self-management of the disease.
The Diabetes Self-Management
Education Program at Saint Peter’s
University Hospital has been recognized
since 1996 by the American Diabetes
Association (ADA) for providing quality
self-management education and support.
The recognition means that the Saint
Peter’s program, which is taught in
multiple Saint Peter’s affiliated sites,
meets the national standards for diabetes
self-management education.
The ADA most recently recognized the
Diabetes Self-Management Education
Program at Saint Peter’s Physician
Associates in Monroe and Somerset,
and at the Saint Peter’s (Adult) Family
Health Center, in New Brunswick. The
program’s primary site is Saint Peter’s
University Hospital at the Thyroid and
Diabetes Center, in pediatric out-patient
endocrinology, and at the Perinatal
Center women’s high-risk clinic.
A team approach
The Saint Peter’s program is coordinated
by Carol Schindler and Therese Wyman,
both registered dietitians and certified
diabetes educators. The Certified Diabetes
Educator (CDE) certification mandates
expertise in the knowledge of diabetes,
teaching and counseling. The Saint Peter’s
CDE team includes registered nurses,
registered dietitians, and licensed clinical
social workers. Together they provide a
coordinated care plan specific to the
patient’s individual needs.
The ADA Education Recognition program
began in the fall of 1986, and is based
on the National Standards for Diabetes
Education that were developed by the
National Diabetes Advisory Board and
endorsed by the diabetes community.
The recognition program has grown to
become the leading quality assurance
measure for diabetes self-management
education programs across the country.
Programs are recognized for having a staff
of knowledgeable health professionals who
can provide state-of-the-art education about
diabetes self-management.
“The goal is to provide you with education
that will empower you to self-manage your
daily diabetes care,” says Carol Schindler,
RD, CDE, the program’s co-coordinator.
“Education is essential to helping patients and
families understand how adopting a healthy
lifestyle helps them to control the disease.”
How the program works
Patients begin the program with an initial
assessment consultation designed to give
educators an understanding of their specific
needs. “We begin with an initial assessment
so the patient and instructor together will
develop an individualized education and
support plan focused on behavior changes
they have identified,” says co-coordinator

physician prescribed
pump training which
is not found in many
diabetes education
programs.
Participants also meet
with a registered
dietitian, who is a
certified diabetes
educator, to learn how to plan meals and
for follow-up diabetes education and
ongoing support.
“The certified diabetes educators work
in conjunction with the primary care
physician or diabetes physician specialist.
While most programs provide care from
a registered nurse and/or a registered
dietitian, our team also counts on the
expertise of a social worker,” says
Wyman. “The end result is we care for
the needs of the whole person on an
ongoing basis. There are patients who
have been part of our program since its
inception 15 years ago.”
The education sessions include:
• Diabetes physiology
• Self-monitoring blood glucose
training, interpretation and pattern
management
• Medication management – insulin,
incretin, and oral agents
• Exercise goal setting
• Healthy eating and meal planning
• Weight management
• Prevention of diabetes complications
• Stress management
• Coping skills
• Ongoing support
Follow-up education is coordinated and
encouraged for the patient’s ongoing care.
To help with healthy lifestyle adjustments the
following are offered:
• On-going diabetes support groups
• Community programs
• Consult with licensed clinical social
worker for supportive counseling and
referrals
