Dozens of U.S. states are currently suing Meta for contributing to the youth mental health crisis by inserting addictive features into their products, while the U.S. Surgeon General is touring the nation to bring awareness to the growing epidemic of loneliness and isolation. The country has endured periods of low national morale, such as in the 1970s when high inflation and the energy crisis worsened public sentiment following the Vietnam War. The current mood, however, feels different. Gallup recently reported that national mental health is at an all-time low, with few bright spots to lift spirits.
To better understand how Americans are feeling and their attitudes towards mental health in general, ThinkNow conducted a nationally representative quantitative survey of 1,500 respondents and found some interesting differences among ethnic, age and gender groups.
Technology
For example, 52% agree that technology and social media have a negative impact on mental health, but when broken out by race, 61% of Whites felt technology had a negative effect, and only 48% of Hispanics thought it did.
While technology has helped us keep in touch with friends and family in faraway places, it appears to have degraded our ability to connect in person. Staying connected online is a double-edged sword since the same news feed that brings us pictures of the grandkids and fluffy kittens also feeds us news about the wars in Israel and Ukraine, the dysfunction in Washington, the latest mass shooting and the climate crisis.
Hispanics may have a built-in defense against the isolation technology breeds, owing to their large, multigenerational households, strong social support systems, and tendency to use social media to stay connected with relatives abroad.
Age and Gender
When asked how individuals rate their mental health, men rate it higher than women by 11 percentage points, and Baby Boomers rank it highest at 83%, saying it’s good or excellent vs. 57% of Gen Z saying the same.
Gen Z spends the most amount of time on social media, so the notion that social media negatively affects mental health appears to be correlated. Unfortunately, Gen Z is also the generation that’s least comfortable discussing mental health concerns with healthcare professionals. Only 40% of them state they’re comfortable discussing their issues with a professional compared to 60% of Millennials and 65% of Boomers.
Race Affects Attitudes
As seen in previous research conducted by ThinkNow, Asian Americans lag other groups when it comes to awareness of mental health issues. Twenty-four percent of Asian Americans believe that having a mental health issue is a sign of weakness compared to the 16% average for all groups. Asians are also considerably less likely to be aware of mental health services in their communities (42% vs. 55%) and most likely to seek out information on social media (51% vs. 35%).

C++ is an enhanced and extended version of C programming language, developed by Bjarne Stroustrup in 1979 as part of his Ph.D. project. Bjarne developed what he called ‘C with Classes’ (later renamed C++) because he felt limited by the existing programming languages that were not ideal for large scale projects. 6. Белки – активаторы протеаз системы свертывания крови
• Факторы V и VIII (антигемофилийный А)
выполняют сходные функции. Они обеспечивают
связывание витамин К-зависимых
активированных факторов IX и X с
фосфолипидами. Наличие таких взаимодействий
увеличивает эффективность реакций активации в
100000 раз.
• Факторы V и VIII синтезируются как полипептиды.
Оба белка содержат по 2 домена, гомологичных
церулоплазмину, по 2 гомологичных C-домена и
большой B-домен, который отщепляется при
активации факторов. Фактор VIII подвергается
протеолизу в С-концевой части В-домена, в крови
присутствует в виде набора гетеродимеров,
взаимодействующих с ионами Са.
7. Белки – активаторы протеаз системы
свертывания крови
• Факторы V и VIII активируются под действием
тромбина или фактора Xa. Тромбин
расщепляет фактор V по остаткам аргинина
(709, 1018, 1545), а фактор VIII – по остаткам
Arg 372, 740 и 1689. В активированной форме
фактор V – гетеродимер (A1-A2/A3-C1-C2), а
фактор VIII – гетеротример (А1/А2/А3-С1-С2),
субъединицы которых взаимодействуют при
участии ионов Ca. С-концевые участки этих
факторов взаимодействуют с фосфолипидами.
8. Фактор фон Виллебранда
• В плазме крови фактор VIII циркулирует в виде комплекса с фактором
фон Виллебранда (vWF) – адгезивным белком, обеспечивающим
связывание тромбоцитов с субэндотелиальными структурами и
склеивание тромбоцитов при агрегации. Кроме того, vWF повышает
стабильность фактора VII и его защиту от инактивации протеином С.
. Недостаточность этого фактора приводит к увеличению риска
кровоизлияния (болезнь фон Виллебранда), в сочетании с
гипертонией увеличивает риск
мозгового кровоизлияния.
Болезнь фон Виллебранда — врождённое отсутствие мультимерных
форм фактора фон Виллебранда, необходимых для агрегации
тромбоцитов. Проявляется как геморрагический синдром,
характеризующийся тенденцией к кровотечению из слизистых
оболочек, увеличением времени кровотечения
9. Тканевой фактор (III)
• Трансмембранный гликопротеид, выполняющий функции рецептора
фактора VII, модулирующий его активность. Активность фактора III
зависит как от белка, так и от фосфолипидов мембраны клеток.
Связывание одноцепочечной формы фактора VII с фактором III
изменяет структуру фактора VII так, что становится возможным его
расщепление на двухцепочечную форму, которая в комплексе с
фактором III активирует X и XI факторы свертывания крови, что
инициирует каскад коагуляции.
• Высокое содержание фактора III характерно для мозга, миокарда,
клеток эпидермиса и эпителия, выстилающего слизистые оболочки.
Клетки эндотелия и моноциты могут экспрессировать фактор III под
действием цитокинов, эндотоксина, окисленных ЛНП и иммунных
комплексов, обеспечивая образование внутрисосудистых тромбов при
ряде заболеваний.

