This document summarizes evidence challenging common myths about geographic variation in health care costs and utilization. It shows that differences in poverty and income levels across regions explain much of the observed variation. Higher poverty areas have significantly higher rates of hospital utilization and costs, even when comparing regions with similar income levels. Efforts to lower national health spending based on geographic averages may harm safety net providers and low-income patients in high poverty areas.
The UN rejected claims for compensation from victims of the Haiti cholera epidemic, citing immunity from legal claims under conventions. Families are demanding millions in damages, accusing the UN of introducing cholera through poor sanitation at a peacekeeping camp. A lawyer for the victims said the case will now be pursued in Haitian courts, challenging the UN's right to immunity when it has not established an alternative accountability mechanism as required.
Physician-Hospital Integration Strategies to Maximize the Bottom Line for Ort...Wellbe
Economic pressures on physicians and hospitals have increased attention on integration and collaboration between providers. The passage of the Patient Protection and Affordable Care Act (ACA) has propelled physician-hospital integration onto the national stage, forcing physicians and hospitals to change and accelerate their alignment structures with each other to meet the ACA’s mandates: quality excellence, population health management, efficiency, and cost savings. This presentation provides a detailed overview of strategies for the orthopedic service line that have been successfully implemented in order to achieve the Triple Aim while enhancing alignment with the service line’s physicians.

The document discusses trends in Calvert County, Maryland related to population growth, demographics, housing, and poverty. While the county has experienced growth and rising incomes overall, there are also signs of increasing inequality. Specifically, despite income gains, poverty has also risen. Additionally, the African American population has declined as a percentage of the total. The document considers how recovery efforts and investments can help address inequality and work towards true sustainability and equitable opportunity for all residents.
This presentation covers the key findings of the "Household Balance Sheet" section of our report on the State of Lending in America and it's Impact on U.S. Households.
This document argues that providing a $4.6 billion tax break for the wealthiest New Yorkers is a bad deal for the state. It notes that $4.6 billion could fund many important priorities instead, like education, healthcare, infrastructure, and closing the state's budget deficit. The document provides statistics showing growing income inequality in New York and that the wealthiest residents already pay a lower overall tax rate. It concludes that most New Yorkers support raising taxes on the highest incomes instead of cutting important services.
This is a great presentation of the choices before New Yorkers. Westchester for Change will be hosting a budget briefing in early February. We will have more details soon. We welcome you to join us for the briefing and follow up actions.

Cell Therapy Expansion and Challenges in Autoimmune DiseaseHealth Advances
There is increasing confidence that cell therapies will soon play a role in the treatment of autoimmune disorders, but the extent of this impact remains to be seen. Early readouts on autologous CAR-Ts in lupus are encouraging, but manufacturing and cost limitations are likely to restrict access to highly refractory patients. Allogeneic CAR-Ts have the potential to broaden access to earlier lines of treatment due to their inherent cost benefits, however they will need to demonstrate comparable or improved efficacy to established modalities.
In addition to infrastructure and capacity constraints, CAR-Ts face a very different risk-benefit dynamic in autoimmune compared to oncology, highlighting the need for tolerable therapies with low adverse event risk. CAR-NK and Treg-based therapies are also being developed in certain autoimmune disorders and may demonstrate favorable safety profiles. Several novel non-cell therapies such as bispecific antibodies, nanobodies, and RNAi drugs, may also offer future alternative competitive solutions with variable value propositions.
Widespread adoption of cell therapies will not only require strong efficacy and safety data, but also adapted pricing and access strategies. At oncology-based price points, CAR-Ts are unlikely to achieve broad market access in autoimmune disorders, with eligible patient populations that are potentially orders of magnitude greater than the number of currently addressable cancer patients. Developers have made strides towards reducing cell therapy COGS while improving manufacturing efficiency, but payors will inevitably restrict access until more sustainable pricing is achieved.
Despite these headwinds, industry leaders and investors remain confident that cell therapies are poised to address significant unmet need in patients suffering from autoimmune disorders. However, the extent of this impact on the treatment landscape remains to be seen, as the industry rapidly approaches an inflection point.

