The brachial plexus is formed from the anterior primary rami of cervical and upper thoracic spinal nerves C5-T1. It is divided into upper, middle, and lower trunks which each divide further into ventral and dorsal divisions, forming three cords - lateral, medial, and posterior. The cords give rise to various peripheral nerves that innervate the shoulder, arm, forearm, and hand.
