Jane, a 9-year-old Latina female, was brought to the clinic for excessive shyness, difficulty falling asleep, and inability to be alone in the house. In addition, she had begun to brood that the family dog would get sick and die. Her mother just returned home following 3 months of psychiatric hospitalization for severe depression. The mother's illness followed her husband’s (Jane's father) leaving the family in order to live with another woman, whom he intended to marry. Jane had been reluctant to attend school in kindergarten and first grade, but this had reportedly been addressed by the school, which has firm limits about school attendance. At home, Jane often sleeps in her parent's bed. In the past 2 years, the problems have worsened considerably. Frequently, Jane fakes illness on school days, and her school performance has begun to deteriorate. Recent testing has revealed reading difficulties that appear to be longstanding in duration, and tutoring has begun. This academic year she is repeating third grade. Jane has taken this poorly and has no friends among her classmates. During the interview, Jane spoke with reluctance and appeared sad. She seemed preoccupied with her dog, Spot, and feared that the dog might fall ill. When asked directly, she said she did not sleep well unless she was in the same bed as her mother. Although she admitted she could not stay in the house alone even for 10 minutes, she claimed this was almost never a problem as long as her older sister, a neighbor, or a housekeeper was with her, which was almost all the time. Case : Three months into Janet's freshmen year, her parents were summoned by the dean of the small out-of-town college she was attending to come and pick up their daughter. CASE STUDY Dean is a White 16-year-old. He is a sophomore at George Washington Carver High School. He lives with his father and his stepmother in a semirural community in the South. His father and mother divorced when Dean was 8 years old, and both parents remarried shortly after the breakup. The Case of L Presenting Problem Client presented in the emergency room (ER) having been brought in the previous night by her parents. Following an argument with her parents, L cut her right wrist. L's mother reported that L started screaming rapidly and became physically violent toward her prior to cutting her own wrist. Psychological Data L is a 17-year-old Hispanic female who resides in Pennsylvania with her mother, father, and older sister. She is in 11th grade at the local public school. L appeared to be of average to above-average intelligence, as she was able to respond to numerous questions in an articulate and intelligent manner. She was well versed about world history and current affairs. Her mother confirmed that she has done well in school, maintaining a B+ average and participating in various school activities (e.g., chorus, school paper) until last year. L slowly dropped out of many activities she liked in the past. Her mother noticed about 8 months ago that L had also begun having difficulty doing schoolwork. Erratic behavior arose during episodes when L also became irritable and explosive. During these repeated episodes, she became quite defiant, cut classes, had to be placed in school detention, and had even assaulted the principal. L has numerous friends and believed she can relate to all types of people. She has a boyfriend who adores her, but she said she doesn't feel the same about him. The school counselor confirmed that L is outgoing, popular, and smart; but during these episodes she became another person, one who is very violent and difficult. Medical History A physical examination by a staff doctor revealed superficial cuts on L’s left and right wrist. The cuts appeared to be a few weeks old. Case study 2: Brandon is an African-American eight-year-old child in third grade, who is in danger of having to repeat the grade. He continues to struggle with being able to concentrate in class, and says he is “bored” when students have to just sit and read; sometimes he talks and walks around the class and other times he draws pictures and pays no attention at all. He frequently gets sent to the principal’s office for this behavior, but does not seem to mind because the principal lets him play and gives him snacks (as he always complains about being hungry). Often the principal compliments him on his artwork. When his parents were told about his behavior, they were exasperated. The parents said he behaves even more poorly at home, stealing and breaking his siblings’ things and sneaking food into his room. "The Predicament" Case Study Discussion Jenny completed her high school education and dreamed of going to college. After 10 years and two children, she finally realized her dream. She started classes at an online university because it matched her current work schedule and allowed her the flexibility to spend time with her children. She completed her first class with excellent grades and was looking forward to her next class session. Just when things seem to be running smoothly, other significant events happened. Jenny’s office had a downsizing, and several individuals lost their jobs. Fortunately, Jenny was one of the employees who was able to stay with the company. However, her boss, Tom, changed her hours and assigned her more responsibility. This change happened during Week 2 of her 5-week session. Getting adjusted to her new duties required a lot of Jenny’s time, and she was too exhausted at the end of the day to complete her coursework.
