This study aims to describe this pathology’s scenario, compare it with cases of choledocholithiasis, and demonstrate if there are differences or similarities. Material and methods: a case-control study was performed where patients sent to gastrointestinal endoscopy with a diagnosis of benign biliary tract obstruction were analyzed between the period from January 2019 to December 2021. Results: there was no statistically significant difference between the characteristics of patients with sphincter of Oddi dysfunction and proven choledocholithiasis. Verifying the statistic revealed differences in cannulation strategies or post-endoscopic retrograde cholangiography pancreatitis was also impossible. Conclusions: type I and type II sphincter of Oddi dysfunction should be considered as the same entity and treated with the same therapy (endoscopic retrograde cholangiopancreatography with sphincterotomy). Choledocholithiasis and sphincter of Oddi dysfunction behave as similar pathological spectra, since the clinical features involved do not show relevant statistical differences.

Jamie is a 3-month-old female who presents with her mother for evaluation of “throwing up.” Mom reports that Jamie has been throwing up pretty much all the time since she was born. Jamie does not seem to be sick. In fact, she drinks her formula vigorously and often acts hungry. Jamie has normal soft brown bowel movements every day and, overall, seems like a happy and contented baby. She smiles readily and does not cry often. Other than the fact that she often throws up after drinking a bottle, she seems to be a very healthy, happy infant. A more precise history suggests that Jamie does not exactly throw up—she does not heave or act unwell—but rather it just seems that almost every time she drinks a bottle she regurgitates a milky substance. Mom thought that she might be allergic to her formula and switched her to a hypoallergenic formula. It didn’t appear to help at all, and now Mom is very concerned.
Cases like these are not uncommon. The mother was concerned and thinking her daughter may have an allergy; she changed to a different formula. However, sometimes babies have immature GI tracts that can lead to physiology reflux as they adapt to normal life outside the uterus. Parents often do not consider this possibility, prompting them to change formulas rather than seeking medical care. As in the case study above, GI alterations can often be difficult to identify because many cause similar symptoms. This same issue also arises with adults—adults may present with symptoms that have various potential causes. When evaluating patients, it is important for the advanced practice nurse to know the types of questions he or she needs to ask to obtain the appropriate information for diagnosis. For this reason, you must have an understanding of common GI disorders such as gastroesophageal reflux disease (GERD), peptic ulcer disease (PUD), and gastritis.

Many older people, with and without dementia can be all of a sudden be affected by delirium, which is defined as acute sudden confusion. When an elderly patient becomes confused very suddenly, it is important to look at all possible underlying causes. Mayne et al. (2019) state that “non-specific symptoms, such as confusion, are often suspected to be caused by urinary tract infection (UTI) and continues to be the most common reason for suspecting a UTI”. Delirium in the elderly is usually associated with lengthened hospital stays, complexed care, institutionalization, along with high mortality rates, difficulties for the caregivers and increased healthcare costs. Signs and symptoms associated with UTIs in the elderly include confusion or delirium, increased lethargy, blunted fever response, new-onset incontinence, as well as anorexia (Rodriguez-Manas, 2020). There are various risk factors associated with UTIs in a male patient. Some of these include prostatic hypertrophy, diabetes, or both, which can lead to high post-void residuals.
Testing
A confused patient who has recently become delirious should be investigated for the source of the delirium. Krinitski et al. (2021) state that “the diagnosis of UTI requires not only confirmed bacteriuria but also the presence of genitourinary symptoms, which often cannot be reliably confirmed in the many delirious individuals who are unable to adequately express themselves”. Does Hepatitis E Virus need to be Considered as a re-emerging problem in both industrialized and developing countries?
I came across an article wrhich was very interesting to me. The five authors - Zhang, Wu, Zhong, Xiao, and Dong observed that Hepatitis E (HEV) as a virus which is now considered by some scientists as an emerging issue as HEV is not only prevalent in developing countries but increasingly detected in industrialized nations (p. 275). I learned that this disease affects the liver and there are over 20 million cases resulting from HEV every year. To give a quick history about hepatitis E, in was in India, in 1978 when the first out break which hit the area and resulted in 52,000 cases and 1,700 deaths all due to water-borne HEV disease (Zhang et al. 2018). At first they did not know how or what this disease was that was killing and people become infected until a Russian scientist in 1983, by the name of Mikhail Balayan, took some stool samples and samples from autopies which had damaged livers and had been infected with this virus and studied it. His team then classified it as Hepatitis E (HEV). I learned from reading this article that there are currently eight known HEV genotypes (parts of the genetic makeup of a cell that determines the phenotype of a specific organism) (Zhang et al. 2018). Some genotypes can infect humans and some animals and some both.
What was most interesting in this article was it included the transmission, symptons, prevention, spreading, and awareness to the public about HEV. I think it is important to known especially for someone like me who wants to travel to other countries where different environments are not as santizied as the comfort we have here at home. Furthermore, HEV infection is high and shows a trend of increasing, which leads to the conclusion and proves there hypothesis that HEV is a re-emerging disease (Zhang et al. 2018).
To give a brief description about HEV, HEV can be transmitted through food consumption, blood transfusion, organ tranplants, pathogens in fecal particles from one person, uncooked foods, raw animal products, close contact with pigs and swine hunters (Zhang et al. 2018). Some of the symptons which many believe they have the cold or the flu are fever, vomiting, faintness. Some other characteristics of symptons can be jaundice, itching skin, pale stools, darkened urine, which lead to chronic HEV diseases. Prevention is mainly with a HEV vaccine, especially for paties who require blood or organ transplants. Lastly, awareness to the public about ways to protect oneself when encountering persons who may be infected, blood donation, eating handling raw foods, or animals, etc.

