AMY Environmental Services
Induction
INTRODUCTION
AMY Nominees works with Joint Venture partners, stakeholders and Indigenous business partnerships to undertake various
contracts throughout South Australia in areas of mining, pastoral and land management.
A.M.Y. Environmental Services Pty Ltd (AMYES) a second subsidiary company of AMY Nominees was created to provide a
range of services to the mining and resource sector. AMYES currently holds a contract with OZ Minerals for provision of a
full range of integrated Waste Management Services at Prominent Hill mine site. This contract has been in place since 2015.
Anangu Mining Pty Ltd began a sub-contract with Thiess Pty Ltd in late 2018 to assist in the operations of the “Run of
Mine” (ROM) contract – transporting ore from the open cut mine to feed the crusher on site at Prominent Hill.
Antakirinja Matu-Yankunytjatjara Aboriginal Corporation RNTBC (AMYAC) is
incorporated under the Corporations (Aboriginal and Torres Strait Islander) Act 2006. It
represents the Antakirinja Matu-Yankunytjatjara native title holders in relation to native
title land encompassing a significant area of the Far North of South Australia including
Tarcoola, Coober Pedy and Cadney Park.
A.M.Y Nominees Pty Ltd (AMY Nominees) is a wholly owned subsidiary of AMYAC and
has been established for the purpose of undertaking on behalf of AMYAC and its
members, sustainable business opportunities that promote indigenous employment and
training in all operations.
Overview of the AMY Environmental Services
Contract
• The scope of work covers the provision of plant, equipment and
personnel to provide Waste Management Services (Services) to OZ
Minerals Prominent Hill Operation.
• Whilst almost all departments at the OZ Minerals Prominent Hill
Operation, and other contracted service providers, will utilise the
Services, AMYES will receive direction only from nominated OZ
Minerals representatives or their nominated alternative
representatives.
• The Services will be required at various locations associated with the
mining operation, and at the accommodation village.
• To ensure continuity in delivery of the Services to all locations at OZ
Minerals Prominent Hill Operation, it is imperative that AMYES develops
and maintains a regular operational timetable of all scheduled and
additional works, and that AMYES Representative/s attends scheduled
planning meetings with the nominated OZ Minerals representatives.
Detail of waste handling and typical daily work
Other specific waste products that are deposited at the landfill
are as follows:
• Xanthate – which is collected in a dedicated skip at the
Processing Plant on site, which is adjacent to the
Reagents Shed; and
• Ore Samples –generated by the on-site laboratory. The
ore samples are contained in blue sample bags which the
on-site laboratory services contractor places adjacent to
the Caledonia Scaffolding laydown area, after their
required sample period. The on-site laboratory services
contractor will liaise with AMYES to determine an
appropriate collection regime. When collected, the waste
is placed into skip bins and deposited at the landfill.
OZ Minerals averages approximately 100 Tonnes of general waste per month. General waste is collected from various set locations
around OZ Minerals Prominent Hill Operation (including surface and underground mining locations such as the underground bench).
The General Waste is then transported to the general waste landfill on site. General waste is collected in wheelie bins, spin bins and
open top skips using front end loader, forklift, LV or Hook truck. General waste collection is undertaken for 4-6 hours each day.
Recycling
In addition to general waste, OZ Minerals Prominent Hill Operation prides itself on diverting potential waste from landfill to
recycling depots in Adelaide. Recycling stations are established in various locations around OZ Minerals Prominent Hill
Operation. Materials deposited at these areas are collected and taken to the Resource Recovery Centre (RRC). Materials at
the RRC are sorted into specific streams for recycling. Recycled products are then packaged, loaded and transported to the
RRC in readiness for offsite transport. Recycled products collection and sorting is undertaken for 6-8 hours each day.
Typical daily operations at AMYES

7. 240L and 140L wheelie bin collection 1.5 - 2 hours
8. Report generation and admin work (including log book entries, time sheets) 1.5 hour
9. Covering of landfill with cover material 30 minutes
10. General cleaning of work area 30 minutes - 1 hour
Other Clean-ups
and requests
AMYES must be able to respond promptly and
effectively to requests from other contracting parties
at OZ Minerals Prominent Hill Operation. This may
require delivering additional bins or skips for a specific
waste; such as wood, steel or cardboard. The requests
may also relate to assist neighbouring pastoral station
owners with loading of donated goods such as empty
IBC’s or unloading donated scrap cars for ERT training.
