Restoring Worn, Chipped & Broken Teeth

  1. 1. Restoring Worn, Chipped & Broken Teeth dayanorthodontics.com
  2. 2. In adult dentitions the patient has been wearing their teeth down in an uneven way due to their previous uneven bite. Once the wear of the tooth breaks through the enamel, the dentin will wear faster leaving the enamel unsupported and chipping off unevenly. dayanorthodontics.com Restoring Worn Teeth
  3. 3. Restoring Worn Teeth dayanorthodontics.com The top and bottom teeth are straight, and from the front it looks like we may be finished. A special retainer design is used to maintain tooth positions until the final restoration is placed for ideal function, stability and esthetics. However the lower left central incisor is worn down from the previous excess forces on it, and it does not touch the top tooth. If we leave it like this, anterior guidance is not ideal and the top tooth will likely fall back lingually. The patient will be unsatisfied with the orthodontic relapse of the top teeth.
  4. 4. dayanorthodontics.com Restoring Worn Teeth Upper right lateral incisor incisal edge severely worn due to crowding in upper arch, missing lower lateral incisors in presence of bruxism habit. This distorts anterior guidance and the lower cuspid guides through upper right lateral incisor. Orthodontic alignment completed, to setup new anterior guidance using upper right cuspid with lower right second bicuspid. Case continued on next slide
  5. 5. dayanorthodontics.com Restoring Worn Teeth Case continued from previous slide Upper right lateral incisor restored and patient biting in centric relation, which is coincident with centric occlusion. Patient shown in right lateral excursion with guidance of upper right cuspid and lower right bicuspid, and in forward mandibular excursion with guidance provided by central incisors only. Upper right lateral incisor not involved in anterior guidance.
  6. 6. Severe and uneven wear related to crooked and uneven bite. A restorative plan for an ideal bite would involve periodontal and endodontic procedures to compromise and remove healthy bone, gum and pulpal tissues to support a restorative plan. dayanorthodontics.com Restoring Worn Teeth Case continued on next slide Even gingiva, uneven incisal edges Decoupled anteriors to be restored Severe wear to be restored As orthodontics completed room for restorations is evident. Now that foundation is aligned, the dentist and patient can enjoy the benefits of ideal dentistry into an ideal foundation, without compromising tissues for restorative purposes.
  7. 7. A wonderfully restored dentition to excellent function and smile. Case continued from previous slide dayanorthodontics.com Restoring Worn Teeth Pre-Orthodontics Post-Orthodontics and Pre-Restorative
  8. 8. dayanorthodontics.com Severe and uneven wear related to crooked and uneven bite. Dentist and patient can enjoy ideal dentistry without need to move gum line or bone tissue unnecessarily. A wonderfully restored smile. Restoring Worn Teeth
  9. 9. dayanorthodontics.com Top front teeth in need of crown restorations, however present positions would compromise the results. Patient in middle of invisible orthodontics. Teeth are now positioned to allow excellent restorative results. Restoring Broken Teeth
  10. 10. dayanorthodontics.com Restoring Worn Teeth Chipped and worn upper left lateral incisor due to poor bite. Invisible orthodontics to correct bite and provide room to restore tooth. Smile restored to excellent function, health and esthetics.
  11. 11. dayanorthodontics.com Severe wear with no room to place restorations. Restoring Worn Teeth There is now room to replace missing tooth material without further reduction of teeth. Restorations placed without the need to go below gum tissue or reduce bone height.
  12. 12. dayanorthodontics.com Teeth decoupled to prepare for restorations. Dentist and patient can enjoy ideal dentistry. A wonderfully restored smile. Restoring Worn Teeth
  13. 13. dayanorthodontics.com Restoring Worn Teeth Severe and uneven wear of top teeth and lower front teeth overgrown to give a reverse smile line. Patient begins to restore worn top teeth as invisible orthodontics makes room for restorations by moving lower front teeth down. The restored smile without the need to remove gum tissue or bone on lower teeth to move the smile down.
  14. 14. dayanorthodontics.com Restoring Worn Teeth Uneven wear related to crooked and uneven bite. Smile during invisible orthodontics making room for restorative dentistry. A wonderfully restored smile.
  15. 15. dayanorthodontics.com Fractured tooth has little sound tooth exposed outside the alveolar bone. Extruded tooth can be restored without encroaching on biological width limitations. Restoring Broken Teeth
  16. 16. dayanorthodontics.com Upper right central incisor has recession and bone loss and is going to be replaced with implant. The concern is that final tissue height will be too high compared to adjacent central incisor. Using esthetic orthodontic techniques patient is able to grow bone and gum tissue into area for future implant. Start Erupting Tooth moves down into appliance space Case continued on next slide Restoring Broken Teeth
  17. 17. Patient now has extra tissue in area for more predictable implant success. Bonded lingual retention as bone and gum tissue mature for implant. Before After During dayanorthodontics.com Case continued from previous slide Restoring Broken Teeth
