This document outlines a proposed worksite wellness program for the New Jersey Department of Health to establish a culture of wellness among employees. About The Presenter
Dr. Marrero received a B.A. (1974), M.A. (1978) and Ph.D. (1982) in Social Ecology from the University of California, Irvine. He joined the IU School of Medicine in 1984 and became the J.O. Ritchey Professor of Medicine in 2004. He was a member of the Diabetes Research & Training Center and served as Director of the Diabetes Prevention and Control Division. He is currently the Director of the Diabetes Translational Research Center. Dr. Marrero is an expert in the field of clinical trails in diabetes and translation research which moves scientific advances obtained in clinical trails into the public health sector. He helped design the Diabetes Prevention Program and the TRIAD study, which evaluated strategies to improve diabetes care delivery in managed care settings. His research interests include strategies for promoting diabetes prevention, care settings, improving diabetes care practices used by primary care providers, and the use of technology to facilitate care and education. Dr. Marrero was twice awarded the Allene Von Son Award for Diabetes Patient Education Tools by the American Association of Diabetes Educators, nominated to Who’s Who in Medicine and Health care in 2000, served as Associate Editor for Diabetes Care (1997-2002) and is currently the Associate Editor for Diabetes Forecast. He was selected as Alumni of the Year for University of California Irvine in 2006 and The Outstanding Educator in Diabetes in 2008 by the American Diabetes Association. He is the current President of the American Diabetes Association.

This study examined the prevalence, awareness, treatment and control of diabetes among elderly persons aged 60 years and older living in an urban slum in Delhi, India. The study found that 18.8% of participants had diabetes based on fasting blood sugar levels. Only 36% of diabetic participants were aware of their condition, about half of those aware were on treatment, and three-quarters of those on treatment had their diabetes under control. The study highlights the need for improved screening, diagnosis and treatment of diabetes among the elderly in urban slums.

Diabetes mellitus in Malaysia: Nation's strategies for control
Diabetes mellitus is a major global public health problem. The rise in global prevalence is expected to reach 5.4% or 300 million worldwide by 2025, with developed countries carrying a larger burden (1). Malaysia is not spared from this phenomena, with an alarming rise in prevalence of Type 2 diabetes mellitus (T2DM) over the past fifteen years, from 8.3% (NHMS 1, 1996) to 20.8% (NHMS IV, 2011) (2). What is most worrying is the figure for undiagnosed diabetics, which recorded almost a ten-fold increase (from 1.8% to 10.1%) within the same period. The national economic burden for provision of ambulatory or outpatient care for diabetes patients alone was estimated to cost the Ministry of Health RM 836 million, which took up 2.2% of the nation’s total health expenditure for 2009 (3). The average provider cost per outpatient visit for diabetes treatment at primary care was RM393.24, compared to RM 2707.44 at Specialist diabetic clinics. Treatment at primary care health centres was also highly cost effective compared to Specialist diabetic clinics (4). Due to the chronic nature of the disease, its many related complications and the progress in medical expertise, the costs to provide health care for the this group can only be expected to escalate in years to come. Strategies to effectively treat the chronic diseases (i.e. NCDs and T2DM) have been in place since the 1990s, however, the National Strategic Planning for Non-Communicable Diseases, (NSPNCD)(5) recommends that efforts should be channeled towards primary prevention, early NCD risk factor identification and NCD risk factor intervention or “clinical preventive services”. The clinical preventive services however, need to be emphasised, as early preventive measures can reduce long-term complications and morbidity related to diabetes. The risk factors which should trigger clinicians to provide clinical preventive measures include: obesity, sedentary lifestyles, dietary indiscretions, elderly (for late onset diabetes, pancreas insufficiency), family history of diabetes (risk in offspring of one diabetic parent: 30%, both parents: 60%). The 10th Malaysian Plan : Country Health Plan aims to restructure the national healthcare financing and healthcare delivery system to ensure universal health coverage of healthcare services to be provided at minimal cost using the existing infrastructure in delivering continuity of care across programmes, across healthcare settings and across healthcare providers (6). To reduce the fragmentation of care which commonly occurs in most NCD programmes, there is a need to involve healthcare providers within the healthcare service to be orientated in their roles and contribution in providing a seamless long-term care programme. It is hoped that this effort will benefit not only the patients but also provide relevant feedback on quality of healthcare service provision by the stakeholders. This study evaluated the impact of a Certified Diabetes Educator Ambassador (CDE-A) working with primary care physicians to manage patients with type 2 diabetes. The CDE-A received initial training from endocrinologists and continued consultation. In the intervention group managed by CDE-A and primary care physicians, glycemic control and other health indices improved more than the control group managed only by primary care physicians. After one year, the intervention group saw reduced HbA1c, LDL, triglycerides, blood pressure, weight, and BMI compared to the control group. up care to prevent comorbidities. With an increased population suffering from diabetes, mainly type 2 diabetes, traditional
treatments are ineffective, and a new treatment approach should be adopted. While this is deemed a plausible solution to curb the
increase of diabetes, research indicates that 70% of quality improvement initiatives fail within twelve months of implementation
(O'Donoghue et al., 2021). Therefore, stakeholders must follow proposed improvements methods closely to achieve meaningful
and sustainable change. To combat widespread chronic diseases such as diabetes, strategies such as self-management support,
intensified treatment, encouraged physical activity, and patient education plays a crucial role in managing a patient's condition.
The disease heavily relies on one self-management abilities. The proposed strategies aim to achieve patient adherence to prevent
other health effects that can be otherwise be contained and ensure that mental distress often experienced by diabetes patients is
adequately dealt with.

