NR509 Advanced Physical AssessmentDebriefing
SessionGuidelines and Grading Rubric
Purpose
Debriefing is a critical conversation to reframe the context of a
situation and to clarify perspectives and assumptions, both
subjectively and objectively. Debriefing yields better retention,
deeper learning, and increases the likelihood of the transfer of
new or reinforced knowledge, skills and abilities to the clinical
or broader healthcare setting. During the debriefing process,
students reflect upon their simulation experience and revisit
their assessments, interventions, and observations. The faculty-
facilitator coaches the students to review the patient data and
reflect upon the interventions performed in response to the
clinical situation presented during the simulation experience.
Course Outcomes
Debriefing session activities are guided by the following Course
Outcomes (COs):
1. Apply advanced practice nursing knowledge to collecting
health history information and physical examination findings
for various patient populations. (PO 1, 2)
2. Differentiate normal and abnormal health history and
physical examination findings. (PO 1, 2)
3. Adapt health history and physical examination skills to the
developmental, gender-related, age-specific, and special
population needs of the individual patient. (PO 1, 2)
The purposes of the debriefing activity are to: (a) reframe the
context of a situation and to clarify perspectives and
assumptions, (b) foster high participant engagement through the
use of interpersonal and communication skills, (c) explore and
analyze actions and thought processes as well as emotional
responses to the physical assessment simulation, and (d)
facilitate transfer of knowledge gained from simulation
experiences to actual clinical practice.
Due Date
The date and time for these synchronous activities will be
posted in the course announcements by the course instructor.
There is not a late assignment policy for debriefing sessions.
Students who do not satisfy the debriefing session guidelines as
outlined below for any given week will complete the NR509
Alternative Writing Assignment (See the NR509 Alternative
Writing Assignment Guidelines and Grading Rubric); otherwise,
a grade of zero (0) will be recorded in the gradebook
Total Points Possible: 25 Points
Preparing for the Activity and Guidelines
1. Complete all physical assessment assignments and concept
labs assigned for the week
2. Answer all reflections questions following each physical
assessment assignment in Shadow Health
3. Save or print a copy of your reflection question responses to
refer to during the debriefing session
4. Register to attend one debriefing session per week using the
registration links posted in the course announcements by the
course instructor
5. Arrive to the scheduled WebEx meeting on-time and
appropriately connect to the WebEx audio system
6. During each debriefing session, every student is expected to:
a. Express ideas and information in a clear and concise manner
in order to facilitate an open exchange of ideas and information
with peers and faculty
b. Be professional and respectful and listen actively to what
others are saying
c. Demonstrate application of knowledge with regard to the
evidenced-based principles of advanced physical assessment,
patient management and safety, and transference of knowledge
and understanding to the actual clinical environment

Case Assignment
The B-LIT, LLC is a new startup that wishes to sale their new
phone to the public called LIT-phone, a more secure version of
the phone to business organizations, called the LIT-phone B+,
and highly secure version of the phone, called the LIT-phone
G+, to the government. Due to the fear of corporate espionage
and government security requirements, there are many security
concerns that must be addressed. As a security professional,
you have been employed to design a network infrastructure for
their two campuses located in Orlando, FL and Anaheim, CA
based upon the following specifications:
1. There needs to be a constant connection between the two
locations that can carry at least 50 Mbps of data.
2. Each facility has three floors. The buildings are rectangular
with each floor being 350’x350’.
3. There will be 200 network connections on each floor with an
additional 100 network connections in the data centers located
on the third floor of each building.
4. The primary data center will be located at the Orlando
location.
5. There will be a failover data center at the Anaheim location.
6. Each location should be protected from intrusions that are not
limited to state change attacks.
7. The Orlando location will house the two secure development
teams. As such, it will need the most security. To further
complicate the design, there will be database servers and the
corporate Web servers housed at that location as well.
8. There will be database servers located at the Anaheim site.
9. The servers must have redundancy.
10. The solution must have a plan to verify security measures.
Your job is to develop a network design to meet the
requirements above.
1. You should submit a network drawing listing the network’s
topology including any necessary hardware.
2. You should list any recommended cable.
3. You can recommend wiring closets wherever you need them.
4. You should recommend ways to assure that you are not
getting attacked.
5. You should build traps to stop attackers.
6. You should recommend any WAN or wireless technologies.
7. You should recommend any technology needed in the data
center for high availability.
8. Justify your recommendations.
Alternative Writing Assignment Guidelines and Grading
RubricPurpose
As a family nurse practitioner, you must possess excellent
physical assessment skills. This alternative writing assignment
mirrors the discussion content of the debriefing session and will
allow the student to expand their knowledge of physical health
assessment principles specific to the advanced practice role.
Course Outcomes
This assignment is guided by the following Course Outcomes

