INTRODUCTION
ā€¢ Third most common type of cancer and second
most frequent cause of cancer-related death.
ā€¢ Most curable form of carcinoma of the
gastrointestinal tract.
ā€¢ Usually begins as a noncancerous polyp that can,
over time, become a cancerous tumor.

MOLECULAR PATHOGENESIS
ā€¢ TWO distinct genetic pathways .
1. APC / Ī²- catenin pathway
ā–« Associated with WNT signaling pathway and the
chromosomal instability pathway.
2. Microsatellite instability pathway
ā–« Associated with defects in DNA mismatch repair
APC / Ī²- catenin pathway
ā€¢ APC ļƒ  tumor suppressor gene (5q21)
ā€¢ Downregulate growth promoting signals (Ī²-
catenin)
ā€¢ Component of WNT signaling pathway.
ā€¢ Catenins ļƒ proteins found in complexes with
cadherin cell adhesion molecules.
ā€¢ Ī²-catenin participates in the WNT signaling
pathway as a growth promoting signals.
WNT signaling pathway
ā€¢ Network of proteins that passes signals from cell
surface receptors to the nucleus through cytoplasm
leading to expression of target genes (transcription
regulator genes- c MYC)
ā€¢ Major role in controlling cell fate, adhesion, and cell
polarity during embryonic development.
ā€¢ WNT signaling is also required for self ā€“renewal of
the hematopoetic stem cells.
Chromosomal instability pathway
(CIN)
ā€¢ Increased rate of chromosome mis-
segregation in mitosis
ā€¢ Due to
ā€“ Gain / loss of chromosome ( aneuploidy)
ā€“ Gross chromosomal rearrangements (GSM)
ā€¢ Earliest event involved in CIN is APC gene
mutation ( 80%)
ā–« K-RAS mutation
ā–« P53 gene mutation
ā€¢ Late event : DCC (deleted in colonic carcinoma)

(MSI )

Satellite DNA
ā€¢ Satellite DNA is composed of
tandemly repeating DNA
( non - coding regions)
ā€¢ Tandem repeats occur in DNA when a
pattern of two or more nucleotides is
repeated.

ā€¢Microsatellite DNA :
If the number of nucleotide repeat is 1-4
ā–« Dinucleotide repeat:
ļ‚– When exactly two nucleotides are repeated.
ļ‚– Eg: ACACACAC
ļ‚– Such regions in DNA are commonly affected in
HNPCC.
ā€¢ Microsatellites are more prone to get unstable compared to
other neutral regions of DNA
ā€¢ This instability is due to many errors , most likely error is
ā–« Slippage during DNA replication .
ā€¢ Such error is normally repaired by Mismatch repair enzymes
which are encoded by MisMatch repair genes.
Defect in MMR gene
Reduced capacity of cells to repair specific types of
DNA damage
Increased rate of mutational accumulation in
microsatellite DNA

ā€¢ MSH2 (2p21) , MSH3, MSH4, MSH5, MSH6

ā€¢ As majority of microsatellites are located in the
non-coding region, these mutations are
generally silent.
ā€¢ Some microsatellites which are present in the
coding region of the gene are involved in the
regulation of cell growth , like those encoding

ā€¢ Terms like : CpG site and CpG Island?

ā€¢ What is hypermethylation ?
ā€¢ How hypermethylation leads to carcinoma?
ā€¢ CpG sites:
ā–« Regions of DNA where a cytosine occurs next to
a guanine.
ā–« DNA methylation occurs at these sites by an
enzyme called DNA methyltransferases.
ā€¢ In humans, 80 to 90% of all CpGs are
methylated.
ā€¢ This methylation results in the conversion of the
cytosine to 5-methylcytosine.
ā€¢ The remaining 10% non-
methylated CpGs are grouped in
a cluster forming CpG island ,
and is usually located in the
promoter regions towards 5ā€™ end.
ā€¢ The unique property of CpG
island is that it is unmethylated in
the germ line.
ā€¢ Methylation of CpG island within
the promoters of genes ļƒ

