Deloitte’s 2016 Survey of US Physicians sought to shed light on physicians’ awareness of MACRA, their perspectives on its implications, and their attitudes and readiness for change. The survey found that many physicians are unaware of MACRA. Regardless of their awareness level, most physicians surveyed would have to change aspects of their practice to meet the law’s requirements and to do well under its incentives. Increasing administrative responsibilities—due to regulatory pressures
and evolving payment and care delivery models—reduce the amount of
time physicians spend delivering direct patient care. When empathetic
listening is used in health care, patients and families are more satisfied
and more open to hearing their physician’s advice. Practicing empathy
can save time and effectively defuse difficult situations. By forging deeper
connections with patients, physicians can experience greater professional
satisfaction and joy in work.

Studies have shown that physician empathy is an essential attribute of
the patient-physician relationship and is associated with better outcomes,
greater patient safety and fewer malpractice claims. However, due to the
rigorous amount of education physicians already need to go through,
communication skills training has traditionally received less attention. This
module provides physicians the training on how to demonstrate empathy
to patients in their practice.

American Journal of Emergency & Critical Care Medicine
Background: The Food and Drug Administration relies on adverse event reports linked with health risks to remove potentially harmful dietary supplements from the market. Many emergency medicine physicians encounter suspected adverse events related to
dietary supplement use but we do not know what proportion of those adverse events are reported to the Food and Drug Administration. The objective of the study was to determine emergency medicine physicians’ practices regarding adverse event reporting and knowledge of dietary supplements.
Methods: A prospective, cross-sectional study was conducted across five medical centers around the U.S: three military and two civilian. A web-based survey was distributed to emergency medicine attending physicians and emergency medicine residents. The questionnaire was created and administered using Lime Survey software. An administrator at each site communicated study details to emergency medicine physicians and residents via email. The survey was kept open for fi ve months. To preserve participant anonymity,
neither email domains, email addresses, Internet Protocol addresses, nor any other personally identifi able or demographic information were collected.

For many years, health care organizations, as well as traditional businesses, have been frustrated that high customer satisfaction scores do not necessarily lead to higher levels of profitability or sales.

Article 1 The increased use of meta-analysis in systematic reviews of health care interventions has highlighted several types of bias that can arise during the completion of a randomized controlled trial. Study publication bias and outcome reporting bias have been recognized as potential threats to the validity of meta-analysis and can make the readily available evidence unreliable for decision making. This update reviews and summarizes the evidence from cohort studies that have assessed study publication bias or outcome reporting bias in randomized controlled trials. Twenty studies were eligible, of which four were newly identified in this update. Only two followed the cohort all the way through from protocol approval to information regarding the publication of outcomes. Fifteen of the studies investigated study publication bias and five investigated outcome reporting bias. Three studies have found that statistically significant outcomes had higher odds of being fully reported as compared to nonsignificant outcomes (range of odds ratios: 2.2–4.7). In comparing trial publications to protocols, it was found that 40–62% of studies had at least one primary outcome that was changed, introduced, or omitted. While physicians remain an important source, digital channels provide a way for patients to educate themselves and get support due to time constraints with doctors. The survey showed consumers are open to various digital communication methods from healthcare providers going forward.
Patient satisfaction white paper
Emergency Medicine Associates implemented an innovative coaching program to improve patient satisfaction scores. The program involved one-on-one coaching sessions for providers struggling with lower satisfaction scores. Coaches used role-playing and feedback to help providers strengthen communication skills. The program contributed to an 11% overall improvement in EMA's patient satisfaction scores from 2008 to 2015.

Purpose. The published literature on medication reconciliation (MR)
interventions, outcomes, and facilitators in ambulatory care settings is
reviewed.
Methods. A scoping review was conducted to characterize ambulatory
care–based MR research in terms of study design, elements of interven-
tions, and outcomes examined. English-language articles on comparative
studies of MR programs targeting adults in ambulatory care settings were
identified using data sources including MEDLINE, PreMEDLINE, EMBASE,
and International Pharmaceutical Abstracts. For each study, steps under-
taken in the MR process were extracted. The Cochrane Effective Practice
and Organisation of Care (EPOC) taxonomy was used to classify types of
interventions; taxonomies for reported outcomes and factors facilitating
implementation of MR initiatives were developed by the authors.
Results. From among 2062 publications screened, 15 were included in the
review. In 13 studies, multiple data sources were used to compile a “best
possible medication history” (BPMH); however, the BPMH was shared with
external healthcare providers in only 4 studies and with patients in only 5
studies. Most reported MR interventions were classified into two EPOC
domains: professional (predominantly educational outreach visits and pa-
tient reminders) and organizational (predominantly provider-oriented inter-
ventions). Process outcomes were reported in 12 studies, with correct per-
formance of MR being the most commonly evaluated process outcome,
and 9 studies identified factors that facilitated MR implementation.
Conclusion. Few studies have examined clinical outcomes of MR in am-
bulatory care settings, with the majority of pertinent reports focusing in-