2. Differentiate normal and abnormal health history and
physical examination findings. (PO 1, 2)
4. Adapt health history and physical examination skills to the
developmental, gender-related, age-specific, and special
population needs of the individual patient. (PO 1, 2)
The purposes of this assignment are to: (a) identify and
articulate advanced assessment health history and physical
examination techniques which are relevant to a focused body
system (CO 1), (b) differentiate normal and abnormal findings
with regard to a disease or condition that impacts the body
system (CO 2), and (c) adapt advanced assessment skills if
necessary to suit the needs of specific patient populations (CO
4).
NOTE: You are to complete this alternative writing assignment
ONLY if you had not participated or do not plan to participate
in a debriefing session for the given week.
Due Date: This alternative written assignment is due no later
than the Sunday of the week in which you did not attend the
weekly debriefing session. For example, if you did not attend a
debriefing session for Week 3, this written assignment is due
the Sunday at 11:59 p.m. MT of Week 4.

Total Points Possible: 25 PointsRequirements:
1. This paper will be graded on the quality of the information,
inclusion of evidence-based scholarly resources, use of
citations, use of Standard English grammar, and organization
based on the required components (see the paper headings and
content details below).
2. Submit to the appropriate location in Canvas by 11:59 p.m.
MT on Sunday of the week due.
3. The length of the paper is to be no less than 1,500 words,
excluding title page and reference list.
4. Create this assignment using Microsoft (MS) Word. You can
tell that the document is saved as a MS Word document because
it will end in “.docx.”
5. APA format (6th edition) is required in this assignment,
explicitly for in-text citations and the reference list. Use 12-
point Times New Roman font with 1 inch margins and double
spacing. See the APA manual for details regarding proper
citation. See resources under Course Resources, “Guidelines for
Writing Professional Papers” for further clarification.
6. Organize the headings and content of your paper using the
outline below:
a. Identify and briefly discuss the body system selected for the
topic of this paper
b. Discuss the physiology (structure and function) of the body
system including details about the major organ systems (if
applicable)
c. Discuss relevant health history questions (subjective data)
pertaining to the body system
d. Discuss an overview of the objective data and expected
normal physical examination findings for this body system
e. Discuss special physical assessment examination techniques
or procedures specific to assessing this body system
f. Discuss how you might adapt your physical assessment skills
or techniques to accommodate each of the following specific
populations:
i. Infant/pediatric
ii. Pregnancy
iii. Geriatric
g. Identify one major disease or disease process that may
significantly impact this body system
h. Discuss the expected abnormal physical examination findings
that may be associated with this disease or disease process
i. Summarize the key points
Preparing the Paper:
1. Select a focused body system from the weekly lesson which
corresponds with the week of the written assignment.
2. Carefully read and review the selected body system in your
course textbooks.
3. Incorporate at least one scholarly peer-reviewed journal
article that relates to the body system. It may be useful to
identify an article that relates to a disease that impacts the body
system.
4. The paper must clearly articulate the relevance of advanced
physical assessment skills, techniques, application of advanced
practice knowledge, and assessment modification (when
necessary) to accommodate for specific patient populations.
5. Provide concluding statements that should summarize key
points of the overall assignment content.
6. In-text citations and reference page(s) must be written using

During the debriefing process, students reflect upon their simulation experience and revisit their assessments, interventions, and observations. The faculty- facilitator coaches the students to review the patient data and reflect upon the interventions performed in response to the clinical situation presented during the simulation experience. Course Outcomes Debriefing session activities are guided by the following Course Outcomes (COs): 1. Apply advanced practice nursing knowledge to collecting health history information and physical examination findings for various patient populations. (PO 1, 2) 2. Differentiate normal and abnormal health history and physical examination findings. (PO 1, 2) 3. Adapt health history and physical examination skills to the developmental, gender-related, age-specific, and special population needs of the individual patient. (PO 1, 2) The purposes of the debriefing activity are to: (a) reframe the context of a situation and to clarify perspectives and assumptions, (b) foster high participant engagement through the use of interpersonal and communication skills, (c) explore and analyze actions and thought processes as well as emotional responses to the physical assessment simulation, and (d) facilitate transfer of knowledge gained from simulation

Students who do not satisfy the debriefing session guidelines as outlined below for any given week will complete the NR509 Alternative Writing Assignment (See the NR509 Alternative Writing Assignment Guidelines and Grading Rubric); otherwise, a grade of zero (0) will be recorded in the gradebook Total Points Possible: 25 Points Preparing for the Activity and Guidelines 1. Complete all physical assessment assignments and concept labs assigned for the week 2. Answer all reflections questions following each physical assessment assignment in Shadow Health 3. Save or print a copy of your reflection question responses to refer to during the debriefing session 4. Register to attend one debriefing session per week using the registration links posted in the course announcements by the course instructor 5.