AMYES is required to manage the cover of the waste
at the landfill site including the carting of fill material
to the landfill area from allocated source areas.
AMYES must be able to handle multiple requests at
any one time.
Safety, Health and Environment, and Community (SHEC)
Safety, Health, Environment, and Community (SHEC) excellence is a priority and the responsibility of every Prominent Hill team
member. OZ Minerals and AMYES are dedicated to continuous efforts to make operations compatible with protecting the
health and safety of all employees and sub-contractor’s employees.
Personal
Protective
Equipment
AMYES will provide at its cost, and ensure its personnel wear, the
appropriate safety apparel, Personal Protective Equipment (PPE) and other
safety equipment required at Site and as per OZ Minerals standards and
operating procedures. These items shall be in good repair and working
order.
PPE shall include, but is not limited to the following items:
• Long Trousers fitted with reflective strips;
• Collared, long sleeved hi-visibility shirt complete with reflective strips;
• Steel capped, laced up safety boots or gumboots
• Hard Hat;
• Safety Glasses;
• Respiratory Protective Equipment which is suitable for the intended
task;
• Hearing Protection; and
• Gloves.
• At the request of the SHEC Department, AMYES personnel may be
required to participate in occupational hygiene monitoring
assessments as part of their normal work activities. Results of any
occupational hygiene monitoring can be provided as requested
AMYES Equipment
and Services
AMYES utilises the following plant and equipment to provide
services in the waste management contract:
• 1 x Baling Machine and straps to enable appropriate baling of
materials
• 1 x Hook Truck – DAF Prime Mover
• 1 x Excavator – 35 tonne CAT
• 1 x Front End Loader (with interchangeable bucket, forklift
tynes and spinner plate) – Hitachi
• 1 x Diesel 2.5T Forklift
• 1 x 10x5 single axle Caged Trailer (for collection of 140L/240L
wheelie bins)
• 1 x 4WD Support Vehicle (LV as per Site requirements) –
Toyota Hilux
• Skip Bins 15 Cubic metre capacity – collecting general waste,
timber etc. (quantity as directed by the OZ Minerals
Representative)
• Spin bins 1 cubic metre capacity – collecting scrap steel,
cardboard etc. (quantity as directed by the OZ Minerals
Representative)
AMYES Crew,
working
hours and
qualifications
The AMYES Team
There are generally two Operators on site at any one time, and AMYES also has an
Aboriginal Trainee position which operates across both swings.
The preferred work roster (swing) for AMYES personnel is 8 days on / 6 days off, unless
otherwise agreed with OZ Minerals.
AMYES personnel are required to work for twelve hours per day when on site.
Work hours are nominally between the hours of 0600 and 1800 hrs.
While some plant operations are required, the AMYES Team have a highly demanding
physical role. This role requires a dedication to safety, timelines and an appreciation
and respect to maintaining equipment and high levels of service to our client.
All AMYES Operators shall hold the following relevant licences:
• HR (Heavy rigid Truck License) and ability to drive a manual vehicle
• Forklift – High Risk License
• Operation of Loader (LL)
• Operation of Excavator (or working towards)
Training and Induction
AMYES must ensure that all its personnel working at site have completed
the following training before commencing any work at Prominent Hill:
• Prominent Hill Site Induction (prior to arrival on site); including the
attachment of a recent National Police check certificate
• Relevant Area Inductions;
• Pre-work training modules as instructed by the OZ Minerals
Representative, including but not limited to:
o OZ Minerals JSEA training
o OZ Minerals Personal Lock Holder
o OZ Minerals Simple Isolation Officer
o Underground Induction
OZ Minerals Travel and
Accommodation
OZ Minerals provides a charter flight service to the AMYES Personnel from
Adelaide and Port Augusta Airports to and from Prominent Hill Mine Site for the
purpose of the Waste Management Contract. A bus service is also available for
AMYES Personnel who live in and around Coober Pedy.
• ‘No shows’ for flights will be required to be paid by AMYES at a rate of $250
(including GST) per flight (one way). AMYES will require employees to cover
the cost of any replacement flights required to get to site in the case of a ‘no
show’ occurring.
