Mental health has been in the news quite a bit lately. Dozens of U.S. states are currently suing Meta for contributing to the youth mental health crisis by inserting addictive features into their products, while the U.S. Surgeon General is touring the nation to bring awareness to the growing epidemic of loneliness and isolation. The country has endured periods of low national morale, such as in the 1970s when high inflation and the energy crisis worsened public sentiment following the Vietnam War. The current mood, however, feels different. Gallup recently reported that national mental health is at an all-time low, with few bright spots to lift spirits.
To better understand how Americans are feeling and their attitudes towards mental health in general, ThinkNow conducted a nationally representative quantitative survey of 1,500 respondents and found some interesting differences among ethnic, age and gender groups.
Technology
For example, 52% agree that technology and social media have a negative impact on mental health, but when broken out by race, 61% of Whites felt technology had a negative effect, and only 48% of Hispanics thought it did.
While technology has helped us keep in touch with friends and family in faraway places, it appears to have degraded our ability to connect in person. Staying connected online is a double-edged sword since the same news feed that brings us pictures of the grandkids and fluffy kittens also feeds us news about the wars in Israel and Ukraine, the dysfunction in Washington, the latest mass shooting and the climate crisis.
Hispanics may have a built-in defense against the isolation technology breeds, owing to their large, multigenerational households, strong social support systems, and tendency to use social media to stay connected with relatives abroad.
Age and Gender
When asked how individuals rate their mental health, men rate it higher than women by 11 percentage points, and Baby Boomers rank it highest at 83%, saying it’s good or excellent vs. 57% of Gen Z saying the same.
Gen Z spends the most amount of time on social media, so the notion that social media negatively affects mental health appears to be correlated. Unfortunately, Gen Z is also the generation that’s least comfortable discussing mental health concerns with healthcare professionals. Only 40% of them state they’re comfortable discussing their issues with a professional compared to 60% of Millennials and 65% of Boomers.
Race Affects Attitudes
As seen in previous research conducted by ThinkNow, Asian Americans lag other groups when it comes to awareness of mental health issues. Twenty-four percent of Asian Americans believe that having a mental health issue is a sign of weakness compared to the 16% average for all groups. Asians are also considerably less likely to be aware of mental health services in their communities (42% vs. 55%) and most likely to seek out information on social media (51% vs. 35%).

Specific Diagnostic Considerations

Alcohol or Drug Abuse

Anemia

Cardiac Patients

Decompression Sickness Patients
ในคนไข้กลุ่มนี้ต้องบันทึกอาการตลอดชนิดของการเจ็บป่วยตลอดจนการรักษาที่ผู้ป่วยได้รับก่อนการ
ลาเลียง และอาการของผู้ป่วยในปัจจุบัน
ความเครียดจากการบินที่จะมีผลต่อคนไข้คือภาวการณ์ลดลงของความกดดันบรรยากาศที่จะมีผล

Diabetes

Insulin และอุปกรณ์ที่ยังไม่ได้ใช้ในขณะเดินทางต้องไม่นาใส่ checked baggage ที่ต้องใส่ใต้ท้อง
เครื่องบินที่จะมีอุณหภูมิต่ามากทาให้ยาเกิดการเสื่อมคุณภาพ

HIV Patients

Infectious Disease

9. Cabin Airflow Design Principles
• Air enters at the top and exits downward and produces no longitudinal airflow. Air from one
person passes another at floor level only.
• The combination of outside air and filtered
recirculated air produces a flowrate that
removes cabin generated contaminants fast
and raises humidity
• Extracted air flows to the outflow
valve and to the recirculation filters
• Airbus aircraft use high efficiency recirculation
filters with no bypass capability - all recirculated air is filtered.
• Ventilation air used for galleys, toilets, cargo holds and avionics compartments is vented
overboard. It is not recirculated.
Airborne Precautions:

