This document discusses eHealth records and patient portals. It defines personal health records (PHRs) which individuals control, and electronic health records (EHRs) which healthcare providers control. 3. Definitions
Personal Health Records - PHRs
• Paper records stored in a manila
file under the bed in a box file
etc.
• Electronic devices personal
health information is recorded
and stored on personal computer;
the most basic form of a PC-
based PHR would be a health
history created in a word-
processing program may be with
some scanned documents
• Web-based using a secure third
party web server to store health
records and health monitoring
data e.g. Health Vault

4. What is a patient Portal?
A secure access point that:-
 gives patients convenient 24-hour access to personal health
information from anywhere with an Internet connection.
 Allows patients to view health information such as:
Recent doctor visits / Discharge summaries / Medications /
Immunizations / Allergies / Lab results / Bills etc.
Some patient portals also allow patients to:
 Exchange secure e-mail with their health care teams
 Request prescription refills / Schedule non-urgent appointments
 Check insurance benefits and coverage / Update contact
information / Make payments
 Download and complete forms

‘extremely confident’ their organisation will be paper-light
by 2020.
14% of those surveyed said they are ‘not at all confident’
or ‘not very confident’ of achieving the target.
Target to have “integrated health and care records,
enabling effective co-ordination of health and social care,
by 2020.
• Some 56% said they were ‘extremely confident’ or
‘quite confident’ of achieving this, but a quarter (24%),
said they were ‘not at all confident’ or ‘not very
confident’.
• Only 28% said they were confident of giving patients
read/write access to their records, while 53% said they
were not confident.