• OZ Minerals will allocate to AMYES the required number of rooms in the
village, but only to the extent that they are actually occupied by AMYES
Personnel, and provide two meals and one crib lunch each calendar day while
the AMYES Personnel are at the Site.
• AMYES Personnel must comply with all accommodation and village rules and
conditions. Failure to do so may result in withdrawal of accommodation and
village access for any non-complying individuals and removal from the Site.
MOBILISATION AND
ROSTER
Your work with AMYES is a FIFO Role from either Adelaide or Port
Augusta to Prominent Hill. (A Bus service is also available from Coober
Pedy to Prominent Hill)
ALL mobilisation services cost money to run and depend on timely
bookings, checking in and use.
Rosters may vary between OZ Minerals and other contractors;
• An 8/6 is : 8 Days on / 6 days off (OZ Minerals / AMYES)
• 4/3: is 4 days on/3 days off (OZ Minerals)
• 8/6/7/7: 8 days on / 6 Days off / 7 Nights on / 7 Days Off (Thiess)
• 2 on /1 off: 2 weeks on/1 week off (Byrnecut)
DOCUMENTATION
Prior to commencing ALL employees will complete the
following:
• Police Clearance
• Pre-Employment Medical
• Prominent Hill – OZ Minerals Online Induction
• Personal Details Form (Contact and Bank Details)
• Tax Declaration
• Superannuation Choice
• Receive and confirm receipt of the Fair Work Information
Statement
• Employment Contract
GETTING PAID
• Our team will complete individual
timesheets that will be approved by
Supervisors/Management.
• It will be your responsibility to ensure these
are completed correctly and
signed/approved by the supervisor.
• Your pay will be deposited in your
nominated bank account fortnightly in
arrears. (Possibly weekly if on 4/3 roster)
• We will confirm a designated day as we
progress.
Leave
• Most permanent employees are entitled to 4 weeks Annual
Leave. In order to schedule cover for your leave, as much
advanced notice as possible is required in order to cover your
absence in the team. A request for Annual Leave Form MUST
be completed, submitted to and authorized by the HR
Manager 4 WEEKS prior to taking any leave.
• If you need to call in sick prior to mobilization, you will need
to call the HR Manager PRIOR to expected mobilization to site
AND your Supervisor preferably.
• If you are sick and unable to attend your shift once on site,
you will need to contact BOTH your HR Manager and your Site
Supervisor PRIOR to the commencement of your shift.
• A leave request form will need to be submitted to the HR
Manager/Payroll with your timesheet.
PPE and
Clothing
• PPE and Clothing will be provided. It is your
responsibility to care for this clothing and
PPE and keep it in a serviceable and
presentable manner. Please do not add
stickers or write on hard-hats as this
destroys the life-span and the warranty of
the product. If PPE or clothing is lost or
damaged, it will be replaced at full cost to
the employee
• You will require out of hours clothing for
your time on site. Please be aware you still
represent AMYES, your team and
management at all times, so please refrain
from offensive or inappropriate clothing.

Customer Service
• All employees of AMYES are expected to apply the highest
possible levels of customer service at all times both internally
and externally and aim to not only meet the customers
expectations, but to exceed them. The basis for excellent
customer service is the development of long-lasting
relationships, individual success, team and company
achievement.
• How you treat others will impact directly on how you are
treated.
• How you are perceived by your customers, managers and most
importantly your workmates, will assist you and anyone
associated with you get future work.
• Standards of customer service expected of employees include
but are not limited to:
• Completing all work safely, to a high standard and efficiently;
• Honesty, respect, fairness and a courteous manner in dealings with
customers, clients, co-workers, suppliers, management and the
general public;
• Ensuring the privacy and protection of a customer’s personal
information;
• A commitment to the process of continuous improvement.
FEEDBACK AND CONTINUAL IMPROVEMENT
• As a part of our team and as a part of our
commitment to never ending improvement, we
encourage all employees to come up with ideas
for improving the way we do things. Simply
document your ideas and/or table them at team
meetings and we would love to hear them.

with how we are doing anything, have
suggestions or improvements, new ideas or
initiatives or would like to provide any positive
feedback you may do that at any time via;

PUNCTUALITY
• As we all work as part of a team, you are expected to
be punctual in arriving to work in order to ensure
plenty of time to be ready to start prior to starting
time. Allow yourself at least 20-30 minutes to be
ready to start work on time.