Microsatellite instability
MSI testing
ā€¢ MSI can be detected by PCR amplification of
microsatellite loci in DNA extracted from CRC
specimens .
ā€¢ Newer tests: Nucleic acid flourescence labelling,
laser scanning , flourescence PCR amplification .
ā€¢ To identify the risk for hereditary cancer and predict
the outcome of CRC.
ā€¢ To detect MLH1 and MSH2 germline mutations .
The Bethesda Guidelines
MSI testing is recommended in people with any of the following
features :
1. Cancer in families that meet Amsterdam criteria.
2. Two HNPCC- related cancers
3. A first degree relative with CRC and/or HNPCC- related
extracolonic cancer and/or colorectal adenoma diagnosed under
40yr.
4. Right-sided CRC with an undifferentiated pattern on HPE.
5. Signet-ring cell type CRC diagnosed under 45yr.
6. Adenomas diagnosed under 40 yr.
CLINICAL FEATURES
ā€¢ Asymptomatic for years.
ā€¢ Left sided colonic carcinomas
ā–« occult bleeding
ā–« changes in bowel habit
ā–« crampy left lower quadrant discomfort
ā€¢ Right sided colonic carcinomas
ā–« fatigue
ā–« weakness
ā–« iron deficiency anemia
(Anemia in females may arise from gynecologic causes, but it is a clinical maxim
that iron deficiency anemia in an older man means gastrointestinal cancer until
proved otherwise)
MORPHOLOGY
ā€¢ 50% ļƒ  rectosigmoid area (involvement of the
proximal colon is increasing)
ā€¢ Right-sided tumors more common in the
ā–« elderly
ā–« blacks
ā–« patients with diverticular disease
GROSS
A. PROXIMAL COLON
ā€¢ Polypoid:
ā–« Bulky mass, well-defined/ rolled margins and a sharp
dividing line with the normal bowel.
ā€¢ Ulcerative:
ā–« Less elevated surface and is centrally ulcerated
ā€¢ These tumors rarely cause obstruction
B. DISTAL COLON
ā€¢ Annular lesions producing ā€œnapkin ā€“ ringā€
constrictions and luminal narrowing .
ā€¢ These tumors can cause obstruction.

Signet ring adenocarcinoma
ā€¢ Rare
ā€¢ Microscopically : more
than 50%
intracellular mucin
ā€¢ About one third cases
are associated with
MSI
ā€¢ Worst prognosis.
Medullary carcinoma
ā€¢ Rare.
ā€¢ Common in proximal colon .
ā€¢ Occurs in elderly.
ā€¢ Microscopic : Sheets of malignant cells with
vesicular nuclei , prominent nucleoli, abundant
pink cytoplasm.
ā€¢ Invariably associated with MSI .
ā€¢ Favourable prognosis .
Serrated adenocarcinoma
ā€¢ 7.5% of all CRCs.
ā€¢ Common in proximal colon.
ā€¢ Derived from serrated
adenoma.
ā€¢ Microscopic:
ā–« serrated, mucinous or trabecular pattern of growth
ā–« abundant eosinophilic cytoplasm
ā–« chromatin condensation
ā–« preserved polarity, and
ā–« no necrosis.
Squamous differentiation
ā€¢ Common in proximal colon.
ā€¢ Usually associated with glandular elements
(adenosquamous carcinoma)
ā€¢ Occasionally , seen in a pure form (squamous cell
carcinoma).
ā€¢ Evidence for human papilloma virus 16
involvement in the pathogenesis of some rectal
cases .
Immunohistochemical features
ā€¢ Conventional adenocarcinoma of large bowel express are MUC1
and MUC3.
ā€¢ Mucinous carcinoma express MUC2.
ā€¢ CRCs invariably positive for cytokeratin (CK) ļƒ  positivity for
CK20 and negativity for CK7
ā€¢ Positive for CEA.
ā€¢ Positive for CDX2, in majority of CRCs.
ā€¢ Tumor-associated glycoprotein (TAG-72) is present in 100 % of
invasive colorectal carcinoma.
ā€¢ CRCs , especially poorly differentiated show loss of blood group
isoantigens and of HLA A, B, and C expression.

Diagnostic modalities
CYTOLOGYBIOPSY
Biopsy
ā€¢ There is a need of POSITIVE BIOPSY before radical
surgery for CRC.
ā€¢ In large lesions, several biopsies should be taken from
diverse areas.
ā€¢ Biopsy from center ļƒ  only granulation tissue
ā€¢ Biopsy from the very periphery ļƒ only hyperplastic
colonic epithelium
Cytology
ā€¢ Its an accurate way of diagnosing CRC.
ā€¢ Little practical value.
ā€¢ Low-lying rectal lesions can be easily sampled.
ā€¢ Brush cytology can also be performed via the
fiberoptic scope.
ā€¢ It is a sensitive technique, perhaps even more so than
endoscopic biopsy, but it has not yet found
widespread acceptance.

