RTFT is a new DCF grant funded program designed to work with families where child(ren) have been removed from the
care of their parents by DCF and placed into foster care. Staff, using a visit coaching model, work with parents toward
reunification with their children.
• Oversee day to day operations of the RTFT program
• Provide clinical supervision to clinicians and case managers.
• Ensure all electronic medical records (Carelogic) are up to date.
• Ensure fidelity to the “visit coaching” model
• Maintain quality assurance of the program.
• Maintain a good working relationship with DCF; responding to inquiries in a timely manner.
• Maintain good relationships with resource families (formerly called foster families) and collateral providers.
• Keep client information confidential pursuant to federal, state and HIPAA regulations.

• Responsible for the implementation/coordination of the program and day to day operations of the group home.
• Responsible for all hiring.
• Provide clinical supervision to therapist and individual/group supervision of residential supervisors/milieu staff.

• Prepare for biannual State of Connecticut, Department of Children and Families re-licensure of the group home.

• Provided intensive home-based long-term clinical and concrete services to DCF referred families with children and
adolescents identified for this service with Axis 1. Service included family therapy, parent guidance, individual
therapy with child and crisis intervention. Interface with agency psychiatrist for medication management.
• Provided intensive, family-based clinical and support services for children returning home from residential care.
Also provided these services to children residing in Treatment Foster Homes.
• Provide “24-hour on call” crisis coverage on a rotating basis.
• Facilitate/coordinate discharge planning for continued services for ongoing support for the child and family.