атеросклеротических бляшек. Фибрин и продукты
его расщепления стимулируют пролиферацию

14. Кинин-калликреиновая система
• Кинин-калликреиновая система состоит из небольших
полипептидов, а также набора активирующих и деактивирующих
компоненты ферментов.
• Освобождение кининов из кининогенов происходит под действием
трипсиноподобных сериновых протеиназ - калликреинов,
локализованных в плазме крови (плазменный калликреин) и в
тканях некоторых органов

артериального давления, коагуляции и возникновении болевых

калликреином.
• ВМК и НМК образуюстя в результате альтернативного сплайсинга
одного гена.
• Прекалликреин - предшественник калликреина плазмы крови
является гликопротеином, представленным одной пептидной
цепью, состоящей из 619 аминокислотных остатков. Синтезируется
прекалликреин в гепатоцитах.
15. Кининогены
• В плазме крови человека присутствуют два кининогена:
высокомолекулярный кининоген (ВМК) и низкомолекулярный
кининоген (НМК), синтез которых кодируется единым геном,
локализованным в хромосоме 3.
• Ген кининогена содержит 11 экзонов, девять из них образуют три
триплетных экзона. Экзон 10 содержит общую для двух кининогенов
кининовую последовательность (экзон 10a) и особую С - концевую
последовательность ВМК (экзон 10b), а экзон 11 кодирует уникальную С
- концевую последовательность НМК. Альтернативный сплайсинг
первичного транскрипта кининогенового гена образует две различные
мРНК, специфичные соответственно для ВМК и НМК
• Кининогены могут обратимо связываться с тромбоцитами,
нейтрофилами и эндотелиальными клетками
• Во всех случаях для связывания кининогенов с клетками необходим
Zn2+. При связывании с клетками ВМК контролирует их функции.
Взаимодействие ВМК (D3) с тромбоцитами, скорее всего через
тромбоспондин, угнетает активность тромбоцитарного кальпаина и
подавляет агрегации тромбоцитов, стимулированной тромбином,
препятствуя тем самым связыванию последнего с клетками

