Scenario:
You are the Refinery Emergency Response Coordinator for an incident at the SJV Refinery which has been in operation since 1966. The refinery processes 120,000 bbls of crude oil per day, which has a sulfur content of 2.5 percent. The refinery converts crude oil to naptha, light oil, and heavy oils using the Atmospheric/Vacuum Distillation Unit with key equipment such as the following:

naptha, kerosene, gasoline, and diesel hydrotreaters;

isomerization unit;

naptha reformer;
·
·
fluid catalytic cracker;

coker;
·
·
hydrocracker;
·

polymerization unit (petrochemical section of the refinery polymerizing olefin gases to produce polyethylene);
·

sulfur recovery Claus plant (catalytic reactors); and
·
·
distillate/gasoline blending tanks.
The refinery was initiating work on a major plant turnaround at the time of the incident to complete required maintenance repairs, mechanical integrity inspections, and modifications to existing equipment. Twenty contractor companies (approximately 150 employees) have been contracted to perform this work under the direction of refinery staff. All of the contractor workers completed the refinery orientation training.
Work for the contractor crews is assigned/scheduled each morning. On the day of the incident, the day-shift (6 am to 6 pm) crew had been tasked with isolating the acid gas feed stream for the Claus unit. Due to other work priorities, the crew did not isolate the line as planned. A shift turnover for the night contractor crew did not happen due to mandatory safety training that delayed their arrival at the worksite. Upon their arrival at the work site, the night crew held a job safety analysis (JSA) review of the scheduled task (line breaking of the acid gas feed line to replace a segment) to be performed and the hazards present. No pressure gauges or monitoring was present to indicate that the acid gas feed line was operational. The crew initiated the line breaking activity (open the line to the atmosphere) at approximately 7:45 pm under self-contained breathing apparatus (SCBA), which almost immediately resulted in the uncontrolled release of acid gas. A nearby ignition source from a welding operation ignited the flammable gas. The following actions were initially taken:

CDHPs strive to control costs and improve quality of care by requiring consumers to take control of their own healthcare decisions. CDHPs are geared to encourage participants to enroll in some type of wellness program and improve their lifestyles. What escapes many businesses when they merge is a concern for the changes in culture. As mergers progress a significant amount of research is done on financials, markets, sales operations, etc. Far less attention is paid to integration of employee/employer relationships after the details of the merger are complete (Featherly, 2006). Kevin Wilde, Chief Learning Officer at General Mills, compares culture to the water in a fish bowl. The water surrounds and sustains, but the fish remain unaware of it's exact properties. When two fishbowls merge, the resulting change is atmosphere is subtle, but still a shock to the system (Featherly, 2006).
One concern newly-merged companies have is the integration of the sales forces. When General Mills and Pillsbury merged in late October of 2001, the combined sales of the new company was estimated at $13 billion and it become the 5th largest food company, doubling it's annual sales (General Mills-Pillsbury gets the ok, 2001). This larger, more complex system took on 40 new countries and major markets (Whitney, 2004). The goal of the Chief Learning Officer on behalf of the organization was to get everyone working toward the same goals.
It's critical to engage the sales team immediately. Without revenue, the company will falter immediately. After some initial training, Wilde thought the two sales teams were working in tandem. It was quite a shock for him to find out that in a climate survey the original Pillsbury team gave very low scores to the question asking if they understood their jobs, their goals, and how to get things done (Whitney, 2004). This was clearly cause for concern.

