• Advanced dental therapeutic techniques have made it possible to keep teeth that were
previously thought to be irreparable.
• Hemisection procedures are used to preserve as much tooth structure as possible.

5. D E F I N I T I O N :
• Hemisection refers to sectioning of a mandibular molar into two halves followed by removal
of the diseased root and its coronal portion. The retained root is endodontically treated and
the furcation area is made self-cleansable.
• Hemisection is the sectioning of teeth with multiple roots, the removal of the damaged root
and its associated crown piece, and the preservation of the healthy root (with crown). When
periodontal, resorption, perforation, or caries damage is limited to one root and the other
root is still largely healthy.

Periodontal Indications:
• Only one root of a tooth with several roots suffers from severe vertical bone loss.
• Complete destruction of the furcation.
• The roots of adjacent teeth are too close to one another, making it difficult to maintain proper
cleanliness in close quarters.
• Serious root exposure as a result of dehiscence.

18. D I S C U S S I O N :
• Hemisection is a useful alternative procedure to save those multirooted teeth which have been
indicated for extraction.
• .Hemisection of mandibular molar may be viable treatment modality when one root has poor
treatment prognosis, and the other root is healthy and that portion of tooth can be act as abutment.
• Correct case evaluation is a key to this treatment's success
• We consider that hemisection, a multidisciplinary approach to treatment that combines endodontic,
restorative, and prosthodontic procedures, represents an additional option for maintaining teeth
and bone structure
• Nevertheless, hemisection is a viable option to be considered before the extraction of molars,
especially in the presence of conditions such as severe vertical bone loss (one root of a
multirooted tooth), furcation destruction, unfavorable proximity of root.

Basten et al. reported that 92% of all resected molars survived an average of 12 years,
Langer et al. found that 36% of root-resected mandibular molars failed over 10 years, most commonly due
to endodontic or restorative problems and not periodontal disease.

with an average reported failure rate of 13.1% among hemisected teeth.
Fugazzotto found that cumulative success rates were 96.8% for root-resected molars while 97.0% for molar
implants

• Hemisection is an alternative, effective, and conservative treatment modality over
conventional procedure or extraction of periodontally and endodontic affected teeth.
• Also reduces the financial burden, psychological trauma, and occlusal dysfunction
associated with tooth loss
• Hemisection has received wide acceptance as a conservative dental treatment and teeth so
treated have fulfilled the demands of function. A dentist should discuss this with his patients
during consideration of treatment options so that the patient can choose best and
conservative treatment available

The treatment plan was explained to the patient
Oral prophylaxis was done out before commencement of the root canal procedure
Access opening was carried out using round bur and safe end bur followed by pulp extirpation
Working length was determined using apex locater and confirmed radiograph
Biomechanical preparation was carried out using rotartey file system following standard irrigation protocols
master cones were selection was done and Gutta percha was shivered off to level of cemento enamel jution and
and obturation was done in the mesial the root of 46
. Post-endodontic restoration was done using composite with 46
under a local anesthesia, full thickness flap was reflected
The vertical cut method was used to resect the crown.
A long shank tapered fissure carbide bur was used to make vertical cut toward the bifurcation area.
A fine probe was passed through the cut to ensure separation
The distal root of 46 was extracted
Socket was irrigated adequately with saline to remove bony chips.
The tooh now looks like a premolar
Crown preparation is done irt 46 47 48
A 3 bridge unit prosthetic rehabitation is planned