QI strategies may play a vital role in supporting the system and

application in public health settings.
Quality improvement in public health involves systematically
evaluating public health programs, practices, and policies and
addressing areas that need to be improved to increase healthy
outcomes. Although QI methods and techniques have only
recently been applied to public health, public health systems offer
a wide range of opportunities for implementing, managing, and
evaluating QI efforts.
The growing field of Public Health Systems and Services Research
(PHSSR) offers the potential to contribute to and support QI efforts
in public health. PHSSR examines the delivery of public health
services within communities as well as the outcomes that result from
dynamic interactions within the public health system. By examining
the public health system, stakeholder interactions, delivery of services,
and outcomes, PHSSR can inform and support the implementation
of QI initiatives.
Most recently, national, state, and local levels have made notable
progress in quality improvement in public health.1, 2 One initia-
tive credited with achieving progress is the Multi-State Learning
Collaborative (MLC). The MLC aims to inform the national accredi-
tation program, incorporate quality improvement practice into pub-
lic health systems, promote collaborative learning across states and
partners, and expand the knowledge base in public health.
Bringing together state and local practitioners and other stakeholders
in a community of practice to achieve MLC goals has yielded several
best practices and lessons for public health stakeholders. However,
more work is needed if QI is to become standard practice in public
health—particularly in understanding health departments’ readiness
for change, building the evidence base for effective public health QI
practices in the context of the public health system, and examining the
sustainability of successful projects, and identifying the determinants
of transformational change.

A researcher-developed questionnaire derived from the literature review and Hypertension Self-Care Activity Level Effects (H-SCALE) adapted from Warren-Find low and Seymour (2011) was used to measure knowledge and practice of salt and diet modification among the participants. Data gathered from participants were expressed using tables and percentages while research questions were answered with descriptive statistics of mean and standard deviation through statistical package for the social science software version 21.
Results
: the study revealed that higher percentage of the participants (81.6%) had poor of knowledge of salt and diet modification pre-intervention, also 92.1% of the participants reported poor practice before intervention. Intervention was given to the participants and results showed a positive change in knowledge and practice of salt and diet practice post-intervention.
Conclusion
: regular training should be given to hypertensive patients by nurses to improve their knowledge and practice of salt and diet modification for effective blood pressure control.

Introduction
The burden of hypertension and other noncommunicable diseases is rapidly increasing and this poses a serious threat to the economic development of many nations. Hypertension is a global public health challenge due to its high prevalence and the associated risk of stroke and cardiovascular diseases in adults.
Globally, hypertension is implicated to be responsible for 7.1 million deaths and about 12.8% of the total annual deaths (World Health
Organization (WHO), 2018). Africa, among other WHO regions was rated highest with increased prevalence of high blood pressure, estimated at 46% from age 25 years and above in which Nigeria contributes significantly to this increase (Okwuonu, Emmanuel, & Ojimadu 2014; Ekwunife, Udeogaranya, & Nwatu, 2018; WHO, 2018). This is so in spite of the availability to safe and potent drugs for hypertension and existence of clear treatment guidelines, hypertension is still grossly not controlled in a large proportion of.

Health and Wealth Coaching
The aim of this study was to investigate if a HWC program conducted by coaching trainees in a university/worksite setting would have a positive impact on participants’ health and well-being. Moreover, we wanted to evaluate the effects of HWC in wellness scores when face-to-face meetings and additional social-embedded support activities are offered to participants. HWC trainees in CtbW used several coaching strategies including coaching role definition, patient centeredness, visioning, participant self-determined goals through self-discovery, promotion of self-mastery and growth mindset, strengths support, accountability and ownership setting, intrinsic motivation, and supporting environmental and social activities.

Significance Research studies have shown that chronic disease is related to sedentary work conditions. Employees spend 36% of their total waking hours at work in sedentary conditions.