In addressing the case study scenario, pay particular attention to the role of emerging trends (presented in this chapter) in resolving the case. President Cathryn Addy Tunxis Community College Farmington, Connecticut Converging Issues as an Opportunity for Change Background Doe Community College, located in an eastern city of 100,000, was founded in 1952. It has an enrollment of 6,780 FTE (full-time equivalent) and a headcount enrollment of slightly over 8,000 in credit classes. Noncredit enrollment is around 3,000 annually. There are 100 full-time faculty who ← 286 | 287 → teach a 12 credit load on average, 280 adjunct faculty, and approximately 60 other professionals and administrators. There is one campus of approximately 300,000 square feet on 10 acres of land, and one satellite campus 50 miles away in a rural area that opened in a renovated high school 20 years ago. Doe CC is one of 14 publicly supported community colleges in the state but gets one-third of its funding locally, drawn proportionately from each of the 17 school districts in its service area. Another one-third comes from the state according to a long established allocation formula, and the remaining one-third comes from tuition, fees, and auxiliary income from grants, noncredit enrollments, and other enterprises such as the bookstore and a small restaurant on campus run by the culinary students. Over the last three years, the local districts have become more and more vocal about no longer being able to afford to provide funding to Doe, and have been threatening to withdraw from the partnership. Colleen has been living on a farm with her aunt. Although she has no prior contact with the law, she admits to having run away from her aunt in the past, though she always returned on her own.
Colleen’s current trouble is centered on a neighbor’s dispute about her dog. After the dog bit the neighbor, the neighbor threatened and attempted to take the dog to animal control. When the neighbor later found Colleen and the dog trespassing on her property, the neighbor verbally threatened to “get” them both. Apparently, the neighbor set fire to a scarecrow on her property to emphasize her point.
At this point, Colleen threw a nearby bucket of liquid on the neighbor. The liquid was later determined to be a pesticide. The pesticide burned the skin on contact and triggered an extreme allergic reaction in the neighbor, resulting in her death. Colleen was charged with involuntary manslaughter and was determined delinquent by the court. Colleen claims that she was simply attempting to douse the fire.
Coincidentally, a few weeks prior, Colleen had been a passenger of a motor home that had collided with a vehicle driven by the neighbor’s sister. This accident resulted in the neighbor’s sister’s death, though it was ruled an accidental homicide. Witnesses report that Colleen took a pair of fancy shoes off the feet of the corpse and immediately put them on her own feet. Colleen defends her actions by claiming that an observer told her to do this.
At the time of her arrest, Colleen’s blood tested positive for opiates. Colleen claims that while traveling with three homeless men, they unknowingly stumbled upon a field of poppies and that before they knew it; the intoxicating aroma caused her to pass out. Colleen claims not to have a drug problem. Colleen states that all she wants to do now is go home and that she misses her Aunt Emily.

The US House of Representatives is deeply concerned by ongoing and pervasive acts of antisemitic
harassment and intimidation at the Massachusetts Institute of Technology (MIT). Failing to act decisively to ensure a safe learning environment for all students would be a grave dereliction of your responsibilities as President of MIT and Chair of the MIT Corporation.
This Congress will not stand idly by and allow an environment hostile to Jewish students to persist. The House believes that your institution is in violation of Title VI of the Civil Rights Act, and the inability or
unwillingness to rectify this violation through action requires accountability.
Postsecondary education is a unique opportunity for students to learn and have their ideas and beliefs challenged. However, universities receiving hundreds of millions of federal funds annually have denied
students that opportunity and have been hijacked to become venues for the promotion of terrorism, antisemitic harassment and intimidation, unlawful encampments, and in some cases, assaults and riots.
The House of Representatives will not countenance the use of federal funds to indoctrinate students into hateful, antisemitic, anti-American supporters of terrorism. Investigations into campus antisemitism by the Committee on Education and the Workforce and the Committee on Ways and Means have been expanded into a Congress-wide probe across all relevant jurisdictions to address this national crisis. The undersigned Committees will conduct oversight into the use of federal funds at MIT and its learning environment under authorities granted to each Committee.
• The Committee on Education and the Workforce has been investigating your institution since December 7, 2023. The Committee has broad jurisdiction over postsecondary education, including its compliance with Title VI of the Civil Rights Act, campus safety concerns over disruptions to the learning environment, and the awarding of federal student aid under the Higher Education Act.
• The Committee on Oversight and Accountability is investigating the sources of funding and other support flowing to groups espousing pro-Hamas propaganda and engaged in antisemitic harassment and intimidation of students. Scenario:
You are the Refinery Emergency Response Coordinator for an incident at the SJV Refinery which has been in operation since 1966. The refinery processes 120,000 bbls of crude oil per day, which has a sulfur content of 2.5 percent. The refinery converts crude oil to naptha, light oil, and heavy oils using the Atmospheric/Vacuum Distillation Unit with key equipment such as the following:

naptha, kerosene, gasoline, and diesel hydrotreaters;

isomerization unit;
·

naptha reformer;
·
·
fluid catalytic cracker;

