This document provides an overview of Gordon's Functional Health Patterns which are used to assess and classify client health issues. It lists over 100 health perception and management concerns, nutritional-metabolic concerns, elimination concerns, and activity-exercise concerns. Each concern is accompanied by a code number and page range reference. The patterns cover a wide breadth of potential client problems from risk of falls to ineffective breastfeeding to impaired wound healing. They provide a framework to systematically evaluate client health status and needs.
Assessment is the first step of nursing process.

This includes data about person’s physical and psychological status or study of the patient as a whole to identify his strengths and weakness and his needs and problems
Nursing assessment does not focus upon disease as do medical assessment. It is based on a board scientific knowledge, keen observation and purposeful listening.
