Rosey Signorini is a Health Outcomes Advisor for My Community Directory. Her role is to listen to the needs of the Metro South community and incorporate those needs into customizing the Simply Health directory. This will help improve discharge planning and access to appropriate health and community services. My Community Directory aims to reduce health expenditures, which reached $147.4 billion in 2012-2013, and prevent increased hospital visits through earlier intervention.

Why My Community Directory?
• Health expenditure in 2012 – 2013 - $147.4 Billion (AIHW 2014)
• Increase of 1.5% on previous year
• Governments provided $100.8 billion or 68.3%
• 2012-13 government funding fell 0.9%, first reduction in
expenditure in a decade – due to 2.4% funding cut
• Estimated Residential Population at 31 March 2014 –
23,425,700 individuals

National Electronic Health Initiatives &
Metro South Participation
• National Electronic Health Transition Authority (NEHTA) overseeing
implementation of
Personally Controlled Electronic Health Record
• Opt in system with unique individual healthcare identifier (IHI) being
assigned to individual participants who have option to control what
information is able to be viewed
• Metro South hospitals – PAH, QEII, Redland, Wynnum now on line to
send inpatient discharge summary to the PCEHR
• All Metro South hospitals and major community health centres are
able to VIEW the PCEHR

Internet Access
• In 2008-09 nearly three quarters or 72% of Australian
households had home internet access (ABS)
• In December 2013, 46% of adult Australians used the
internet on their mobile phones (Roy Morgan, Australian
Communications and Media Authority)
• Predicted in 2014 there will be more individual mobile
phone plans than there are individual people in the world
- 7.3 billion
• In 2014 Nielsen reported that for the first time the
majority of Australians of all age groups (including 55+)
with mobile phone plans owned smartphones
