Case report on post streptococal glomerulonephritis

1. CASE REPORT ON POST STREPTOCOCCAL
GLOMERULONEPHRITIS
An 7-year old boy was admitted inour hospital with a two-day history of fewer,tonsillopharyngitis,
and stomach ache associatedwith vomiting,periorbital edemaand body edema.A boy was
hospitalizedfor9 days. Biochemical and clinical parameters were observed.The boy was 40kg
weight,140cm high,dispnoic,euhydric,with expressededemaandblood pressure 175/120 mmHg.
After the hematologicanalysesthe next valuesof measuredparameterswere recorded:
erythrocytes3.1x1012/L, hemoglobin94g/L, hematocrit0.31 L/L, leukocytes12.4x109/L, (neutrophils
0.58 x109/L, lymphocytes0.36x109/L, monocites0.06 x109/L). The valuesof ureawere 3.2, 2.9, and 3.1
mmol/Linthe first, fifthandtenthday,respectively.The valuesof creatininewere 41,69, and 48 mol/L
inthe first,fifth,tenthday,respectively;clearance was85 mmol/L;albuminwas29 and 30 g/dL inthe
firstand tenthday,respectively;Na+150 mmol/L,K+4.8 mmol/L,alkaline phosphatase519-476 U/L,
antistreptolysine Otest600 U/mL, C reactive protein
After the urine analysis, the valuesof erythrocyteswere 15-20per HPF,and the valuesof leukocyets
were 2-3 HPF.
ECHO of abdomen: leftkidneywithoutchanges,rightkidneyindiameter78x40mm.Diuresiswas700ml
per24h
DIAGNOSIS:The valuesof biochemical andclinical parametersrecordedatthe admissionindicatedthat
the patientdevelopedPSGN.
TREATMENT; Afterhospital admission,patientwastreatedwith –
penicillinduringthe periodof 10 days,
antihypertensive medications(captopril,furosemide),
includingrestricteddietof salt.
Afterthe treatment,patientbecame better.Ondemissionitwasfoundproteinuriaand
microhematuria.
DISCUSION
Post-streptococcal acute glomerulonephritisisone of the mostimportantandintriguingconditionsin
the pediatricnephrology.Althoughthe eventualoutcome isexcellentinmostcases, PSAGN remainsan

2. importantcause of acute renal failure andhospitalizationforchildreninbothdevelopedand
underdevelopedareas[5].Post-streptococcal acute glomerulonephritisremainsanimportantnon
suppurative complicationof groupA streptococcal infectionworldwide.The estimatedworldwide yearly
burdenof PSAGN is472,000 cases;approximately404,000 of those occur inchildren
CONCLUSION
PSGN is veryseriousdisease,whichleavessevere complicationsif the validtherapywithpenicillinisnot
usedinproprietytime,duringthe recommendedperiodof 10day