The Partners for Change Outcome Management System: Duncan & Reese, 2015
Despite overall psychotherapy efficacy (Lambert, 2013), many clients do not benefit (Reese, Duncan, Bohanske, Owen, & Minami, 2014), dropouts are a problem (Swift & Greenberg, 2012), and therapists vary significantly in success rates (Baldwin & Imel, 2013), are poor judges of negative outcomes (Chapman et al., 2012), and grossly overestimate their effectiveness (Walfish, McAlister, O'Donnell, & Lambert, 2012). Systematic client feedback offers one solution (Duncan, 2014). Several feedback systems have emerged (Castonguay, Barkham, Lutz, & McAleavey, 2013), but only two have randomized clinical trial support and are included in the Substance Abuse and Mental Health Administration’s National Registry of Evidence based Programs and Practices: The Outcome Questionnaire-45.2 System (Lambert, 2010) and the Partners for Change Outcome Management System (PCOMS; Duncan, 2012). This article presents the current status of the Partners for Change Outcome Management System, the psychometrics of the PCOMS measures, its empirical support, and its clinical and training applications. Future directions and implications of PCOMS research, training, and practice are detailed. Finally, we propose that systematic feedback offers a way, via large scale data collection, to re-prioritize what matters to psychotherapy outcome, reclaim our empirically validated core values and identity, and change the conversation from a medical model dominated discourse to a more scientific, relational perspective.

Attitude and Career Preferences of Pharmacists
This document summarizes a study that examined the attitudes and career preferences of fourth year Doctor of Pharmacy students in Pakistan. The study found that most students were unaware of the scope of pharmacy before their program. Most believed pharmacy impacts healthcare. Their preferred career areas were clinical, industrial, and hospital pharmacy. Personal interest was the main factor in their career selection. Many were interested in research but unaware of postgraduate options.

Increasing administrative responsibilities—due to regulatory pressures
and evolving payment and care delivery models—reduce the amount of
time physicians spend delivering direct patient care. When empathetic
listening is used in health care, patients and families are more satisfied
and more open to hearing their physician’s advice. Practicing empathy
can save time and effectively defuse difficult situations. By forging deeper
connections with patients, physicians can experience greater professional
satisfaction and joy in work.

Statement of Need
Studies have shown that physician empathy is an essential attribute of
the patient-physician relationship and is associated with better outcomes,
greater patient safety and fewer malpractice claims. However, due to the
rigorous amount of education physicians already need to go through,
communication skills training has traditionally received less attention. This
module provides physicians the training on how to demonstrate empathy
to patients in their practice.

and communications skills, professionalism, systems-based practice and

American Journal of Emergency & Critical Care Medicine
Background: The Food and Drug Administration relies on adverse event reports linked with health risks to remove potentially harmful dietary supplements from the market. Many emergency medicine physicians encounter suspected adverse events related to
dietary supplement use but we do not know what proportion of those adverse events are reported to the Food and Drug Administration. The objective of the study was to determine emergency medicine physicians’ practices regarding adverse event reporting and knowledge of dietary supplements.
Methods: A prospective, cross-sectional study was conducted across five medical centers around the U.S: three military and two civilian. A web-based survey was distributed to emergency medicine attending physicians and emergency medicine residents. The questionnaire was created and administered using Lime Survey software. An administrator at each site communicated study details to emergency medicine physicians and residents via email. The survey was kept open for fi ve months. To preserve participant anonymity,
neither email domains, email addresses, Internet Protocol addresses, nor any other personally identifi able or demographic information were collected.

Article 1 The increased use of meta-analysis in systematic reviews of health care interventions has highlighted several types of bias that can arise during the completion of a randomized controlled trial. Study publication bias and outcome reporting bias have been recognized as potential threats to the validity of meta-analysis and can make the readily available evidence unreliable for decision making. This update reviews and summarizes the evidence from cohort studies that have assessed study publication bias or outcome reporting bias in randomized controlled trials. Twenty studies were eligible, of which four were newly identified in this update. Only two followed the cohort all the way through from protocol approval to information regarding the publication of outcomes. Fifteen of the studies investigated study publication bias and five investigated outcome reporting bias. Three studies have found that statistically significant outcomes had higher odds of being fully reported as compared to nonsignificant outcomes (range of odds ratios: 2.2–4.7). In comparing trial publications to protocols, it was found that 40–62% of studies had at least one primary outcome that was changed, introduced, or omitted. It was decided not to undertake meta-analysis because of the differences between studies. This update does not change the conclusions of the review in which 16 studies were included. A survey of 1,238 consumers across 6 Asian markets found that 89% had accessed the internet for healthcare information, primarily about conditions and available treatments. While physicians remain an important source, digital channels provide a way for patients to educate themselves and get support due to time constraints with doctors. The survey showed consumers are open to various digital communication methods from healthcare providers going forward.
Patient satisfaction white paper
Emergency Medicine Associates implemented an innovative coaching program to improve patient satisfaction scores. The program involved one-on-one coaching sessions for providers struggling with lower satisfaction scores. Coaches used role-playing and feedback to help providers strengthen communication skills. The program contributed to an 11% overall improvement in EMA's patient satisfaction scores from 2008 to 2015.