How Race, Age and Gender Shape Attitudes Towards Mental HealthThinkNow
Mental health has been in the news quite a bit lately. Dozens of U.S. states are currently suing Meta for contributing to the youth mental health crisis by inserting addictive features into their products, while the U.S. Surgeon General is touring the nation to bring awareness to the growing epidemic of loneliness and isolation. The country has endured periods of low national morale, such as in the 1970s when high inflation and the energy crisis worsened public sentiment following the Vietnam War. The current mood, however, feels different. Gallup recently reported that national mental health is at an all-time low, with few bright spots to lift spirits.
To better understand how Americans are feeling and their attitudes towards mental health in general, ThinkNow conducted a nationally representative quantitative survey of 1,500 respondents and found some interesting differences among ethnic, age and gender groups.
Technology
For example, 52% agree that technology and social media have a negative impact on mental health, but when broken out by race, 61% of Whites felt technology had a negative effect, and only 48% of Hispanics thought it did.
While technology has helped us keep in touch with friends and family in faraway places, it appears to have degraded our ability to connect in person. Staying connected online is a double-edged sword since the same news feed that brings us pictures of the grandkids and fluffy kittens also feeds us news about the wars in Israel and Ukraine, the dysfunction in Washington, the latest mass shooting and the climate crisis.
Hispanics may have a built-in defense against the isolation technology breeds, owing to their large, multigenerational households, strong social support systems, and tendency to use social media to stay connected with relatives abroad.
Age and Gender
When asked how individuals rate their mental health, men rate it higher than women by 11 percentage points, and Baby Boomers rank it highest at 83%, saying it’s good or excellent vs. 57% of Gen Z saying the same.
Gen Z spends the most amount of time on social media, so the notion that social media negatively affects mental health appears to be correlated. Unfortunately, Gen Z is also the generation that’s least comfortable discussing mental health concerns with healthcare professionals. Only 40% of them state they’re comfortable discussing their issues with a professional compared to 60% of Millennials and 65% of Boomers.
Race Affects Attitudes
As seen in previous research conducted by ThinkNow, Asian Americans lag other groups when it comes to awareness of mental health issues. Twenty-four percent of Asian Americans believe that having a mental health issue is a sign of weakness compared to the 16% average for all groups. Asians are also considerably less likely to be aware of mental health services in their communities (42% vs. 55%) and most likely to seek out information on social media (51% vs. 35%).
AI Trends in Creative Operations 2024 by Artwork Flow.pdfmarketingartwork
Creative operations teams expect increased AI use in 2024. Currently, over half of tasks are not AI-enabled, but this is expected to decrease in the coming year. ChatGPT is the most popular AI tool currently. Business leaders are more actively exploring AI benefits than individual contributors. Most respondents do not believe AI will impact workforce size in 2024. However, some inhibitions still exist around AI accuracy and lack of understanding. Creatives primarily want to use AI to save time on mundane tasks and boost productivity.
Organizational culture includes values, norms, systems, symbols, language, assumptions, beliefs, and habits that influence employee behaviors and how people interpret those behaviors. It is important because culture can help or hinder a company's success. Some key aspects of Netflix's culture that help it achieve results include hiring smartly so every position has stars, focusing on attitude over just aptitude, and having a strict policy against peacocks, whiners, and jerks.

Google's Just Not That Into You: Understanding Core Updates & Search IntentLily Ray
1. Core updates from Google periodically change how its algorithms assess and rank websites and pages. This can impact rankings through shifts in user intent, site quality issues being caught up to, world events influencing queries, and overhauls to search like the E-A-T framework.
2. There are many possible user intents beyond just transactional, navigational and informational. Identifying intent shifts is important during core updates. Sites may need to optimize for new intents through different content types and sections.
3. Responding effectively to core updates requires analyzing "before and after" data to understand changes, identifying new intents or page types, and ensuring content matches appropriate intents across video, images, knowledge graphs and more.

3. Three Myths
of Geography and Poverty
1. Hospital Referral Regions: Variation in health care
utilization among hospital referral regions (HRRs)
is due to the overuse of supply-sensitive services.
2. Academic Medical Centers: Variation in physician
inputs among academic medical centers is a sign of
waste and inefficiency.
3. HRR Quintiles: If the entire US could achieve
spending equivalent to the lowest-spending region,
30% of health care spending could be saved.
4. “Regional differences in poverty and income
explain almost none of the observed variation.”
Skinner and Fisher 2009
5. The Inconvenient Truth
*************************
Geographic variation in health care is principally
the result of geographic differences in poverty.
Payment changes made according to geographic
norms will harm to low-income patients and the
providers who care for them.

30. Myth #2
Dartmouth’s Quintiles
“The 30% Solution”

the country could cut perhaps 20 to 30 percent
off its health care bill, a tremendous saving.”

35. Dartmouth Doubletalk
Outcomes were no different because differences
could not be discerned.
But since outcomes were no better, spending in
“high spending” “regions” must have been wasted.
And because this “wasted spending” could not be
explained (by them), it must have been due to an
over-supply of specialists providing low value care.

the end of life.
Given this apparent inefficiency, the supply
pipeline is sufficient to meet future needs
for physicians through 2020.”

But how do you ensure that patients were not more
severely ill at some hospitals than at others?
Dartmouth:
The study focused only on patients who died, so we
could be sure that all patients were similarly ill.
By definition, the prognosis was identical
– all were dead.
Therefore, variations among hospitals cannot be
explained by differences in the severity of
patient’s illnesses.

Geographic variation in health care is principally
related to geographic differences in poverty.
Payment changes made according to geographic
norms would result in major harm to low-income
patients and the providers who care for them.
