Dear Friends:
We at Delta Community Supports are proud to reflect on yet another year of
helping adults with developmental disabilities, children and families forge ahead
in their development. We empower them to imagine, and then create, a future
that is fuller, more rewarding and full of victories, small and large. Because to us,
leadership is about empowerment. About service. And about walking the walk
right beside every person for whom we have the honor of caring.
To empower literally means inspire, to invest in, to embolden. And when we
shape our programs and services, we keep these thoughts close in mind and
heart and strive to put them into practice, every day, every year.
We are proud to share with you here a snapshot of the many ways empowerment
changes lives. We are grateful for your role in helping us fulfill our mission and
for all you do for children and youth and their families, and for Delta
Community Supports.
In the last year alone we have:
• Empowered over 650 individuals to live more fulfilling lives in their
own communities
• Worked to re-unite children and their families through one of
our programs or services
• Invested over $32 million in programs and services that
advance our mission
• Placed 363 children into the safety of foster care
• Facilitated the adoption process, finding loving forever-families
for children
• Helped young adults live in a supported, yet
independent environment
• Provided respite for 130 families to allow them to
re-energize and re-commit to their loves one’s needs
• Placed and supported individuals in
supported employment
With your commitment and support we, together as
a community, made these achievements possible.

At Delta Community Supports, we believe
that all people deserve exceptional care. In a
nurturing environment, exceptional people are
empowered and grow in extraordinary ways. As
a mission driven agency, it is our goal to enrich
the lives of individuals and families through
outstanding community-based supports and
mutually rewarding relationships. We’ve been
guided by this philosophy since 1977, and have
proudly worked to empower individuals and
families in southeastern Pennsylvania and
New Jersey.
Delta Community Supports has worked side by
side with people living with developmental
disabilities, creating an environment and providing
support to help them thrive.
Delta Community Supports also works to ensure
that all children have a place to call home.
We help find appropriate foster care homes for
children and teens, where they can feel safe
and secure.

practice. More than 600 Delta staff members

TOP®
for Teens: Empowering
The teenage years are full of promise, but fraught with peril. Teens need
guidance, acceptance and information that enables them to shape the
positive futures they dream about. Delta is committed to empowering teens
to make their dreams come true by providing them with the tools they need
to mature into productive, happy and healthy adults. Delta’s leadership
today shapes the leaders of tomorrow.
With teens in mind, Delta is proud to introduce our newest program, the Teen
Outreach Program (TOP®
). Delta is the first certified replicator in Pennsylvania
of this nationally field-tested teen development strategy, and its goals align
with Delta’s mission. We are humbled and excited to undertake this important
program, in partnership with the Wyman Center in St. Louis.
TOP®
is a nationally recognized nine-month program for teens in 6th to
12th grade that reduces the risk of negative behavior and increases
healthy behaviors. It provides school-year support to build the leadership
capacity of teens, and increase college access and persistence. Using
curriculum-guided discussion and community service learning throughout
the school year, TOP®
discussions and activities support brain maturity as
teens are engaged in thinking about their goals, values and behaviors
during a critical time in adolescent brain development.
Delta has long track record of success, strong community ties and a
dedicated staff, which has been recognized by the state of Pennsylvania
for its success in attaining excellent program outcomes for youth in its care.
With these factors in place, we confidently undertake TOP®
and look forward
to guiding teens to mature and grow into the futures of their dreams.

Master of Oneself: TOP®
Develops Teens’ self-regulatory, social and emotional skills:
• Ability to build and maintain positive relationships
• Critical thinking about social environment, thoughts and actions
• Formation of high standards and integrity
• Communication skills and assertiveness
• Goal-setting and evaluation of self
• Analysis, problem solving, decision making and priority setting
• Belief in one’s ability, confidence, assertiveness and persistence
• Self-discipline, time management and balance
• Social competencies to choose appropriate behaviors in various circumstances
• Evaluation and integration of physically and emotionally healthy choices for oneself

Teens need guidance, acceptance and information that enables them to shape the positive futures they dream about. Delta is committed to empowering teens to make their dreams come true by providing them with the tools they need to mature into productive, happy and healthy adults. Delta’s leadership today shapes the leaders of tomorrow. With teens in mind, Delta is proud to introduce our newest program, the Teen Outreach Program (TOP® ). Delta is the first certified replicator in Pennsylvania of this nationally field-tested teen development strategy, and its goals align with Delta’s mission. We are humbled and excited to undertake this important program, in partnership with the Wyman Center in St. Louis. TOP® is a nationally recognized nine-month program for teens in 6th to 12th grade that reduces the risk of negative behavior and increases healthy behaviors. It provides school-year support to build the leadership capacity of teens, and increase college access and persistence. Using curriculum-guided discussion and community service learning throughout the school year, TOP® discussions and activities support brain maturity as teens are engaged in thinking about their goals, values and behaviors during a critical time in adolescent brain development. Delta has long track record of success, strong community ties and a dedicated staff, which has been recognized by the state of Pennsylvania for its success in attaining excellent program outcomes for youth in its care. With these factors in place, we confidently undertake TOP® and look forward to guiding teens to mature and grow into the futures of their dreams.