13. การทาการผ่าตัดทางสมอง

Neutropenia
ข้อมูลที่จาเป็นของคนไข้กลุ่มนี้คือปริมาณเม็ดเลือดขาว และถ้าต้องการการแยกต่างหากต้องแจ้งให้
ทราบ ความเครียดจากการบินไม่มีผลกระทบกับผู้ป่วยกลุ่มนี้นอกจากสภาพแวดล้อมการบินมีคนมากมาย
และไม่สะอาด
Obstetrics
โดยทั่วไปแล้วผู้ป่วยที่ตั้งครรภ์เพียงอย่างเดียวสามารถลาเลียงทางอากาศได้อย่างปลอดภัยจนถึงอายุ

Ophthalmology

Orthopedics

ประสาทและระบบหลอดเลือดของแขนขาที่มีปัญหา (neurovascular status of the limb) มีการใช้เครื่องยึด
ตรึงหรือไม่ (any traction is in use) เพื่อป้องกันปัญหาจากการบวมของเนื้อเยื่อ ผู้ป่วยที่กระดูกหักให้ใส่เฝือก
และทาการผ่าแยกเฝือกก่อนบิน 48 ชั่วโมง,( recent fractures should be casted and bivalved at least 48
hours prior to flight) เพราะเครื่องมือผ่าเฝือกจะไม่ได้อยู่ในชุดลาเลียง
ผู้ป่วยที่ถูกมัดขากรรไกร ซึ่งอาจจะเกิดอันตรายเมื่อมีการอาเจียนเนื่องจากการเมาเครื่องบินขึ้น
เตรียมกรรไกรตัดลวด (quick release devices applied or have wire cutters) ไว้ใกล้ๆผู้ป่วยที่มัดขากรรไกร
เพื่อช่วยเปิดทางเดินหายใจในกรณีอาเจียน
ไม่ให้ใช้การดึงรั้งด้วยการถ่วงน้าหนัก (Swinging weights are not allowed. NATO or Collins
traction is available, but must be applied by the physician. A Hare traction splint should also be
considered) ให้ใช้ Collins traction หรือ Hare traction
Pediatric
ต้องทราบว่าผู้ป่วยมีปัญหาเรื่องการหยุดหายใจหรือไม่ และอะไรเป็นสาเหตุของการหยุดหายใจ
ความเครียดจากการบินที่มีผลต่อผู้ป่วยเด็กคือ อุณหภูมิที่เปลี่ยนแปลงไป ความชื้นที่ลดต่าลง ถ้าผู้ป่วยมี
ปัญหาเรื่องการหยุดหายใจจาเป็นต้องมีเครื่องวัดออกซิเจนในเลือดและเครื่องตรวจติดตามภาวะหัวใจไปด้วย
ในกรณีเด็กอ่อนต้องมีตู้ปรับอุณหภูมิด้วย (A pulse oximeter and /or a cardiac monitor should be used for
AE. Consider the use of an ALSS incubator for neonates and small infants.)
Post-operative patients
ข้อมูลที่ต้องการคือสาเหตุที่ต้องผ่าตัด วันที่ทาการผ่าตัด มีผลแทรกซ้อนจากการผ่าตัดหรือไม่
รวมทั้งภาวะท้องอืด (ileus) สภาวะของผู้ป่วยในปัจจุบัน ตาแหน่งของแผลผ่าตัด ความเครียดจากการบินที่มี
ต่อผู้ป่วยคือการลดลงของความกดดันบรรยากาศที่มีต่อภาวะท้องอืด มาตรการแก้ไขเช่นเดียวกับในกรณีโรค

