Mobile applications are promising tools for strengthening service quality and have been an area of considerable mHealth innovation. Despite growing demand for data to guide policymakers, donors, and program managers in making sound investments, there is a paucity of evidence on the cost-effectiveness of mHealth technologies. To address this gap, the HFG Project analyzed a mobile decision support tool with the following objectives: First, it aimed to provide a transparent and detailed methodology for categorizing the costs of building, deploying, and scaling-up mobile decision support tools in Malawi. Second, it evaluated the incremental cost-effectiveness of a mobile tool’s use in improving clinical care. Finally, the evaluation addressed challenges faced in conducting cost-effectiveness analyses of mHealth interventions when they are scaled up and become multifunctional.

Patient health, community health, and environmental health are all negatively affected by multiple aspects of hospital construction, design, and maintenance. Increasing numbers of studies have found correlations between the materials used in hospitals and human disease. For instance, volatile organic compounds like formaldehyde, acetaldehyde, naphthalene, and toluene are released into the air from the particle boards and carpets. These toxins, breathed in by patients and hospital staff, have been correlated to longer patient recovery times and more sick days for staff. The inadequate ventilation found in most hospitals also contributes to the poor indoor air quality and pollution. The production of polyvinyl chloride (PVC), widely used in the production of IV bags, plastic tubing, and other health care supplies, releases the carcinogen dioxin, which has been associated with a number of other health problems, including developmental defects, endometriosis, learning disabilities, and endocrine disorders. Mercury – a component of thermometers, blood pressure cuffs, and other supplies – is widely found in hospital waste released into the air and water. Other chemicals used to clean and maintain hospitals add additional toxins to the environment.

Introduction
Health care reform has challenged the state of Oregon and its regional health care providers. These challenges have created the need to approach strategic planning from a long-term and short-term perspective. Legacy Health is positioned at the center of Oregon’s reform challenges and opportunities. Transformation is an organizational reality. A strength-based analysis supports discovery into our readiness to engage current and future challenges. This paper offers a summary of organizational strengths, weakness, external opportunities and threats as well as suggestions for strategic planning.

A SWOT analysis is a valuable tool used by organizations to brainstorm on where the organization is doing its best and where the organization needs to improve (Chapman, 2012). By doing a assessment of Legacy Health structure and policies, the following SWOT analysis has been developed.
Legacy Health’s Strengths
Legacy Health is an organization with many locations throughout Oregon and Southwest Washington, operating in different areas of health care consisting of hospitals, clinics, imaging locations, laboratory, drug testing sites, physical therapy locations, and hospice (legacy health, 2012). By providing the different services, Legacy Health can meet the needs of the community in different ways, focusing on improving the health of its employees and the communities it serves.
Legacy Health’s Weaknesses
Although Legacy Health has a large percentage of bachelor degree prepared nurses (52.59%) there is a low percentage of masters prepared nurses (3.51%) this could be considered a weakness because higher educated nurses use evidence-based practice to provide the most up-to-date and quality care for patients (Altmann, 2011). By increasing the amount of master’s prepared nurses the organization can ensure that the safest and quality care to the communities that they serve.
External Opportunities
Marriner-Tomey (2009) punctuates the importance of an assessment of external factors that create organizational opportunities. Aligning a thoughtful assessment and a responsible plan positions Legacy Health to respond to historical, existing, and future external factors in a way that can have a positive influence on our resources and our outcomes.
Technological
Legacy Health has implemented EPIC as the electronic medical record (EMR) in 2011 in all six hospitals as part of the meaningful use incentive promoted by Obamacare. Legacy’s strategic plan includes optimizing EMR use by timely updates on the EPIC. Legacy is slatedto implement its biggest upgrading of EPIC version in spring 2014. Recent JOINT .

As technology continues to peak at a high rate, there is a need for the hospital to undertake a strategic change to optimize the available opportunities and reduce the negative impacts they are currently facing. The strategy seeks to empower the hospital`s mission and vision in a way that heightens strong commitment to evidence based care practices. One of the notable shortcoming of the hospital is that it lacks a strong workforce that is actively involved in decision making and thus, hinders growth and development. It is key to note that, collaboration and innovation are currently not completely favored by the leadership team and thus hindering other employees to air their thoughts about the business.

The first six months should be spent recruiting and hiring an enrollment manager to drive the enrollment initiative. Recruitment costs are expected to be approximately $100,000 over this six month period.Once the enrollment manager is hired, he or she should concentrate enrollment efforts on businesses near the Lawson and Pratt City clinics, which require only 448 more members to reach breakeven. During this time, the enrollment manager should also begin organizing Health Awareness presentations for those employers who enroll. This will include ensuring that there is sufficient staff to support this initiative as well as deciding what rewards would be appropriate for their participation in this program.
Beginning in year 2, the enrollment campaign is expected to increase enrollment by 50% annually. During this time, Lawson and Pratt City will continue to grow past the breakeven enrollment level, we have estimated that each clinic can serve over 2,000 members based on an average of 3 visits per member. The average number of primary care visits per year in the area is estimated to be 1.5 visits per person, so the estimate we’ve used is conservative and leaves some room for higher than expected utilization to occur without overcrowding the clinics. By the end of year 2, enrollment levels should have more than doubled from base year to 5,000 members.The goal in year 3 is for all CCP clinics to reach or exceed breakeven enrollment. By the end of this year, every clinic will be a self-sustaining entity of Jefferson Health System. With continued efforts, the plan will cover more of the low-income population and provide them with access to healthcare, ultimately improving community health.
