Infection on Systemic

2. Periodontal disease can theoretically
affects systemic health by one or more
of several mechanisms
1) Direct extension of periodontium infection from to deeper
adjacent tissues ( facial plane, sinus or brain infections).
2) Passage of inflammatory mediators from the
periodontium into the circulation (e.g. atherosclerosis).
3) Penetration of oral bacteria into the systemic circulation to
(endocarditis, thrombosis/atherosclerosis).
4) Inhalation or ingestion of oral bacteria(gram-ve), their products
(pulmonary or gastrointestinal tract infection- lung abscess).

a role in the pathogenesis of important
systemic diseases
infective endocarditis,
atherosclerosis
pulmonary infections,
diabetes mellitus
preterm low birth weight child.

6. Periodontal infection as a possible risk
factor for pregnancy complications
Throughout
gestation ( in
normal healthy
condition )
the amniotic level of
prostaglandins, especially
PGE2, rises steadily
until a sufficient level is
achieved in the amnion
to induce labor and
delivery

So additional inflammatory PGE2 and TNF-α +steadily rise of
gestational PGE2 may pose a potential threat for pregnancy
outcome (uterine contraction before the normal date of birth)
Periodontal

and inflammatory mediators
as PGE2 and TNF-α
leading to onset of
preterm labor.
8. Periodontal infection & Diabetes

After periodontal treatment and reduced inflammation
might restore insulin sensitivity over time& improve
glycemic control

such as pulmonary
abscess infection
Periodontal infection does not cause diseases but complicate medical problems.
Surgical space : is a loose tissue between anatomy and muscle and it’s very easy for the infection to go through it.
Periodontal disease is almost painless, patient is being annoyed when periodontal abscess appears.
- Periodontal disease Does not cause diabetes but complicate diabetes control. Especially in pregnant female.
- Bi-directional relation between pregnancy and periodontal disease ???
Rheumatic fever : repeated attacks of streptococcal infection to tonsils but wasn’t properly treated so it make vegetations on heart valves then patient treated by massive dose of penicillin and healed some how, but some roughness created on the valve.
So after that you made scaling for the patient for example and didn’t know If the patient having rheumatic fever, bacteria enter and get out in normal cases but here in the case of rheumatic fever, bacteria enter and make vegetation again and if you didn’t prescribe regimental antibiotic patient will die.
If this vegetation (bacteria/platelet/fibrin) was detached and circulate in blood will lead to septic embolism.
- Blood vessels has high tonicity
Injury to endothelium layer leads to fibrosis and clotting inside it.
This periodontal disease complicate the condition
Bi-directional relation between pregnancy and periodontal disease ???
We can prevent the increased secretion of prostaglandin by improving oral hygiene of this pregnant female and treat her in second trimester.
One session once she know she is pregnant and the second after 3 months and the third after giving birth.
Diabetologist refer to periodontist a diabetic patient taking massive dose of insulin and not getting better with high blood glucose level, and ask the periodontist if this patient having oral or periodontal disease ? So periodontist make diagnosis and found periodontal abcess.
Stress hormones are secreted with severe infection cases like (glucagon-adrenaline-cortisol) and they antagonize the effect of insulin (increase insulin resistance).
Treated by nerve block anesthesia then making drainage by lancet leading to dramatic improvement in glycemic control.