Psychiatric Patients

ต้องการการดูแลอย่างใกล้ชิด ต้องได้รับยาระงับประสาทและต้องผูกมัดอยู่บนเปล ประเภท 1 ข. (CLASS
IB) ได้แก่ผู้ป่วยโรคจิตที่มีอาการทางจิตปานกลางต้องได้รับยาระงับประสาท ลาเลียงเป็นผู้ป่วยนอนเปล
ผู้ป่วยไม่ต้องผูกมัด แต่ต้องเตรียมเครื่องผูกมัด (RESTRAINTS) ไว้พร้อมบนเปลผู้ป่วย การผูกมัดผู้ป่วยต้อง
ระวังเครื่องตรึงรัดอย่าให้ทาให้เกิดอันตรายต่อผู้ป่วยเช่นมัดแน่นเกินไป ประเภท 1ค. (CLASS IC) ได้แก่
ผู้ป่วยโรคจิตที่ไม่มีอาการทางจิต ไว้ใจได้และให้ความร่วมมือในการรักษาพยาบาล จัดเป็นผู้ป่วยนั่งและดูแล
อย่างใกล้ชิด
Pulmonary Patients
ข้อมูลที่จาเป็นสาหรับผู้ป่วยโรคปอด คือ สภาวะโรคทางปอดในปัจจุบัน ความจาเป็นในการได้รับ
ออกซิเจน การมีการคั่งของคาร์บอนไดออกไซด์ ภาวะมีลมในช่องปอด การใส่สายในช่องปอด (any need
for O2, any carbon dioxide retention, any pneumothorax, any chest tube (s), ) ในกรณีที่เป็นวัณโรคต้องมี
ข้อมูลใน เป็นการป่วยหรือสงสัย โรคยังดาเนินอยู่หรือไม่ ขณะนี้ใช้ยาอะไรอยู่และใช้มานานเท่าไรแล้ว เป็น
วัณโรคชนิดดื้อยาหรือไม่ และขณะนี้ผู้ป่วยอยู่ในระยะติดต่อหรือไม่ ความเครียดจากการบินที่มีผลต่อโรค
ทางเดินหายใจคือ การลดลงของระดับความดันย่อยของออกซิเจน ซึ่งมีผลทั้งต่อผู้ป่วยที่ใช้เครื่องช่วยหายใจ
และผู้ป่วยที่ได้รับ ออกซิเจน 100% การลดลงของความกดดันบรรยากาศ การลดลงของความชื้นในอากาศ
มาตรการแก้ไขคือ การให้ออกซิเจนระหว่างการลาเลียง ตามตาราง พิจารณาเครื่องตรวจวัดระดับออกซิเจน
การจากัดความสูงในห้องโดยสาร ถ้าผู้ป่วยมี ลมในช่องเยื่อหุ้มปอด (pneumothorax) ต้องใส่ท่อระบาย
อากาศ (chest tube(s)) ผู้ป่วยที่ใส่ chest tube ต้องมี Heimlich valve ก่อนการลาเลียง ในกรณีของ chest tube
ที่ต้องการ เครื่อง suction ให้ใช้ Pleur-Evac unit ร่วมกับ Heimlich valve โดยทั่วไปจะไม่ลาเลียงผู้ป่วย
หลังจากถอด chest tube 24 ชั่วโมง และเมื่อจะลาเลียงต้องมีการ chest x ray และต้องรวมทั้งท่า lordotic and
full inspiratory views ต้องพิจารณาการมีลมในช่องเยื่อหุ้มปอด (Free air within the chest cavity is of
concern in these patients) ให้ส่ง X ray ไปพร้อมกับผู้ป่วย ในผู้ป่วยวัณโรคให้ใช้ mask แม้ว่า ผล culture จะ
negative
การใช้เครื่องช่วยหายใจต้องมีผู้ที่สามารถจะใช้เครื่องช่วยหายใจได้ลาเลียงไปด้วยและสามารถที่จะ
reintubating the patient if necessary.
Renal Patients
ข้อมูลที่จาเป็นสาหรับผู้ป่วย คือชนิดของโรคไตที่ผู้ป่วยเป็น และตารางการทาการล้างไต
ความเครียดจากการบินที่มีผลต่อผู้ป่วยคือการลดลงของความดันออกซิเจนที่จะมีผลต่อผู้ป่วยที่โลหิตจาง
16. ผู้ป่วยที่ต้องทา Hemodialysis ต้องให้การระวังในการลาเลียง โดยการประสานแผนการลาเลียงกับตารางการ
ทา dialysis schedule ให้เหมาะสม ในผู้ป่วยที่
ทา Peritoneal dialysis จะต้องไม่ลืมที่จะนาน้ายาและอุปกรณ์ การทา peritoneal dialysis ติดตาม
ผู้ป่วยไปด้วยประมาณ 3 วัน สาหรับผู้ป่วยที่มีภาวะโลหิตจางต้องให้ออกซิเจนระหว่างการลาเลียงโดย
พิจารณาจากการตรวจวัดระดับออกซิเจนโดยใช้ pulse oximeter
Sickle Cell
ข้อมูลที่จาเป็นว่าเป็น sickle cell trait or disease และตัวกระตุ้นให้เกิดอาการ ความเครียดจากการ
บินคือภาวะลดลงของความดันออกซิเจน ซึ่งอาจทาให้เกิด sickling ความกดดันของออกซิเจนในถุงลมที่
10,000 ฟุต ต่าพอที่จะทาให้เกิดอาการ sickling ได้ ในผู้ป่วยที่จาเป็นต้องลาเลียงทางอากาศต้องให้ออกซิเจน
ระหว่างการลาเลียงรวมทั้งต้องให้ความชื้นให้พอเหมาะด้วยโดยจะใช้ทางหลอดเลือดหรือให้ทางปากก็ได้
สาหรับผู้ป่วย sickle trait ไม่มีข้อจากัดในการลาเลียงทางอากาศ
สาหรับผู้ที่มี individuals with less than 41 percent Hb-S are now being accepted for Air Force

