  • #3 Failsafe’s: Lack of, not monitored regularly, no contingency for absence etc. Guideline’s SOP’s – not ratified, not in Trust format, out of date for review, don’t reflect current practices and/or National standards Risk management – screening not on risk registers, risk de-escalated with no evidence of risk reduced, not clear how risks from TSSG escalated Incident management: Screening incidents not reported to QA and commissioners, lack of awareness of guidance on managing incidents in NHS screening programmes, no evidence that action plans are followed up to completion Data collection and ratification: manual collection, no evidence of engagement with IT/data departments to find electronic solutions or lack of escalation of issues, lack of scrutiny, sign off within the Trust before submitted externally Job description and role structures: Screening roles not identified within JD’s, responsibilities not defined, who does what? Who has overall responsibility for screening? Audit: Screening audits not part of the maternity audit schedule, no audits of screening pathways undertaken, no action plans developed from findings Escalation processes: Not clear how risks identified in screening pathways are escalated and by whom, not clear how TSSG reports upwards, not clear how items are entered onto risk register, etc.
