  • 4. Pre-eclampsia/ Eclampsia AND PRES: ● Single centre, retrospective study from 2001-2010 ● 23 antepartum eclampsia/ 24 postpartum eclampsia ● 46 of 47 patients had PRES ● Conclusion: PRES is a core component of pathogenesis of eclampsia.
  • 5. ● Retrospective study of 46 women with eclampsia/ pre-eclampsia with clinical suspicion of PRES between August 2007-August 2012. ● 38 patients with pre eclampsia and 8 with eclampsia underwent MRI → 4 (pre-eclampsia) and 5 (eclampsia) had PRES ● Conclusion: PRES was common in younger female with higher systolic/ diastolic B.P and was also seen in pre-eclampsia (10.5%). No single or set of findings were reliable predictor of PRES.
  • 6. ● Single-center, retrospective, cohort study between 2008 and 2013. ● 81 eclamptic women were divided into two groups: 45 (PRES group) and 36 (non-PRES group) ● PRES group- Most common- headache and visual symptoms(60%) vs Non-PRES group- only headache (50%) ● PRES group-higher body mass index value (p = 0.005), longer hospitalization time (p = 0.001), and higher level of proteinuria (p = 0.012). ● Non-PRES group had higher Apgar scores (p = 0.002) than those in the PRES group. ● Conclusion:PRES manifests predominantly with headache and visual impairment together. Adverse neonatal outcomes are also common in these patients.

  • 8. AIMS: ● To compare the clinical and radiological characteristics of PRES with those without PRES in patients with pre-eclampsia/eclampsia and attempts to find independent predictors of PRES. ● Early recognition of pre-eclamptic/eclamptic women at risk of developing PRES.
  • 9. Methods and materials: ● Single centre observational study ● conducted in unit two of the obstetrics and gynecology department during 2018-2021. ● Inclusion criteria - Women diagnosed with pre-eclampsia/eclampsia in the antepartum or postpartum period. ● Diagnosis of pre-eclampsia-criteria set by the American College of Obstetricians & Gynecologists (Table no.1) The occurrence of manifest seizures confirmed the diagnosis of eclampsia in these patients. ● Exclusion criteria: History of any neurologic disorder Systolic blood pressure ≥140 mmHg or diastolic blood pressure ≥90 mmHg on at least two occasions at least 4 h apart after 20 weeks of gestation in a previously normotensive patient AND the new onset of one or more of the following: Proteinuria ≥0.3 g in a 24-h urine specimen or protein/creatinine ratio ≥0.3 (mg/mg) (30 mg/mmol) in a random urine specimen or dipstick ≥2+ if a quantitative measurement is unavailable Platelet count <100,000/microL Serum creatinine >1.1 mg/dL (97.2 micromol/L) or doubling of the creatinine concentration in the absence of other renal diseases Liver transaminases at least twice the upper limit of the normal concentrations for the local laboratory Pulmonary edema New-onset and persistent headache Visual symptoms (eg, blurred vision, flashing lights or sparks, scotomata) Table 1: Criteria for diagnosis of pre-eclampsia (ACOG 2020).
  • 10. ● Detailed obstetrics history ● Calculate period of gestation from LMP and ultrasonography ● Detailed clinical examination ● B.P monitoring ● Symptoms suggestive of PRES were noted ● Assessment of sensorium by glasgow coma scale ● Brain imaging: to exclude alternate diagnosis and confirm PRES (MRI or MR angiography) Diagnosis of PRES- presence of vasogenic edema, characterized by T1 iso/hypo and T2W/FLAIR hyper-intensity and the absence of secondary causes. ● Management:No randomized controlled trials on the interventions for treating PRES. The mainstay of management is supportive care, hydration, dyselectrolytemia correction along with control of hypertension. It is important to expedite the delivery of the fetus and placenta in the pregnant patient.

  • 17. MRI brain axial section showing FLAIR (B) hyper-intensities in the parieto- occipital region extending to frontal lobes, without corresponding diffusion restriction (B & C), suggestive of vasogenic edema in a patient with eclampsia. MRI was suggestive of PRES.
  • 18. Predictors of PRES: ● Multivariate logistic regression analysis ● On regression analysis, eclampsia was found to be the independent predictor of PRES (OR 20.9; 95% CI 3.0-147.0, p = 0.02). Age (OR 1.3; 95% CI 0.95-1.67, p = 0.12) and parity (OR 6.8; 95% CI 0.5-91.3, p = 0.15) did not predict PRES.

Conclusion: ● In this study, patients with PRES were younger and significantly more likely to be primiparous and have eclampsia ● Headache followed by seizures and altered sensorium were the most common clinical manifestations ● Subcortical white matter hyperintensities involving fronto-parieto-occipital lobes were the most common radiological finding. ● Eclampsia emerged as an independent risk factor for PRES.