Spinal Cord Injuries
ข้อมูลที่จาเป็นคือ ระดับและความรุนแรงของการบาดเจ็บ (the level of injury and deficit) อาการใน
ปัจจุบันตลอดจนการทาการผ่าตัดหรือไม่ ความเครียดจากการบินโดยตรงไม่มีผลต่อความรุนแรงของการ
บาดเจ็บ แต่การสั่นสะเทือนของเครื่องบินอาจทาให้การบาดเจ็บรุนแรงเพิ่มขึ้น (further neurological
compromise) ผู้ป่วยจะต้องได้รับความสนใจอย่างเต็มที่ตลอดเวลาการลาเลียงในด้าน
immobilization/stabilization ผู้ป่วย spinal cord injury, severe burn หรือผู้ป่วยที่ต้องได้รับความช่วยเหลือ
ตลอดเวลาเพื่อพลิกตัวผู้ป่วยได้ระหว่างการลาเลียง อาจต้องใช้ Stryker frame เพื่อพลิกตัวผู้ป่วยทุก 2 ชั่วโมง
ในท่านอนหงาย และทุก 1 ชั่วโมง ในท่านอนคว่า เครื่องมือนี้จะติดตั้งอย่างมั่นคงบนฐานที่เป็นไม้เพื่อลด
ความสะเทือนที่มีต่อผู้ป่วย และติดต้องบนพื้นเครื่องบินด้วย D-rings and ratchet type tie down straps
The Collins traction เป็นเครื่องมือที่ใช้ spring tension instead of hanging weights เพื่อใช้แทน
ลูกตุ้มที่แกว่งไปมาในการทา traction during flight เครื่องมือนี้ใช้ในการลาเลียงควรจะติดตั้งเครื่องมือนี้กับ
ผู้ป่วยในรพ.ต้นทางแต่ถ้าจาเป็นอาจติดตั้งก่อนการลาเลียงใน (can be applied on the flight line) และมีแพทย์
คอยดูแลระหว่างการติดตั้งด้วย
Documentation
การดาเนินการด้านเอกสารให้สมบรูณ์ก่อนการลาเลียงทางอากาศมีความสาคัญเช่นเดียวกับการ
ดาเนินการในผู้ป่วยที่อยู่ในโรงพยาบาล การจัดทาเอกสารต้องเรียบร้อยในเรื่องการให้การรักษาพยาบาล
ระหว่างการลาเลียง รวมทั้งยาที่ผู้ป่วยใช้เอง ให้ส่งรายงานประวัติผู้ป่วย x-ray, ผลการตรวจทาง