ā€¢ Fecal occult blood test
ā€¢ Double contrast barium enema
ā€¢ Digital rectal examination
Staging and Grading
ā€¢ In 1937, Dukes proposed staging for rectal
carcinoma .
ā€¢ In 1954, Astler and Coller proposed different
staging system.
ā€¢ American Joint committee on Cancer(AJCC)
ā€¢ The Union Internationale Countre Le Cancer
(UJCC)
Dukesā€™ Stage A
ā€¢ The tumor involve the
wall of the bowel only.
ā€¢ Treatment is surgery to
remove the tumor and
some surrounding lymph
nodes
Dukesā€™ Stage B
ā€¢ The cancer extend through the
wall has not spread to the
lymph nodes.
ā€¢ Colon cancer is treated with
surgery and, in some cases,
chemotherapy after surgery.
ā€¢ Rectal cancer is treated with
surgery, radiation therapy, and
chemotherapy
Dukesā€™ Stage C
ā€¢ The cancer has spread to the
regional lymph nodes (lymph
nodes near the colon and
rectum)
ā–« C1: regional L.N
ā–« C2: mesenteric B.V. ligature
ā€¢ Colon cancerļƒ surgery and
chemotherapy
ā€¢ Rectalcancerļƒ surgery,
radiation therapy, and
chemotherapy
Dukesā€™ Stage D
ā€¢ Spread outside of the colon
or rectum to other areas of
the body
ā€¢ Treatment : chemotherapy.

ā–« Confined to the bowel wall but with nodal metastasis

Microscopically, colorectal carcinoma can
be graded into
ā–«I ā€“ well differentiated
ā–«II- moderately differentiated
ā–«III- poorly differentiated
Spread and Metastasis
ā€¢Common sites
ā–«Regional lymph nodes
ā–«Liver
Lymph node metastasis
ā€¢ More common in the tumors showing
ā–« poorly differentiated areas
ā–« highly infiltrative pattern of growth.
ā€¢ Minimum number of nodes recovered from a
surgical specimen of colorectal carcinoma
should be 14 or 15.
Liver metastases
ā€¢ More common in the tumors showing evidence
of blood vessel invasion.
ā€¢Other relatively common
metastatic sites include
ā–«peritoneum
ā–«lung
ā–«ovaries.
PROGNOSIS
ā€¢ The 5-year survival rate after curative resection for
CRC ranges between 40% and 60% .
ā€¢ Local recurrence and/or regional lymph node
metastases occur in over 90% of the failure cases.
ā€¢ Over two-thirds of the recurrences are evident within
the first 2 years and 91% by 5 years.
ā€¢ The prognosis of colorectal carcinoma is related to a
number of clinical and pathologic parameters.
To conclude
ā€¢ Development of CRC is a complex process involving
combination of environmental and genetic factors.
ā€¢ Most carcinomas arise because of epithelial
hyperproliferation
ā€¢ It seems more genes are involved in the
carcinogenic pathway will be found to clarify
additional aspects of CRC
ā€¢ Presumed genes include p16 loss, a putative
modifier of FAP located at 1p35 and other loci of
allelic loss at as 3p22 (E-cadherin) and 16q22 (B-
catenin).

APC / Ī²- catenin pathway ā€¢ APC ļƒ  tumor suppressor gene (5q21) ā€¢ Downregulate growth promoting signals (Ī²- catenin) ā€¢ Component of WNT signaling pathway. ā€¢ Catenins ļƒ proteins found in complexes with cadherin cell adhesion molecules. ā€¢ Ī²-catenin participates in the WNT signaling pathway as a growth promoting signals.

  1. Colorectal cancer is the third most common cancer in both men and women. Colorectal cancer incidence rates have been decreasing for most of the past two decades, which has largely been attributed to increases in the use of colorectal cancer screening tests that allow the detection and removal of colorectal polyps before they progress to cancer. From 2004 to 2008, annual declines in white men were much larger than those in African American men, 2.9% versus 0.8%, respectively; whereas, among women, declines among whites (2.2% per year) and African Americans (1.7% per year) were similar. In contrast to the overall declines, colorectal cancer incidence rates have been increasing by 1.7% per year since 1992 among adults younger than 50 years of age, for whom screening is not recommended for those at average risk.