Theme 1
Each student will have to select and discuss a case study. The Case Study can involve a bacterial, viral, parasitic, or fungal infection or an intoxication caused by an infectious agent (such as Botulism or
Staphylococcus aureus
food poisoning, to name two).
The discussion should present information about the disease, including symptoms, pertinent diagnostic tests, laboratory (including Microbiology) and physical findings, treatments, and prognoses.

Case 10
A 22-year old woman presents at the walk-in health clinic with slight fever (38.5C), a complaint of frequent urination, burning on urination, vaginal discharge and a small lesion on the labia.
The woman reported that she was moderately sexually active and had three sexual partners in the past six months. Her last sexual contacts were about 7 days earlier. She had developed mild symptoms about 5 days earlier, beginning with a discharge from the vagina. She began having pain on urination about 3 days earlier.
Urine analysis revealed a pH of 8.2, some white cells and a few red blood cells. There was protein in the urine. A smear of the vaginal secretion showed a number of Gram-negative cocci.

Does Hepatitis E Virus need to be Considered as a re-emerging problem in both industrialized and developing countries?
I came across an article wrhich was very interesting to me. The five authors - Zhang, Wu, Zhong, Xiao, and Dong observed that Hepatitis E (HEV) as a virus which is now considered by some scientists as an emerging issue as HEV is not only prevalent in developing countries but increasingly detected in industrialized nations (p. 275). I learned that this disease affects the liver and there are over 20 million cases resulting from HEV every year. To give a quick history about hepatitis E, in was in India, in 1978 when the first out break which hit the area and resulted in 52,000 cases and 1,700 deaths all due to water-borne HEV disease (Zhang et al. 2018). At first they did not know how or what this disease was that was killing and people become infected until a Russian scientist in 1983, by the name of Mikhail Balayan, took some stool samples and samples from autopies which had damaged livers and had been infected with this virus and studied it. His team then classified it as Hepatitis E (HEV). I learned from reading this article that there are currently eight known HEV genotypes (parts of the genetic makeup of a cell that determines the phenotype of a specific organism) (Zhang et al. 2018). Some genotypes can infect humans and some animals and some both.
What was most interesting in this article was it included the transmission, symptons, prevention, spreading, and awareness to the public about HEV. I think it is important to known especially for someone like me who wants to travel to other countries where different environments are not as santizied as the comfort we have here at home. Furthermore, HEV infection is high and shows a trend of increasing, which leads to the conclusion and proves there hypothesis that HEV is a re-emerging disease (Zhang et al. 2018).
To give a brief description about HEV, HEV can be transmitted through food consumption, blood transfusion, organ tranplants, pathogens in fecal particles from one person, uncooked foods, raw animal products, close contact with pigs and swine hunters (Zhang et al. 2018). Some of the symptons which many believe they have the cold or the flu are fever, vomiting, faintness. Some other characteristics of symptons can be jaundice, itching skin, pale stools, darkened urine, which lead to chronic HEV diseases. Prevention is mainly with a HEV vaccine, especially for paties who require blood or organ transplants. Lastly, awareness to the public about ways to protect oneself when encountering persons who may be infected, blood donation, eating handling raw foods, or animals, etc.

Knowledge and skills frameworks, generally called competency frameworks, for ELT teachers, trainers and managers have existed for a few years now. However, until I created one for my MA dissertation, there wasn’t one drawing together what we need to know and do to be able to effectively produce language learning materials.
This webinar will introduce you to my framework, highlighting the key competencies I identified from my research.
