Richard is a 50-year-old male presenting with nasal congestion, sneezing, rhinorrhea, and postnasal drainage for 5 days. On examination, he has pale, boggy nasal mucosa with clear thin secretions and enlarged nasal turbinates obstructing airflow, but clear lungs and mildly erythematous throat. A focused history and physical exam is needed to determine if this is an upper respiratory infection or something more serious like sinusitis. Differential diagnoses could include the common cold, allergic rhinitis, sinusitis, and nasal polyps.
