Mental health has been in the news quite a bit lately. Dozens of U.S. states are currently suing Meta for contributing to the youth mental health crisis by inserting addictive features into their products, while the U.S. Surgeon General is touring the nation to bring awareness to the growing epidemic of loneliness and isolation. The country has endured periods of low national morale, such as in the 1970s when high inflation and the energy crisis worsened public sentiment following the Vietnam War. The current mood, however, feels different. Gallup recently reported that national mental health is at an all-time low, with few bright spots to lift spirits.
To better understand how Americans are feeling and their attitudes towards mental health in general, ThinkNow conducted a nationally representative quantitative survey of 1,500 respondents and found some interesting differences among ethnic, age and gender groups.
Technology
For example, 52% agree that technology and social media have a negative impact on mental health, but when broken out by race, 61% of Whites felt technology had a negative effect, and only 48% of Hispanics thought it did.
While technology has helped us keep in touch with friends and family in faraway places, it appears to have degraded our ability to connect in person. Staying connected online is a double-edged sword since the same news feed that brings us pictures of the grandkids and fluffy kittens also feeds us news about the wars in Israel and Ukraine, the dysfunction in Washington, the latest mass shooting and the climate crisis.
Hispanics may have a built-in defense against the isolation technology breeds, owing to their large, multigenerational households, strong social support systems, and tendency to use social media to stay connected with relatives abroad.
Age and Gender
When asked how individuals rate their mental health, men rate it higher than women by 11 percentage points, and Baby Boomers rank it highest at 83%, saying it’s good or excellent vs. 57% of Gen Z saying the same.
Gen Z spends the most amount of time on social media, so the notion that social media negatively affects mental health appears to be correlated. Unfortunately, Gen Z is also the generation that’s least comfortable discussing mental health concerns with healthcare professionals. Only 40% of them state they’re comfortable discussing their issues with a professional compared to 60% of Millennials and 65% of Boomers.
Race Affects Attitudes
As seen in previous research conducted by ThinkNow, Asian Americans lag other groups when it comes to awareness of mental health issues. Twenty-four percent of Asian Americans believe that having a mental health issue is a sign of weakness compared to the 16% average for all groups. Asians are also considerably less likely to be aware of mental health services in their communities (42% vs. 55%) and most likely to seek out information on social media (51% vs. 35%).

Tudják-e, hogy a modern időkben az
emberek nem a fogszuvasodás és
parodontózis miatt veszítik el foga-
ikat? A pacienseknek ezen betegsé-
gei a jó minőségű otthoni fogápo-
lás és a “Recall” higiéniai program
következtében valójában már nem
létezik. Ezzel szemben növekvő ten-
denciát mutat a fogak elvesztésé-
nél a nagymértékű abrázió, atrició,
erózió, nem várt repedések, lepat-
tanások - törések, és foggyökér de-
fektek. További témakör a növekvő
tendencia a TMK betegség, okklúzi-
ós-muszkuláris szindróma, roncho-
patia, szájon keresztüli légzés, ob-
strukciós szindróma HDC és alvási
apnoé, ezek következetes megfigye-
lésében és gyógyításában jelentős
mértékben részt vehet a fogorvos.
Legalább is ezt állítja a Canadian
Institute of occlusion tanulmánya
és engem naponta meggyőz a saját
tapasztalatom azon páciensekkel
melyek betartják a szájhigiéniás ke-
zelést és a Recall rendszeres látoga-
tóik és mégis a fenti symptomákkal
állandóan szenvednek.
A fog elvesztésének a fő okozója az
izmok az állkapocs ízület és a fogak
között fellépő erővektorok kiegyen-
lítetlensége. Ugyan úgy a csontok
fejlődésének és növekedésének ide-
je alatt, mint annak befejeződése
után.
Ennek a problémának a megoldá-
sában kiemelkedő feladata van az
u.n. harmonikusan kiegyenlített
„equilibrált” harapásnak, ami a
modern ember esetében a termé-
szetben gyakorlatilag nem fordul
elő. Az University of Arkansas, Dep.
of Anthropology tanulmányának
összegzésében olvashatjuk az oko-
kat: 95% minden nyitott harapás-
nak, a genetika mindössze 5%-ban
játszik szerepet. Ez elvitathatatlan
tény. Más szóval a felső állcsont,
maxilla és az alsó álkapocs mandi-
bula, rendellenes fejlődése ontog-
enetikai jelenség, ami az étkezési
szokásokkal és életvitellel egészen
a szoptatástól kezdve, a kamuflá-
zsok használata és egyéb rossz szo-
kásokkal magyarázható. A fenti ál-
lítást bizonyítják a több mint 600
éves ásatási leletek a nem moder-
nül elkészített ételek az egyéneknél
minimális vagy semmilyen ortho-
dontikai anomáliát nem mutatnak.

Ideálisan kiegyenlített fogsorok-
hoz jutunk, amennyiben a köz-
ponti helyzete az alsó fogsornak

daciónál” zárt harapásnál. Ez az
egyetlen létező állapota az álka-
pocs ízületnek, amely lehetővé
teszi a dinamikus okklúziót min-
denféle lehetséges zavaró “inter-
ferencia” nélkül. Továbbá ez az
egyetlen létező állapot, amely le-
hetővé teszi a rágóizmok teljes rel-
axációját, főként a M.pterygoideus
lateralis, nemkívánatos állandó
zsugorodása az okozója többnyire
az okklúzió-muszkuláris problé-
mának és a diszharmonikus hara-
pásnak. Nem utolsó sorban ez az
egyetlen konstans állapot, amely-
nek valósságát ellenőrizni lehet és