• REMEMBER – You will need to allow time to have
breakfast AND make your lunch, prior to getting a
scheduled bus/transport to site. ONCE on site you
will be expected at a PRE-START meeting prior to
commencing work.
• We believe that attendance and punctuality are
paramount to the fundamental principals of
professionalism and courtesy to others. Punctuality is
an important part of employment. As our company
and the larger site, works as one large team, it is
important to be at work on time. If you are running
late for work or for meetings for ANY reason, please
inform you site supervisor/manager.
Smoking, Drugs and Alcohol
SMOKING
• We encourage a smoke-free environment. Smoking is prohibited in all buildings,
camp rooms, vehicles etc and only permitted in designated smoking areas and
generally only in designated breaks.
DRUGS AND ALCOHOL
• The use of drugs and alcohol is not permitted in the workplace. If you have a
genuine reason for a particular prescription drug, prescribed by a doctor for
health reasons, you will need to ensure the HR Manager is aware of this fact. The
use of any other drugs in the workplace (or being under the influence of any
drug) will not be tolerated.
• Alcohol is not allowed to be consumed during working hours. While there is a
“wet mess” situated in camp, you will be expected to report to work punctually
prior to your shift with a 0.00% Blood alcohol level. Daily and possibly random
Drug and alcohol testing WILL occur on site and we will adopt the same zero
tolerance policy as our client and OZ Minerals. Only responsible use of alcohol
will be tolerated.
PERSONAL USE OF MOBILE
PHONES
• Personal use of mobile phones during your shift is
discouraged. Please contact relatives and loved ones
after hours or during break times. Please ensure you
communicate this to your family so the urge to reply
to calls and text messages is kept to a minimum.
• It is however, necessary at times to communicate
with supervisors and management in a timely fashion.
Please save your direct Supervisor/Manager’s contact
number in your phone in addition to emergency and
other relevant contacts contained in this information.
DISCRIMINATION, SEXUAL
HARASSMENT AND BULLYING
AMYES is committed to providing a workplace free from

constitutes discrimination, sexual harassment or bullying will not
be tolerated and will lead to action being taken, which may
include dismissal.
Employees must report any behavior that constitutes sexual

INCIDENTS, NEAR MISSES AND
WORKERS COMPENSATION
• With numerous inductions for site and our customer you will
be encouraged to report ALL incidents and near misses. This
process is designed to protect you, your workmates and
others on site from similar occurrences. Be respectful to the
safety processes, complete all necessary forms, hazard
identification exercises and participate in any investigations.
• If you suffer an accident or injury at work you MUST report it
to your supervisor AND Anangu Mining / AMYES immediately
(within 24 hours) Whether you simply require some minor
first aid or more serious medical treatment by a doctor,
reporting the incident will be more welcomed than allowing
the injury to get worse.
• In the event of any injury to Anangu Mining employees, the
company is covered by the Return to Work scheme. This
insurance is designed to protect employees and employers
from the expense of injuries and implements a stringent
process in order to do so.
• While your injuries or illnesses will be covered you will be
expected to participate in ALL necessary appointments and
rehabilitation procedures that are required to return to work
safely and a quickly as possible.
YOUR RESPONSIBILITIES
• It is your responsibility to make sure that you adhere to ALL safety rules from
Anangu Mining, our client and the Prominent Hill site owner, OZ Minerals. You
are required to carry out your duties in a safe and responsible manner, taking
into account the health and safety of yourself, your work mates, clients and
visitors. If you see any unsafe conditions or any breaches of safety, you are
required to bring these to the attention of those involved and your
supervisor/manager.
• ANY act you walk past, is an act you accept and condone.
• Failure to comply with any of the safety rules and regulations is taken very
seriously as a breach of workplace health and safety practices.
• If you are injured away from the workplace, after hours or during your breaks,
you MUST provide a medical clearance certifying you are fully fit to perform
your duties before you return to work.
