3. A 24-year-old male patient
Roney Dewan reported to
Prosthodontic Department,
BSMMU after
dislodgement of his left
central incisor tooth crown-
treated last year by a local
dentist, with the complaints
of
 Unpleasent look
 Difficulty in speech
 Also patient wants to
restore that tooth.

4. HISTORY OF PRESENTING ILLNESS:
According to the statement of the patient, he
was relatively all right ten years back, then he
got injured by sudden trauma in the upper
anterior segment.
Then he remained untreated for the next nine
years. Suddenly he felt pain and swelling on
the left central incisor tooth.
For that reason he went to a local dentist and
that tooth endodontically treated and restored
with Full veneer crown, which was dislodged
last month. Now he comes to our department
for better management.

5. Medical History: Nothing contributory
Dental History: That tooth was endodontically treated also
restored with FVC.
Patient’s expectation: Restoration of aesthetics and function.

6. Clinical examination:
Extra oral examination:
Face is bilaterally
symmetrical.
No scar mark on the face.
TMJ- Normal.
Mouth opening: 40 mm

Arch length-Large
Arch form: Tapering.
Tongue: Normal.
Fractured upper left center
incisor.

7. Diagnosis:
Horizontal fracture of
upper left central incisor
tooth.
Investigation: Xray. PA
view.

8. Treatment plan:
Restoration of tooth by Fiber post and core followed by Full
Veneer crown.