Eredménye a „középpontban” a har-
monikus kiegyensúlyozott harapás-
nak az azonnali diskluzíó laterális
szakaszon és a rágó izmok teljes
nyugalmi állapota. Az első bekez-
désben megnevezett többnyire elle-
nőrizhetetlen kellemetlenséget oko-
zó diszharmonikus erőket 80%-al
csökkentjük. Így a fizikai feltételes
reverzibilis kellemetlenség megszű-
nik, a többit stabilizáljuk és az újabb
kellemetlenségek megjelenését ki-
zárjuk.
Egy megoldás a harmonikus hara-
pás eléréséhez az equilibráció. Más
szóval “artikulációs fogpreparálás
a középpontban, akkor az artiku-
lációs preparálása a fogsornak a
központi viszonya az álkapocsnak.
Az egyes számú alapvető feladat
elkészíteni egy „nem végleges” ta-
nulmánymodellt az equilibráció-
hoz. Így pontosan fogjuk tudni az
eredményt amit el szeretnénk érni,
abban az esetben ha túlzottan nagy
preparálása szükséges a fognak a
„középpontban” a konkrét equi-
libracéböl elengedjük vagy éppen
ellenkezőleg észleljük a nem várt
egyszerű megoldását esetleg szük-
séges a kemény szövet adaptációs
pótlása, vagy észleljük, hogy a kö-
zéppontban preparálással a hara-
pás egyszerű javítását egyáltalán
nem lehet elérni.
Miután, mi és a paciens az „equi-
libráció” elérhető eredményéről
meggyőződtünk, mint jól válasz-
tott megfelelő megoldás, tovább-
lépünk a magához a kezeléshez
a szájban. Legelőször a kontakt-
pontok egyenletes elosztását kell
elérnünk a középpontban azonos
intenzitással, esetleg a laterális
szakaszon alacsonyabb intenzi-
tással. Ezt célozza a fogív a tartó
csücskök és a szomszédos foss
aljának preparálása. Nagy hiba
az egyéni csücsökcsúcsok prepa-
rálása (tip of the cusp) és a saját
mélyedések fizurák fossy (pit of
the fossa). Hiszen ezzel csökkent-
jük a paciens harapását. A kezelés
alatt a paciens pontosan megha-
tározza, hol harap jobban és hol
kevésbé vagy egyáltalán nem. Így
közösen vele együtt és segítségé-
vel a harapás indikációjával újabb
és újabb artikulációs papírral úgy,
hogy lépésről lépésre elkészítik a
harmonikus harapást, amikor a
paciens nem érez semmilyen első
és utolsó érintkezést, hanem har-
monikus egyszeri harapást min-
den fogon.
Következő lépés az azonnali dis-
kluzió elérése a laterális szakaszon.
Csak azután, miután elérjük ezt a
két első követelményt, léphetünk
tovább a harmonikus szemfog és
frontvezetés kifinomítására. Paci-
enseknél, akinél szükséges a “long
centric” ezt a követelményt még
kifinomítjuk.
A kezelés befejeztével ellenőrizzük a
funkcionális okklúzió stabilitásának
a követelményét.
Ne feledkezzünk meg róla, hogy egy
látogatással az equilibráció nem fe-
jeződik be. Az esetek többségénél
szükséges a kezelést egy hét után
ellenőrizni és az apróbb változáso-
kat kifinomítani. Körülbelül egy hó-
nap múlva az ellenőrzést hasonlóan
megismételjük újból.
Az okklúzió elvei és a T-scan NOVUS
illetve az equilibráció a gyakorlatban
digitális támogatással

Kiemelkedő szerepet játszik kettő
színben az alkalmazott artikulációs
fólia. Így különböztetjük meg a dina-
mikus és statikus okklúziót. Továbbá
szerepet játszik a vastagság. Rendel-
kezésünkre áll 200, 100, 20, 12 és 8
mikronos. Természetesen a vasta-
gabbal kezdünk a vékonyabbak felé.
A munkánknak az elérhető legna-
gyobb pontosságát a legvékonyabb a
8 mikronos vastagság biztosítja.
Amennyiben szeretnénk elérni va-
lóságos kiegyenlített harapást,
előnyös a T-scan digitális készülék
használata a harapási erő százalékos
mérésére. „Különbség az elérhető
eredménynél körülbelül olyan mint
az endodoncia mikroszkóppal vagy
anélkül“.
Az equilibrációs kezelés befejez-
tével a pacienst egy eddig nem
ismert izom relaxáció követi,
meglepő az újonnan megszerzett
rágási mozdulat - az arc of closu-
re pályáján - egyszeri harapással.
A pacienst utasítjuk: “nyissa ki
és csukja be, mintha a fogakat
csattogtatná”. Az új ráharapás ha-
sonlít a “harkály kopogtatásra”,
ami általánosan nagyon pozití-
van tekintenek. A zene szavaival
ezt úgy lehetne kifejezni az egész
életnyi kakofónia (hangzavar)
harmonikus átmenete egyedi
hangzatoknak – akkordba. Minél
több staccato, annál jobb. Követ-
ve a megjelenését ennek a hall-
ható effektusnak tulajdonképpen
a kezelési munkánk menetének
az ellenőrzése és valamilyenfajta
jutalom a kezelőorvosnak a min-
denütt jelenlévő átmenete „nem
központból” a „központba” a pa-
ciensnél.
A „központ” és a fog equilibráció
problematikája hosszabb idejű téma
és nem lehet egy szakmai folyóirat
egyetlen cikkében megmagyarázni.
Maga a munka megkövetel bizo-
nyos tudást, gyakorlatot és ennek a
megszerzését követi bizonyos “ta-
nulási vonal”. Amennyiben érdek-