SAFE USE OF PLANT AND EQUIPMENT
• As operators and training operators it is important that you ONLY operate
equipment that you are trained and authorised to operate. All equipment you
operate or utilize is your responsibility to ensure it is working in a safe and
reliable manner. If you notice something may be dangerous or does not look
right, report it to your supervisor and tag the equipment appropriately so it
does not present a danger to anybody else. If you are asked to operate
equipment you are unfamiliar with or untrained in the use of, please notify
Anangu Mining at your first opportunity.

AMYES Induction August 2020

  • 2. INTRODUCTION AMY Nominees works with Joint Venture partners, stakeholders and Indigenous business partnerships to undertake various contracts throughout South Australia in areas of mining, pastoral and land management. A.M.Y. Environmental Services Pty Ltd (AMYES) a second subsidiary company of AMY Nominees was created to provide a range of services to the mining and resource sector. AMYES currently holds a contract with OZ Minerals for provision of a full range of integrated Waste Management Services at Prominent Hill mine site. This contract has been in place since 2015. Anangu Mining Pty Ltd began a sub-contract with Thiess Pty Ltd in late 2018 to assist in the operations of the “Run of Mine” (ROM) contract – transporting ore from the open cut mine to feed the crusher on site at Prominent Hill. Antakirinja Matu-Yankunytjatjara Aboriginal Corporation RNTBC (AMYAC) is incorporated under the Corporations (Aboriginal and Torres Strait Islander) Act 2006. It represents the Antakirinja Matu-Yankunytjatjara native title holders in relation to native title land encompassing a significant area of the Far North of South Australia including Tarcoola, Coober Pedy and Cadney Park. A.M.Y Nominees Pty Ltd (AMY Nominees) is a wholly owned subsidiary of AMYAC and has been established for the purpose of undertaking on behalf of AMYAC and its members, sustainable business opportunities that promote indigenous employment and training in all operations.
  • 3. Overview of the AMY Environmental Services Contract • The scope of work covers the provision of plant, equipment and personnel to provide Waste Management Services (Services) to OZ Minerals Prominent Hill Operation. • Whilst almost all departments at the OZ Minerals Prominent Hill Operation, and other contracted service providers, will utilise the Services, AMYES will receive direction only from nominated OZ Minerals representatives or their nominated alternative representatives. • The Services will be required at various locations associated with the mining operation, and at the accommodation village. • To ensure continuity in delivery of the Services to all locations at OZ Minerals Prominent Hill Operation, it is imperative that AMYES develops and maintains a regular operational timetable of all scheduled and additional works, and that AMYES Representative/s attends scheduled planning meetings with the nominated OZ Minerals representatives.
  • 4. Detail of waste handling and typical daily work Other specific waste products that are deposited at the landfill are as follows: • Xanthate – which is collected in a dedicated skip at the Processing Plant on site, which is adjacent to the Reagents Shed; and • Ore Samples –generated by the on-site laboratory. The ore samples are contained in blue sample bags which the on-site laboratory services contractor places adjacent to the Caledonia Scaffolding laydown area, after their required sample period. The on-site laboratory services contractor will liaise with AMYES to determine an appropriate collection regime. When collected, the waste is placed into skip bins and deposited at the landfill. OZ Minerals averages approximately 100 Tonnes of general waste per month. General waste is collected from various set locations around OZ Minerals Prominent Hill Operation (including surface and underground mining locations such as the underground bench). The General Waste is then transported to the general waste landfill on site. General waste is collected in wheelie bins, spin bins and open top skips using front end loader, forklift, LV or Hook truck. General waste collection is undertaken for 4-6 hours each day.