Purpose. The published literature on medication reconciliation (MR)
interventions, outcomes, and facilitators in ambulatory care settings is
reviewed.
Methods. A scoping review was conducted to characterize ambulatory
care–based MR research in terms of study design, elements of interven-
tions, and outcomes examined. English-language articles on comparative
studies of MR programs targeting adults in ambulatory care settings were
identified using data sources including MEDLINE, PreMEDLINE, EMBASE,
and International Pharmaceutical Abstracts. For each study, steps under-
taken in the MR process were extracted. The Cochrane Effective Practice
and Organisation of Care (EPOC) taxonomy was used to classify types of
interventions; taxonomies for reported outcomes and factors facilitating
implementation of MR initiatives were developed by the authors.
Results. From among 2062 publications screened, 15 were included in the
review. In 13 studies, multiple data sources were used to compile a “best
possible medication history” (BPMH); however, the BPMH was shared with
external healthcare providers in only 4 studies and with patients in only 5
studies. Most reported MR interventions were classified into two EPOC
domains: professional (predominantly educational outreach visits and pa-
tient reminders) and organizational (predominantly provider-oriented inter-
ventions). Process outcomes were reported in 12 studies, with correct per-
formance of MR being the most commonly evaluated process outcome,
and 9 studies identified factors that facilitated MR implementation.
Conclusion. Few studies have examined clinical outcomes of MR in am-
bulatory care settings, with the majority of pertinent reports focusing in-
stead on process outcomes. Facilitators of successful MR interventions

The Partners for Change Outcome Management System: Duncan & Reese, 2015
Despite overall psychotherapy efficacy (Lambert, 2013), many clients do not benefit (Reese, Duncan, Bohanske, Owen, & Minami, 2014), dropouts are a problem (Swift & Greenberg, 2012), and therapists vary significantly in success rates (Baldwin & Imel, 2013), are poor judges of negative outcomes (Chapman et al., 2012), and grossly overestimate their effectiveness (Walfish, McAlister, O'Donnell, & Lambert, 2012). Systematic client feedback offers one solution (Duncan, 2014). Several feedback systems have emerged (Castonguay, Barkham, Lutz, & McAleavey, 2013), but only two have randomized clinical trial support and are included in the Substance Abuse and Mental Health Administration’s National Registry of Evidence based Programs and Practices: The Outcome Questionnaire-45.2 System (Lambert, 2010) and the Partners for Change Outcome Management System (PCOMS; Duncan, 2012). This article presents the current status of the Partners for Change Outcome Management System, the psychometrics of the PCOMS measures, its empirical support, and its clinical and training applications. Future directions and implications of PCOMS research, training, and practice are detailed. Finally, we propose that systematic feedback offers a way, via large scale data collection, to re-prioritize what matters to psychotherapy outcome, reclaim our empirically validated core values and identity, and change the conversation from a medical model dominated discourse to a more scientific, relational perspective.

- 2. Introduction A pharmaceutical sciences graduate of St. John’s University, Ellen Scharaga is an operational consultant with Alegria Specialty Infusion and Pain Pharmacy. Engaged in her field, Ellen Scharaga maintains membership in professional organizations such as the Academy of Managed Care Pharmacy (AMCP). AMCP has announced the results of a survey it conducted earlier this year in which some 63 percent of people receiving Medicare say they would encourage friends and family to use Part D medication therapy management (MTM) services. Fewer than half of the participants, however, report satisfaction with the Standardized Format (SF) as useful summaries of their MTM services.
- 3. AMCP According to AMCP, SF has been a point of contention by Medicare providers because it does not mesh well with electronic medical record systems and is not very portable. This survey is the first to gauge feedback from Part D MTM participants on their perceptions of the format. Even though SF has been widely criticized, there is still no universally accepted model among providers as to how it should be laid out. AMCP provided these insights to the Centers for Medicaid and Medicare Services (CMS) in late 2018. AMCP believes these findings will help CMS improve the system and streamline the standardized format.
