Form Personal Assessment Wheel

This document contains an assessment to measure satisfaction across 9 areas of life. Participants rate statements about each area on a scale from 1 to 10. Total scores are calculated for each area and converted to percentages to plot on a wheel diagram. The wheel shows an overall picture of life balance. Participants identify the area most wanting improvement and answer questions to help expand satisfaction in that area.