·
coker;
·
·
hydrocracker;
·
·
polymerization unit (petrochemical section of the refinery polymerizing olefin gases to produce polyethylene);
·
·
sulfur recovery Claus plant (catalytic reactors); and
·
·
distillate/gasoline blending tanks.
The refinery was initiating work on a major plant turnaround at the time of the incident to complete required maintenance repairs, mechanical integrity inspections, and modifications to existing equipment. Twenty contractor companies (approximately 150 employees) have been contracted to perform this work under the direction of refinery staff. All of the contractor workers completed the refinery orientation training.
Work for the contractor crews is assigned/scheduled each morning. On the day of the incident, the day-shift (6 am to 6 pm) crew had been tasked with isolating the acid gas feed stream for the Claus unit. Due to other work priorities, the crew did not isolate the line as planned. A shift turnover for the night contractor crew did not happen due to mandatory safety training that delayed their arrival at the worksite. Upon their arrival at the work site, the night crew held a job safety analysis (JSA) review of the scheduled task (line breaking of the acid gas feed line to replace a segment) to be performed and the hazards present. No pressure gauges or monitoring was present to indicate that the acid gas feed line was operational. The crew initiated the line breaking activity (open the line to the atmosphere) at approximately 7:45 pm under self-contained breathing apparatus (SCBA), which almost immediately resulted in the uncontrolled release of acid gas. A nearby ignition source from a welding operation ignited the flammable gas. The following actions were initially taken:

CDHPs strive to control costs and improve quality of care by requiring consumers to take control of their own healthcare decisions. What escapes many businesses when they merge is a concern for the changes in culture. As mergers progress a significant amount of research is done on financials, markets, sales operations, etc. Far less attention is paid to integration of employee/employer relationships after the details of the merger are complete (Featherly, 2006). Kevin Wilde, Chief Learning Officer at General Mills, compares culture to the water in a fish bowl. The water surrounds and sustains, but the fish remain unaware of it's exact properties. When two fishbowls merge, the resulting change is atmosphere is subtle, but still a shock to the system (Featherly, 2006).
One concern newly-merged companies have is the integration of the sales forces. When General Mills and Pillsbury merged in late October of 2001, the combined sales of the new company was estimated at $13 billion and it become the 5th largest food company, doubling it's annual sales (General Mills-Pillsbury gets the ok, 2001). This larger, more complex system took on 40 new countries and major markets (Whitney, 2004). The goal of the Chief Learning Officer on behalf of the organization was to get everyone working toward the same goals.
It's critical to engage the sales team immediately. Without revenue, the company will falter immediately. After some initial training, Wilde thought the two sales teams were working in tandem. It was quite a shock for him to find out that in a climate survey the original Pillsbury team gave very low scores to the question asking if they understood their jobs, their goals, and how to get things done (Whitney, 2004). This was clearly cause for concern.

President Cathryn Addy Tunxis Community College Farmington, Connecticut Converging Issues as an Opportunity for Change Background Doe Community College, located in an eastern city of 100,000, was founded in 1952. It has an enrollment of 6,780 FTE (full-time equivalent) and a headcount enrollment of slightly over 8,000 in credit classes. Noncredit enrollment is around 3,000 annually. There are 100 full-time faculty who ← 286 | 287 → teach a 12 credit load on average, 280 adjunct faculty, and approximately 60 other professionals and administrators. There is one campus of approximately 300,000 square feet on 10 acres of land, and one satellite campus 50 miles away in a rural area that opened in a renovated high school 20 years ago. Doe CC is one of 14 publicly supported community colleges in the state but gets one-third of its funding locally, drawn proportionately from each of the 17 school districts in its service area. Another one-third comes from the state according to a long established allocation formula, and the remaining one-third comes from tuition, fees, and auxiliary income from grants, noncredit enrollments, and other enterprises such as the bookstore and a small restaurant on campus run by the culinary students. Colleen M.
is a 15-year-old Caucasian girl. She has lived in the state for her entire life. Her father abandoned the family when she was an infant and her mother is currently in a drug rehabilitation facility. Colleen has been living on a farm with her aunt. Although she has no prior contact with the law, she admits to having run away from her aunt in the past, though she always returned on her own.
Colleen’s current trouble is centered on a neighbor’s dispute about her dog. After the dog bit the neighbor, the neighbor threatened and attempted to take the dog to animal control. When the neighbor later found Colleen and the dog trespassing on her property, the neighbor verbally threatened to “get” them both. Apparently, the neighbor set fire to a scarecrow on her property to emphasize her point.
At this point, Colleen threw a nearby bucket of liquid on the neighbor. The liquid was later determined to be a pesticide. The pesticide burned the skin on contact and triggered an extreme allergic reaction in the neighbor, resulting in her death. Colleen was charged with involuntary manslaughter and was determined delinquent by the court. Colleen claims that she was simply attempting to douse the fire.
Coincidentally, a few weeks prior, Colleen had been a passenger of a motor home that had collided with a vehicle driven by the neighbor’s sister. This accident resulted in the neighbor’s sister’s death, though it was ruled an accidental homicide. Witnesses report that Colleen took a pair of fancy shoes off the feet of the corpse and immediately put them on her own feet. Colleen defends her actions by claiming that an observer told her to do this.
At the time of her arrest, Colleen’s blood tested positive for opiates. Colleen claims that while traveling with three homeless men, they unknowingly stumbled upon a field of poppies and that before they knew it; the intoxicating aroma caused her to pass out. Colleen claims not to have a drug problem. Colleen states that all she wants to do now is go home and that she misses her Aunt Emily.