  • 5. Recycling In addition to general waste, OZ Minerals Prominent Hill Operation prides itself on diverting potential waste from landfill to recycling depots in Adelaide. Recycling stations are established in various locations around OZ Minerals Prominent Hill Operation. Materials deposited at these areas are collected and taken to the Resource Recovery Centre (RRC). Materials at the RRC are sorted into specific streams for recycling. Recycled products are then packaged, loaded and transported to the RRC in readiness for offsite transport. Recycled products collection and sorting is undertaken for 6-8 hours each day. Typical daily operations at AMYES Activity Expected Duration 1. Attendance at morning Pre Start Meeting 30 minutes 2. Collection of spin bins 5 hours 3. Bailing cardboard 1- 2 hours 4. Recycling cans and bottles 1.5 - 2 hours 5. Skip collection 3 hours 6. Hauling of cover material for active landfill cell from stockpiles Estimated at approx. 1 day per month. 7. 240L and 140L wheelie bin collection 1.5 - 2 hours 8. Report generation and admin work (including log book entries, time sheets) 1.5 hour 9. Covering of landfill with cover material 30 minutes 10. General cleaning of work area 30 minutes - 1 hour
  • 6. Other Clean-ups and requests AMYES must be able to respond promptly and effectively to requests from other contracting parties at OZ Minerals Prominent Hill Operation. This may require delivering additional bins or skips for a specific waste; such as wood, steel or cardboard. The requests may also relate to assist neighbouring pastoral station owners with loading of donated goods such as empty IBC’s or unloading donated scrap cars for ERT training. AMYES is required to manage the cover of the waste at the landfill site including the carting of fill material to the landfill area from allocated source areas. AMYES must be able to handle multiple requests at any one time.
  • 7. Safety, Health and Environment, and Community (SHEC) Safety, Health, Environment, and Community (SHEC) excellence is a priority and the responsibility of every Prominent Hill team member. OZ Minerals and AMYES are dedicated to continuous efforts to make operations compatible with protecting the health and safety of all employees and sub-contractor’s employees.
  • 8. Personal Protective Equipment AMYES will provide at its cost, and ensure its personnel wear, the appropriate safety apparel, Personal Protective Equipment (PPE) and other safety equipment required at Site and as per OZ Minerals standards and operating procedures. These items shall be in good repair and working order. PPE shall include, but is not limited to the following items: • Long Trousers fitted with reflective strips; • Collared, long sleeved hi-visibility shirt complete with reflective strips; • Steel capped, laced up safety boots or gumboots • Hard Hat; • Safety Glasses; • Respiratory Protective Equipment which is suitable for the intended task; • Hearing Protection; and • Gloves. • At the request of the SHEC Department, AMYES personnel may be required to participate in occupational hygiene monitoring assessments as part of their normal work activities. Results of any occupational hygiene monitoring can be provided as requested
  • 9. AMYES Equipment and Services AMYES utilises the following plant and equipment to provide services in the waste management contract: • 1 x Baling Machine and straps to enable appropriate baling of materials • 1 x Hook Truck – DAF Prime Mover • 1 x Excavator – 35 tonne CAT • 1 x Front End Loader (with interchangeable bucket, forklift tynes and spinner plate) – Hitachi • 1 x Diesel 2.5T Forklift • 1 x 10x5 single axle Caged Trailer (for collection of 140L/240L wheelie bins) • 1 x 4WD Support Vehicle (LV as per Site requirements) – Toyota Hilux • Skip Bins 15 Cubic metre capacity – collecting general waste, timber etc. (quantity as directed by the OZ Minerals Representative) • Spin bins 1 cubic metre capacity – collecting scrap steel, cardboard etc. (quantity as directed by the OZ Minerals Representative)
  • 10. AMYES Crew, working hours and qualifications The AMYES Team There are generally two Operators on site at any one time, and AMYES also has an Aboriginal Trainee position which operates across both swings. The preferred work roster (swing) for AMYES personnel is 8 days on / 6 days off, unless otherwise agreed with OZ Minerals. AMYES personnel are required to work for twelve hours per day when on site. Work hours are nominally between the hours of 0600 and 1800 hrs. While some plant operations are required, the AMYES Team have a highly demanding physical role. This role requires a dedication to safety, timelines and an appreciation and respect to maintaining equipment and high levels of service to our client. All AMYES Operators shall hold the following relevant licences: • HR (Heavy rigid Truck License) and ability to drive a manual vehicle • Forklift – High Risk License • Operation of Loader (LL) • Operation of Excavator (or working towards)
  • 11. Training and Induction AMYES must ensure that all its personnel working at site have completed the following training before commencing any work at Prominent Hill: • Prominent Hill Site Induction (prior to arrival on site); including the attachment of a recent National Police check certificate • Relevant Area Inductions; • Pre-work training modules as instructed by the OZ Minerals Representative, including but not limited to: o OZ Minerals JSEA training o OZ Minerals Personal Lock Holder o OZ Minerals Simple Isolation Officer o Underground Induction
  • 12. OZ Minerals Travel and Accommodation OZ Minerals provides a charter flight service to the AMYES Personnel from Adelaide and Port Augusta Airports to and from Prominent Hill Mine Site for the purpose of the Waste Management Contract. A bus service is also available for AMYES Personnel who live in and around Coober Pedy. • ‘No shows’ for flights will be required to be paid by AMYES at a rate of $250 (including GST) per flight (one way). AMYES will require employees to cover the cost of any replacement flights required to get to site in the case of a ‘no show’ occurring. • OZ Minerals will allocate to AMYES the required number of rooms in the village, but only to the extent that they are actually occupied by AMYES Personnel, and provide two meals and one crib lunch each calendar day while the AMYES Personnel are at the Site. • AMYES Personnel must comply with all accommodation and village rules and conditions. Failure to do so may result in withdrawal of accommodation and village access for any non-complying individuals and removal from the Site.
