AFFIDAVIT OF INC GRADE COMPLETION
I, MAT PERATER MACOTE, of legal age, single, Filipino citizen and a resident of Lumbo,
Valencia City, Bukidnon, Philippines, after having been duly sworn to in accordance with law depose and
say:
1. That I am presently the DEAN of School of Computer Studies of Philippine Countryville
College, Inc. at Panadtalan, Maramag, Bukidnon;
2. That Mr. Jonathan C. Villas is our college instructor under the School of Computer Studies
teaching ITPE 1 – Object Oriented Programming of the said school;
3. That Ms. Elizabeth N. Bonggo is a student of Philippine Countryville College taking up
Associate in Computer Technology under the School of Computer Studies of the said school;
4. That Ms. Elizabeth N. Bonggo submitted her requirements last April 5, 2021;
5. That Mr. Jonathan C. Villas accepted her requirements due to a valid reasons;
6. That Ms. Elizabeth N. Bonggo already complied all requirements in her subject stated above;
7. That I have signed the complied INC form of Ms. Elizabeth N. Bonggo due to a valid reasons.
That this Affidavit is executed by me for the purpose of attesting the true facts above stated and to
request Commission on Higher Education Region 10 to accept the INC grade completion of Ms. Elizabeth
N. Bonggo.
