This document provides a summary of the most frequently cited serious OSHA violations from October 1, 2013 to September 30, 2014 organized by topic. The top violations were in general industry and included failures to have: a written hazard communication program, proper machine guarding, adequate hazard communication training, eye washing facilities, and proper use and installation of electrical equipment. Other common serious violations involved fall protections, means of egress, powered industrial vehicles, lockout/tagout procedures, and hearing conservation programs.
