3. Introduction
Exercise ↑Metabolism ↑Cardiopulmo
nary work
The muscles contraction ↑O2 transport to
appropriates the work load exercising
(exercise) muscles
* Exercise = Stressor of Cardiovascular and Respiratory Systems
Exercise is limited by dyspnea/fatigue/chest pain if a person has
some the systems abnormalities.

4. Test of Cardiopulmonary Endurance
• Maximal O2 uptake or maximal O2
consumption (VO2max) is a valid mean of
determining a person’s MAXIMAL
AEROBIC POWER. It is first choice in
measuring to assess a person’s
cardiopulmonary fitness.
• VO2max test measure the actual
oxidative capacity of the individual.
Padkao T 4
5. Maximum O2 Uptake (VO2max)
Ideally, any test of VO2max should meet at least
the following general requirements:
1. The exercise must involve large m. groups.
2. The work rate must be measurable and
reproducible.
3. The test conditions must be such that the
results are comparable and reproducible.
4. The test must be tolerated by any healthy
individual.
5. The mechanical efficiency (skill) required to
perform the task should be as uniform as
possible in the population to the tested
Padkao T 5
6. Methods of Testing VO2max
• DIRECT
Maximal (exhaustive) graded exercise: with
direct VO2 measurement
• INDIRECT
Submaximal exercise: without direct
VO2max measurement
Prediction of VO2max using
nomogram or equation
Assumptions: such as a linear relationship
between HR, VO2max and work capacity
Padkao T 6
7. Criteria used to indicate
the attainment of VO2max
• Oxygen uptake (VO2) reaches a plateau
and does not increase with further
increase in work load.
• Heart rate is equal to age related
maximum (220-age) ± 10 beats/min
• Respiratory exchange ratio greater than
1.10
• Volitation exhaustion
• Blood lactate levels of 90 to 100 mg/dl
(10.1 to 11.2 mM/L) in young adult
Padkao T 7
8. Objectives of Exercise Stress Test
• Diagnose and Assess Severity of Disease
• Evaluate Functional Capacity
such as guild to select athletics, predict
a competition
• Evaluate Treatment
such as drug using, OR, diet control
• Plan an Exercise Prescription
Padkao T 8
9. Type of EST
1. Progressive Multistage Test or Graded
Exercise Test:
(Maximal Oxygen
Consumption: VO2max)
Padkao T 9
10. Type of EST
2. Steady-state test:
Padkao T 10
11. Characteristics of the Exercise Workload
Work = Force x Displacement (W=F*S)
unit: kilopond-meters (kpm)
Power = Work /Time (P=W/t)
unit: kpm/min, watt
1 MET = 3.5 mLO2/min/kg
Target Heart Rate = (220 – Age) x 0.85
, unit = beat/min
Padkao T 11
12. EST Protocols
• Treadmill Test
• Bicycle Ergometer Test
• Arm-Crack Ergometer Test
• Stepping Test
• Time Walk Test
Padkao T 12
13. How to select these tests
Depend on:
- The clinical question being asked
- Available equipment
- Having a facility that is able to manage
the test protocols
- Activity or exercise that will do in the
future
Padkao T 13
15. Treadmill Test
• Treadmill test is a test (sometimes called the
exercise treadmill test or exercise test) in
which a continuous electrocardiogram (ECG or
EKG) recording of the heart is made as the
patient performs increasing levels of exercise
on the treadmill which is tilted to produce the
effect of going up a small hill.
• Treadmill test provides a more common from of
physiologic stress (e.g. walking) in which subject
are more likely to attain a slightly higher
VO2max and peak HR than cycle ergometer
testing.
Padkao T 15
16. Treadmill Test Protocols
• Naughton; Best for deconditioned person
• Balk; Best for normal, inactive person
• Bruce; Best for active, young person
• Each protocol has advantage and
disadvantages. For example:
Bruce protocol – benefits to ischemic
ECG responses and means the person
must tolerate the increased exercise
level
Padkao T 16
18. For detect:
- Exercise capacity: VO2 max , work rate
maximum
- Cardiovascular function: oxygen pulse,
aerobic threshold
- Ventilatory function: ventilator reserve,
maximum RR
- Gas exchange: dead space ventilator,
oxyhemoglobin saturation
Padkao T 18
19. Exercise Testing: Bruce Treadmill Test
Bruce
Predicted VO2max
VO2max = Maximal O2 consumption มีหน่วยเป็น ml/kg*min
t = เวลาที่ใช้ไปทั้งหมด มีหน่วยเป็น min
Gender = เพศ: ชาย = 1 , หญิง = 2
Padkao T 19
21. Bicycle Ergometer Test
• The term “ergometry” stems from the Greek
“ergon” (work), and “metron” (measurement)
• Cycle ergometer is portable and less expensive.
• Upper body movement is minimal
– easier to measure exercise heart rate and blood pressure
• More applicable for measuring cycling
performance
• However, many people may not be accustomed to
cycling
– muscular fatigue may occur prematurely
– fail to reach true maximum cardio output on max cycle test
– 10 - 15% underestimation of true VO2 max

YMCA
22. Bicycle Ergometer Test
• 2 most common ergometer:
Electrical braked ergometer: preselected
power output remains fixed within a range of
pedaling frequencies
Weight-load, friction-type ergometer: power
output relates directly to friction resistance and
pedaling rate
• Power output expressed in kg·m/min or watt (1
watt = 6.12 kg·m/min)
YMCA Padkao T 22
23. Exercise Testing: Astrand-Rhyming Cycle
Ergometer Test
• Single-stage test & Constance work rate in 6
min ergometry exercise.
• Levels of workload are separated:
1. Male – Unconditioned: 50 -100 watts (300-600 kg·m/min)
2. Male - Conditioned: 100 – 150 watts (600 – 900 kg·m/min)
3. Female – Unconditioned: 50 – 70 watts (300 – 400 kg·m/min)
4. Female – Conditioned: 75 – 100 watts (450 – 600 kg·m/min)
Padkao T 23
• Pedaling at 50 revolution per minute (rpm) in 6
min
• Target HR 125 – 170 beats/min at the 5th and
6th minutes