  • 13. MOBILISATION AND ROSTER Your work with AMYES is a FIFO Role from either Adelaide or Port Augusta to Prominent Hill. (A Bus service is also available from Coober Pedy to Prominent Hill) ALL mobilisation services cost money to run and depend on timely bookings, checking in and use. Rosters may vary between OZ Minerals and other contractors; • An 8/6 is : 8 Days on / 6 days off (OZ Minerals / AMYES) • 4/3: is 4 days on/3 days off (OZ Minerals) • 8/6/7/7: 8 days on / 6 Days off / 7 Nights on / 7 Days Off (Thiess) • 2 on /1 off: 2 weeks on/1 week off (Byrnecut)
  • 14. DOCUMENTATION Prior to commencing ALL employees will complete the following: • Police Clearance • Pre-Employment Medical • Prominent Hill – OZ Minerals Online Induction • Personal Details Form (Contact and Bank Details) • Tax Declaration • Superannuation Choice • Receive and confirm receipt of the Fair Work Information Statement • Employment Contract
  • 15. GETTING PAID • Our team will complete individual timesheets that will be approved by Supervisors/Management. • It will be your responsibility to ensure these are completed correctly and signed/approved by the supervisor. • Your pay will be deposited in your nominated bank account fortnightly in arrears. (Possibly weekly if on 4/3 roster) • We will confirm a designated day as we progress.
  • 16. Leave • Most permanent employees are entitled to 4 weeks Annual Leave. In order to schedule cover for your leave, as much advanced notice as possible is required in order to cover your absence in the team. A request for Annual Leave Form MUST be completed, submitted to and authorized by the HR Manager 4 WEEKS prior to taking any leave. • If you need to call in sick prior to mobilization, you will need to call the HR Manager PRIOR to expected mobilization to site AND your Supervisor preferably. • If you are sick and unable to attend your shift once on site, you will need to contact BOTH your HR Manager and your Site Supervisor PRIOR to the commencement of your shift. • A leave request form will need to be submitted to the HR Manager/Payroll with your timesheet.
  • 17. PPE and Clothing • PPE and Clothing will be provided. It is your responsibility to care for this clothing and PPE and keep it in a serviceable and presentable manner. Please do not add stickers or write on hard-hats as this destroys the life-span and the warranty of the product. If PPE or clothing is lost or damaged, it will be replaced at full cost to the employee • You will require out of hours clothing for your time on site. Please be aware you still represent AMYES, your team and management at all times, so please refrain from offensive or inappropriate clothing. Inappropriate! Customer Service • All employees of AMYES are expected to apply the highest possible levels of customer service at all times both internally and externally and aim to not only meet the customers expectations, but to exceed them. The basis for excellent customer service is the development of long-lasting relationships, individual success, team and company achievement. • How you treat others will impact directly on how you are treated. • How you are perceived by your customers, managers and most importantly your workmates, will assist you and anyone associated with you get future work. • Standards of customer service expected of employees include but are not limited to: • Completing all work safely, to a high standard and efficiently; • Honesty, respect, fairness and a courteous manner in dealings with customers, clients, co-workers, suppliers, management and the general public; • Ensuring the privacy and protection of a customer’s personal information; • A commitment to the process of continuous improvement.
