The following document serves as a summary of findings from a limited onsite
consultation conducted at the Irvine Animal Care Center on December 15th and December
16th 2014, by Dr. Jyothi Robertson of JVR Shelter Strategies, LLC and Dr. Chumkee
Aziz of the UC-Davis Koret Shelter Medicine Program. It also includes limited
recommendations from a site visit conducted by Megan Webb from February 12th to
February 14th, 2015.
The recommendations made here are based on current best practices and available
research in the field of shelter medicine. It is important for management to understand
that shelter medicine is a growing discipline that is developing new research daily.
Policies and procedures should be continuously evaluated in the context of IACC’s
population data to assure that practices adopted are creating a positive impact and
increasing the number of lives saved while working within the “care-ing capacity” of the
organization. This document does not examine population data or statistical analysis of
the shelter population. This document should be viewed as a dynamic report that will
require ongoing evaluation based on what is most current in the field.

Hamdard Laboratories (India), is a Unani pharmaceutical company in India (following the independence of India from Britain, "Hamdard" Unani branches were established in Bangladesh (erstwhile East Pakistan) and Pakistan). It was established in 1906 by Hakeem Hafiz Abdul Majeed in Delhi, and became
a waqf (non-profitable trust) in 1948. It is associated with Hamdard Foundation, a charitable educational trust.
Hamdard' is a compound word derived from Persian, which combines the words 'hum' (used in the sense of 'companion') and 'dard' (meaning 'pain'). 'Hamdard' thus means 'a companion in pain' and 'sympathizer in suffering'.
The goals of Hamdard were lofty; easing the suffering of the sick with healing herbs. With a simple tenet that no one has ever become poor by giving, Hakeem Abdul Majeed let the whole world find compassion in him.
They had always maintained that working in old, traditional ways would not be entirely fruitful. A broader outlook was essential for a continued and meaningful existence. their effective team at Hamdard helped the system gain its pride of place and thus they made an entry into an expansive world of discovery and research.
Hamdard Laboratories was founded in 1906 in Delhi by Hakeem Hafiz Abdul Majeed and Ansarullah Tabani, a Unani practitioner. The name Hamdard means "companion in suffering" in Urdu language.(itself borrowed from Persian) Hakim Hafiz Abdul Majeed was born in Pilibhit City UP, India in 1883 to Sheikh Rahim Bakhsh. He is said to have learnt the complete Quran Sharif by heart. He also studied the origin of Urdu and Persian languages. Subsequently, he acquired the highest degree in the unani system of medicine.
Hakim Hafiz Abdul Majeed got in touch with Hakim Zamal Khan, who had a keen interest in herbs and was famous for identifying medicinal plants. Having consulted with his wife, Abdul Majeed set up a herbal shop at Hauz Qazi in Delhi in 1906 and started to produce herbal medicine there. In 1920 the small herbal shop turned into a full-fledged production house.
Hamdard Foundation was created in 1964 to disburse the profits of the company to promote the interests of the society. All the profits of the company go to the foundation.
After Abdul Majeed's death, his son Hakeem Abdul Hameed took over the administration of Hamdard Laboratories at the age of fourteen.

I. PURPOSE
To establish a uniform procedure for processing license tags issued to assistance dogs.
II. POLICY
A pet license certificate and a permanent assistance license tag shall be issued when the
following requirements have been met:
1. The dog is four (4) months of age.
2. Resides within the Orange County Animal Care Services licensing jurisdiction.
3. Has been vaccinated with a rabies serum approved by the State of California and the
period of immunization covers the desired licensing period.
4. Completion of the identification tag application.
5. Affixed signature of the owner/custodian to the affidavit.
6. Certificate issued by a licensed trainer has been provided verifying dog is in training or
has been trained to provide assistance to persons with disabilities.
III. SCOPE
Applicable to all Customer Services personnel involved in the issuance of assistance license
tags.

Guide Dog: any dog that has been trained by a person licensed under Chapter 9.5 of Division 3
of the Business Professions Code. This dog is certified by a licensed guide (Seeing Eye) dog
agency and is being used by a blind person.
Signal Dog: any dog trained to alert an individual who is deaf or hearing impaired.
Service Dog: any dog individually trained to the requirements of the individual with a
disability, including, but not limited to, minimal protection work, rescue work, pulling a
wheelchair, or retrieving dropped items.
VII. PROCEDURE
A. Issuing an Assistance License Tag.
1. A Person ID and an Animal ID must be created and stored before an assistance
license tag may be issued. Query to see if the owner/custodian exists in
Chameleon.
2. If pet owner/custodian is not in the database, it is necessary to create and store
the Person ID and Animal ID.
3. Place cursor in Tag No. Box and enter license tag number preceded by L and
the last two digits of the current year (i.e. tag #000123, year 2006 enter L06-
000123). To issue owner/custodians an assistance license tag, see Licensing
Supervisor.
4. From the Tag/Link Window, tab over to the “Tag Type” and enter LIC GUIDE
(use this only if documentation has been provided showing that the animal is a
certified assistance dog).
5. Enter the total amount of money collected as 0.00.
6. Enter today’s date.
7. The “term” of the license will always remain twelve (12) months.
8. Enter the license expiration date.
9. Enter the status as “current” unless the license being processed is incomplete in
which case the status will be “temporary”.
10. Enter vaccination date and select either 12 or 36-month vaccination term.
11. Store the information. The Person ID and Animal ID will be linked under the
Tag/Link Window as one record; therefore it is extremely important to store the
information before leaving this window. The upper left corner will display the
message “Store OK!”

12. Provided the preferences were set for “Receipt on Store”, prior to issuing
licenses, Chameleon will automatically issue a receipt. To create a receipt if
preferences have not been set:
a. Pop-up (F4) the Receipt Window.
b. Verify that the Person ID and Animal ID are correct.
c. In the Item Code field, LIC GUIDE will appear. If not, close the
Receipt Window and store (F9) the Tag/Link Window. Pop-up (F4) the
Receipt Window and the Item Code, license number, Animal ID,
Department Number, Fee Type and Price must be displayed. Click on
the green “Add” button. Enter the total amount of monies received and
store (F9) the information. A window will appear giving employee the
choice to print the receipt. Make selection.
