2. What is RevolutionPHR?
RevolutionPHR is a secure way to
view portions of your eye care PHR
(Personal Health Record) online.

3. RevolutionPHR allows you to:
➤ Access information from your exams online
➤ Confirm your appointments online
➤ Look up your glasses or contact lens prescriptions
➤ Communicate with your doctor directly and securely

4. Logging on
➤ If you do not yet have a login, call our office at
507.645.9202 to request a username and password
➤ Visit https://www.revolutionphr.com/
➤ Enter your login information in the top right boxes

6. Managing your information
➤ Click on the section marked “About Me”
➤ Verify that your contact information is correct
➤ View your medical records/view your lens
prescriptions

9. Messaging your doctor securely
➤ Your doctor is listed in the upper right-hand corner of
the page
➤ Click “send message”
➤ Select “To:”
➤ Choose your doctor send a message