Связывание фактора XII (фактора Хагемана) с
компонентами субэндотелиального слоя,
активированными тромбоцитами, мицеллами из
фосфолипидов или бактериальными полисахаридами
изменяет его конформацию, так что:
1) он становится чувствительным к расщеплению
калликреином; 2) в комплексе с кининогеном повышает
активацию прекалликреина, т.е образуется большое
количество фактора активированного фактора XIIa
Начальный этап – расщепление фактора XII по Arg353 с
образованием фермента, состоящего из двух
полипептидных цепей, соединенных дисульфидными
связями. Протеазная активность находится в легкой
цепи, а в тяжелой – домены взаимодействия.
Калликреин может расщеплять далее по Arg334 и Arg343,
что приводит к потере второй цепи и способности
взаимодействовать с партнерами.
17. Активация фактора XI
• Далее фактор XIIa активирует фактор XI,
расщепляя в нем связь с Arg369, образуя
легкую и тяжелую цепи, соединенные
дисульфидными мостиками (как и в случае с
калликреином).
• Наследственный дефицит фактора XII и
каллекреина не проявляется как повышенная
кровоточивость
18. Активация фактора IX
• Фактор XIa активируется в результате
расщепления связей Arg145-Ala146 и Arg180-
Val181 с освобождением пептида активации.
Может происходить как под действием XIа,
так и комплекса фактора VII с тканевым
фактором в присутствии Са, причем оба пути
примерно одинаково эффективны.
Симптомы кровоточивости у больных с
дефицитом фактора XI менее выражены, чем
у больных с дефицитом факторов IX или VIII,
что свидетельствует об их физиологической
значимости.
19. Активация фактора X
• Активация фактора X под действием фактора IXa
происходит на поверхности фосфолипидов при
участии ионов Са и фактора VIIIa, который
обеспечивает связывание и оптимальное
взаимодействие факторов XIa и X, увеличивая
скорости реакции в 100 000 раз.
• И вновь фактор X состоит из двух цепей,
взаимодействующих за счет образования
дисульфидных связей, образуются они вследствие
расщепления связей Arg139-Arg140 и/или Arg142-
Ser143. Активация фактора X происходит вследствие
расщепления связи Arg194-Ile195 в N-концевой части
цепи с освобождением пептида активации. Другой
важный компонент активации фактора X –комплекс
тканевого фактора и фактора VIIa
20. Внешний путь активации
• При контакте крови с клетками,
экспрессирующими тканевой фактор (ТФ), фактор
VII связывается с ТФ с Kd 3 нм. Образование
комплекса увеличивает чувствительность фактора
VII к протеолизу, приводящему к расщеплению
связи Arg152-Ile153. Активация происходит под
действием следов факторов X и IX, а также
вследствие аутоактивации. В плазме около 1%
фактора VII постоянно находится в
двухцепочечной форме, которая без ТФ не
ингибируется антитромбином III и не может
активировать свои естественные субстраты:
факторы X и IX.
• Фактор VII может активироваться и под действием
фактора XIIa
21. Образование тромбина
• Фактор X, активированный на поверхности фосфолипидов
комплексом IXa-VIIIa или комплексом фактор VIII- TФ,
образует комплекс с фактором V и протромбином. В результате
последовательного расщепления двух связей в молекуле
протромбина (Arg320-Ile321) образуется мейзотромбин,
состоящий из двух половин молекулы, соединенных
дисульфидной связью.
• Мейзотромбин связывается с протеином С и активирует
вазоконстрикцию. После гидролиза в мейзотромбине связи
Arg271-Thr образуется N-концевой фрагмент активации
протромбина и молекула -тромбина, состоящая из двух цепей,
соединенных дисульфидной связью. А цепь состоит из 49
аминокислот, но у человека от нее может отщепляться 13
аминокислот без изменения активности. Протеазный и субстрат-
связывающие центры находятся на В-цепи (259 аминокислот).
• После протеолиза тромбин диссоциирует из комплекса и
обеспечивает следующие функции: прокоагулятная
антикоагулянтная, вазоактивная и митогенная. Он обеспечивает
превращение фибриногена в фибрин и активирует фактор XIII,
стабилизирующий фибрин. Кроме того, он активирует свое
образование, оказывая прокоагулянтное действие.
22. Функции фосфолипидов
• Ограничение свертывания крови участком
повреждения достигается за счет локализации
участников реакции на поверхности
поврежденных или стимулированных клеток.
При этом необходимо участие клеток,
содержащих ТФ, и активированных тромбоцитов.
• Наружный слой мембраны содержит холиновые
фосфолипиды, внутренний - фосфатидилсерин и
фосфатидилэтаноламин, обладающие
прокоагулянтными свойствами. Асимметричное
распределение определяется флиппазой и АВС-
транспортером. Повышение внутриклеточной
концентрации Са ингибирует транспортер,
снижает поверхностный уровень
прокаогулянтных фосфолипидов.
23. Система противосвертывания:
• Аннексин V: секретируется из
эндотелиальных клеток и оказывает
локальное антикоагулянтное действие,
связываясь с прокоагулятными
фосфолипидами (напр. Фосфатидилсерином)
• Ингибиторы сериновых протеаз: в основном
из семейства «серпинов», кроме того, ИПТФ
(ингибитор пути тканевого фактора) и -
макроглобулин.
24. Система фибринолиза: плазминоген и
плазмин
• Фермент, расщепляющий фибрин – плазмин, образуется
при активации плазминогена (24 S-S связи, 5 крингл-
доменов).
• Крингл-домены связывают плазминоген с фибрином. От
молекулы плазмина освобождается N-концевой пептид,
что активирует гидролиз фибрина. Рабочая форма
плазмина –две цепи с соединенные дисульфидными
связями. Наиболее чувствительна к протеолизу
плазмином С-концевая часть цепи фибрина. Полная
потеря свертываемости – после отщепления одного из D-
доменов, являющегося периферическим участком
полимеризации.
• Один из тканевых активаторов плазминогена –урокиназа
(EGF, крингл и каталитический домен).
• Культура гемолитического стрептококка производит
стрептокиназу, образующую комплекс с плазминогеном, в
результате чего формируется активный центр протеазы.
