Since administrative costs and medical recordkeeping account for nearly 13 percent of U.S health care spending, improving medical record keeping systems has been targeted as a major path to cost savings and even higher quality health care. Originally, the IOM called them computer-based patient records.1 During their evolution, the EHR have had many other names, including electronic medical records, computerized medical records, longitudinal patient records, and electronic charts. All of these names referred to essentially the same thing, which in 2003, the IOM renamed as the electronic health records, or EHR.

Result management
Computerized results can be accessed more easily (than paper reports) by the provider at the time and place they are needed.

· Having electronic results can allow for better interpretation and for easier detection of abnormalities, thereby ensuring appropriate follow-up.

Computerized provider order entry (CPOE) systems can improve workflow processes by eliminating lost orders and ambiguities caused by illegible handwriting, generating related orders automatically, monitoring for duplicate orders, and reducing the time required to fill orders.

· The use of CPOE, in conjunction with an EHR, also improves clinician productivity.

Computerized decision support systems include prevention, prescribing of drugs, diagnosis and management, and detection of adverse events and disease outbreaks.

An electronic health record (EHR) is a computer-readable record of health-related information
on an individual. The compiled data in an EHR can include information about patient
demographics, medical history, family history, immunization records, labotary data, ongoing
health problems, progress notes, medications, vital signs, and radiology reports. Ideally, EHRs
incorporate data from all healthcare facilities a patient uses, making the data easily accessible to
healthcare professionals.
EHRs hold out the promise of improving health care and reducing costs, but for now, many
hospitals are struggling to automate the capture of raw data from the various patient monitoring
devices - such as vital sign monitors, ventilators, and electrocardiagram machines - and pass the
data directly into each patient\'s EHR. This task is made more difficult because different devices
and/or vendors often use different standards for communicating over the network. As a result,
specialized software is required to receive the data and translate it into a form suitable for
updating the EHR. Until communications standards implemented across the healthcare industry,
each new piece of monitoring equipment that outputs a nonstandard signal requires a new
interface with the EHR. So if a promising new vital sign monitoring device is developed, some
hospitals looking to use the device may be required to create a new software middleware layer to
connect the new device to the EHR. Connecting monitoring devices and EHRs is expected to
become a major business growth area over the next decade.
Many software vendors and device manufacturers are moving quickly to capitalize on the
opportunities involved with automating the many clinical-support activities that involve
minotoring devices. THe center for Medical Interoperability has enlisted many of the nation\'s
largest healthcare systems as part of its effort to strongly encourage device vendors to adopt
communications standards that will ease the problems with interoperablity. The FDA is working
to encourage the development of interoperable devices by defining some 25 device standards.
Solving the interoperability problem will require an agreement on standards through the
cooperation of multiple shareholders.

How has this change impacted the quality of care?
The change to electronic medical records has proven to be successful and helpful in providing quality patient care. The transporting of records from one physician to another is much quicker now because it can be done by a click of a button. This is speeding up the process in being able to provide quick and quality care so the patient does not need to wait as long as they would have had to in the past.

The health care delivery system in our country has its roots in the beliefs and values of the people (Shi & Singh, 2012). The firm belief in technological innovations leads to higher expectations of people, which has fueled the growth in technological innovations. The culture of individualism has led the medical practice to keep the individual healthy. Patients tend to evaluate the institutions by their acquisition of advanced technology. The expectation of Americans on what technology can do to cure illness is higher compared to the Canadians and Germans (Shi & Singh, 2012, p. 168). The main criticism leveled against electronic health records is that they don’t work perfectly.
Any technology that proves cumbersome to use may meet with disapproval and dissatisfaction
and that is the case with EHRs now. Many doctors across the United States have inhibitions
against implementing digital records that are flawed. They find themselves in a tight spot now
with the federal government warning that providers who don’t use electronic records will be
penalized. The government has been giving out $30 billion in incentives to support doctors in
installing and using electronic medical records with the objective to improve patient care.
Digitization is expected to make reviewing healthcare records, coordinating care and sharing
medical information easier and more efficient. The fact remains though, that the technology
still needs a major improvement.
Medical societies such as the AMA (American Medical Association) did notify the HHS about the
wrong direction in which the certification program was heading, elaborating on the flaws of the
current EHR systems. They highlighted the fact that many of the systems had a negative impact
on efficiency and even posed safety issues for patients. The CMS assured that it would ease
reporting burdens on physicians vide a proposed rule that was to be introduced this spring.
However, this proposed regulation would not eliminate penalties.
Need for EHRs that Can Communicate with Each Other
The important thing is to bring greater communication between diverse electronic record
systems. For instance, when a patient is moved from one healthcare facility to another, it is not
easy for his/her medical records to be transferred between the EHR systems of the two
facilities just because these systems don’t “talk” to each other. Therefore until and unless that
problem is resolved, sharing patient records will remain an impasse for doctors. Paper printouts
and fax machines still remain necessary components in healthcare facilities.
As per federal statistics, 78% of office-based physicians implemented and used electronic
health records in 2013, an evident increase from 18% in 2001. Those who fail to implement the
system will face cuts in Medicare reimbursements beginning with 1% this year. More than 257,
000 doctors are being notified that they are likely to incur that penalty. The hitches associated

with EHRs such as usage/user errors, high cost, and conflicting computer systems are the major
factors that deter physicians from using the system.
Government Move to Improve the Situation
Realizing the lack of communication capability of the available systems, the government is
taking measures to set some standards regarding sharing digital information and improving
interoperability. As an incentive to providers, Medicare is starting to increase pay to hospitals
and physicians who work together and penalize those who don’t. To ensure coordination
among different doctors who may be treating a patient and thereby improve care, reliable EHR
systems that feature interoperability are necessary.
Even though electronic health records have issues that need to be addressed effectively, users
acknowledge its benefits. It is important to understand that EHR systems that are installed need
timely upgrading to function flawlessly. With providers, vendors and other entities involved
realizing the need for interoperability of these systems; we can hope to see in the near future
advanced and glitch-free electronic medical record systems that can deliver real value.