  • 19. FEEDBACK AND CONTINUAL IMPROVEMENT • As a part of our team and as a part of our commitment to never ending improvement, we encourage all employees to come up with ideas for improving the way we do things. Simply document your ideas and/or table them at team meetings and we would love to hear them. • Your feedback is always welcomed whether it is positive or constructive.   • 20. PUNCTUALITY • As we all work as part of a team, you are expected to be punctual in arriving to work in order to ensure plenty of time to be ready to start prior to starting time. Allow yourself at least 20-30 minutes to be ready to start work on time. • REMEMBER – You will need to allow time to have breakfast AND make your lunch, prior to getting a scheduled bus/transport to site. ONCE on site you will be expected at a PRE-START meeting prior to commencing work. • We believe that attendance and punctuality are paramount to the fundamental principals of professionalism and courtesy to others. Punctuality is an important part of employment. As our company and the larger site, works as one large team, it is important to be at work on time. If you are running late for work or for meetings for ANY reason, please inform you site supervisor/manager.
  • 21. Smoking, Drugs and Alcohol SMOKING • We encourage a smoke-free environment. Smoking is prohibited in all buildings, camp rooms, vehicles etc and only permitted in designated smoking areas and generally only in designated breaks. DRUGS AND ALCOHOL • The use of drugs and alcohol is not permitted in the workplace. If you have a genuine reason for a particular prescription drug, prescribed by a doctor for health reasons, you will need to ensure the HR Manager is aware of this fact. The use of any other drugs in the workplace (or being under the influence of any drug) will not be tolerated. • Alcohol is not allowed to be consumed during working hours. While there is a “wet mess” situated in camp, you will be expected to report to work punctually prior to your shift with a 0.00% Blood alcohol level. Daily and possibly random Drug and alcohol testing WILL occur on site and we will adopt the same zero tolerance policy as our client and OZ Minerals. Only responsible use of alcohol will be tolerated.
  • 22. PERSONAL USE OF MOBILE PHONES • Personal use of mobile phones during your shift is discouraged. Please contact relatives and loved ones after hours or during break times. Please ensure you communicate this to your family so the urge to reply to calls and text messages is kept to a minimum. • It is however, necessary at times to communicate with supervisors and management in a timely fashion. Please save your direct Supervisor/Manager’s contact number in your phone in addition to emergency and other relevant contacts contained in this information.

  • 24. INCIDENTS, NEAR MISSES AND WORKERS COMPENSATION • With numerous inductions for site and our customer you will be encouraged to report ALL incidents and near misses. This process is designed to protect you, your workmates and others on site from similar occurrences. Be respectful to the safety processes, complete all necessary forms, hazard identification exercises and participate in any investigations. • If you suffer an accident or injury at work you MUST report it to your supervisor AND Anangu Mining / AMYES immediately (within 24 hours) Whether you simply require some minor first aid or more serious medical treatment by a doctor, reporting the incident will be more welcomed than allowing the injury to get worse. • In the event of any injury to Anangu Mining employees, the company is covered by the Return to Work scheme. This insurance is designed to protect employees and employers from the expense of injuries and implements a stringent process in order to do so. • While your injuries or illnesses will be covered you will be expected to participate in ALL necessary appointments and rehabilitation procedures that are required to return to work safely and a quickly as possible.
  • 25. YOUR RESPONSIBILITIES • It is your responsibility to make sure that you adhere to ALL safety rules from Anangu Mining, our client and the Prominent Hill site owner, OZ Minerals. You are required to carry out your duties in a safe and responsible manner, taking into account the health and safety of yourself, your work mates, clients and visitors. If you see any unsafe conditions or any breaches of safety, you are required to bring these to the attention of those involved and your supervisor/manager. • ANY act you walk past, is an act you accept and condone. • Failure to comply with any of the safety rules and regulations is taken very seriously as a breach of workplace health and safety practices. • If you are injured away from the workplace, after hours or during your breaks, you MUST provide a medical clearance certifying you are fully fit to perform your duties before you return to work. SAFE USE OF PLANT AND EQUIPMENT • As operators and training operators it is important that you ONLY operate equipment that you are trained and authorised to operate. All equipment you operate or utilize is your responsibility to ensure it is working in a safe and reliable manner. If you notice something may be dangerous or does not look right, report it to your supervisor and tag the equipment appropriately so it does not present a danger to anybody else. If you are asked to operate equipment you are unfamiliar with or untrained in the use of, please notify Anangu Mining at your first opportunity.