. is a 17-year-old African American male and documented gang member. His prior juvenile adjudications include purse snatching, breaking and entering, and drug possession. His first juvenile adjudication occurred when he was 13 years old. He has served a year of custody in the juvenile correctional facility and has been placed on probation twice previously. His latest arrest is for possession of a concealed weapon, to which he has pled guilty.
Xander lives with his mother in the housing projects. He reports no contact with his father. His mother presently works two jobs. Postsecondary education is a unique opportunity for students to learn and have their ideas and beliefs challenged. However, universities receiving hundreds of millions of federal funds annually have denied
students that opportunity and have been hijacked to become venues for the promotion of terrorism, antisemitic harassment and intimidation, unlawful encampments, and in some cases, assaults and riots.
The House of Representatives will not countenance the use of federal funds to indoctrinate students into hateful, antisemitic, anti-American supporters of terrorism. Investigations into campus antisemitism by the Committee on Education and the Workforce and the Committee on Ways and Means have been expanded into a Congress-wide probe across all relevant jurisdictions to address this national crisis. The undersigned Committees will conduct oversight into the use of federal funds at MIT and its learning environment under authorities granted to each Committee.
• The Committee on Education and the Workforce has been investigating your institution since December 7, 2023. The Committee has broad jurisdiction over postsecondary education, including its compliance with Title VI of the Civil Rights Act, campus safety concerns over disruptions to the learning environment, and the awarding of federal student aid under the Higher Education Act.
• The Committee on Oversight and Accountability is investigating the sources of funding and other support flowing to groups espousing pro-Hamas propaganda and engaged in antisemitic harassment and intimidation of students. The Committee on Oversight and Accountability is the principal oversight committee of the US House of Representatives and has broad authority to investigate “any matter” at “any time” under House Rule X.
• The Committee on Ways and Means has been investigating several universities since November 15, 2023, when the Committee held a hearing entitled From Ivory Towers to Dark Corners: Investigating the Nexus Between Antisemitism, Tax-Exempt Universities, and Terror Financing. Julius Caesar’s dictatorship and subsequent assassination in 44 BCE created a power vacuum, leading to a civil war. Octavian, later Augustus, emerged victorious, heralding the Roman Empire’s birth.
Under Augustus, the empire experienced the Pax Romana, a 200-year period of relative peace and stability. Augustus reformed the military, established efficient administrative systems, and initiated grand construction projects. The empire's borders expanded, encompassing territories from Britain to Egypt and from Spain to the Euphrates. Roman legions, renowned for their discipline and engineering prowess, secured and maintained these vast territories, building roads, fortifications, and cities that facilitated control and integration.
The Roman Empire’s society was hierarchical, with a rigid class system. At the top were the patricians, wealthy elites who held significant political power. Below them were the plebeians, free citizens with limited political influence, and the vast numbers of slaves who formed the backbone of the economy. The family unit was central, governed by the paterfamilias, the male head who held absolute authority.
Culturally, the Romans were eclectic, absorbing and adapting elements from the civilizations they encountered, particularly the Greeks. Roman art, literature, and philosophy reflected this synthesis, creating a rich cultural tapestry. Latin, the Roman language, became the lingua franca of the Western world, influencing numerous modern languages.
Roman architecture and engineering achievements were monumental. They perfected the arch, vault, and dome, constructing enduring structures like the Colosseum, Pantheon, and aqueducts. These engineering marvels not only showcased Roman ingenuity but also served practical purposes, from public entertainment to water supply.
